1) Do a clinical examination by your Doctor Neurologist and to confirm exact pathological cause please do few investigations like Carotid ultrasonography, ECHO, MRI, and Arteriography

This might be due to lack of blood circulation in the hands and we suggest to do contrast bath therapy ie. Immersing in cold/hot water in the interval of 3-5 minutes which will help you reduce the pain.
It must be due to pressure on one side of the body and due to your bad posture in lying. Pour hot water while taking bath which will help you to get relieved from pain.

Take tab calcium once daily in between meals and Vit D3 sachet once in a week .take good nutritious diet ,do yoga, exercises ,8 to 10 glasses of water, fruits,salads. And daily reduce 1 cigarette than your total daily consumption .get checked for ESR, CRP, RA.

It looks like you are anaemic. If you have back pain after you sit for long hours then it is due to your haemoglobin levels as it is important to check that. Anaemia always leads to the symptoms of being tired.

This is a normal problem faced by everyone. For increasing only muscle weight, you should be very careful about your food intake. Your carb intake should be low, processed carbs in form of packed food, white sugar and oil should be zero or minimal.
