If the nail is infected, you will need appropriate ointments and/or oral tablets to get rid of the infection. It will depend on whether it is a bacterial or a fungal infection. Either the entire nail (avulsion) or part of the nail (debridement) can be removed. This procedure is almost always painless. Your doctor will first place cloth adhesive tape on the normal skin around the infected nail. A urea ointment is then put directly on the nail surface and covered with plastic and tape.

Well most of the times its normal it's not harmful, but there can be some underlying causes also like endocarditis, raynard's disease, kidney disease, nail infection, fungal infection, it can be due to injury also.

Fungal infections may affect the toenails (called onychomycosis) or the skin of the toes and feet (tinea pedis or athlete's foot). Onychomycosis can cause thickening and discoloration of the toenail. Changes in the nail can also cause pain, especially with pressure on the nail while wearing shoes.

Subungual hematoma is the medical term for bleeding and bruising under the nail. This is usually the result of some kind of injury to the blood vessels under the nail bed. For example, blood vessels can break and leak blood into the area underneath the nail when a door slams on a finger or a heavy object crushes a toe.
