As per your description, you might get relief with biologics, however that is a costly treatment. In that case you can go to govt hospitals and ask for help from charitable trusts or pm funds or cm funds. There is one cheap drug available but its toxicity profile is high, so it had to be used very cautiously. I hope you find your way to pain free life.

Dear lybrate-user, your knee must be in bad condition of osteoarthritis. It will be better if you see a knee specialist near you and get x ray done. He might suggest you surgery (i am just assuming from your complaints). Start doing knee exercise. Static cycling is of good help. Initial few days it will be painful. But later your pain will subside.

You r b27 positive but do you have any stiffness in back. If not then good. My suggestion to you will be do daily back cervical hip exercise so they are flexible (see a physiotherapist doctor). In hla b27 exercise only helps to slow down the illness. Ayurvedic medicine I have no experience so can't comment, also I have come across no ayurvedic doctor claiming to cure you completely in hla b27
in case your condition worsens then see an orthopedic consultant near you to help you better.

mri gives the better of cartillage and is reliable, if not there is another method of diagnosis called joint fluid analysis. In this the fluid from joint will be drawn by a doctor from the effected part for getting accurate result.
