Placental Insufficiency - What Causes It?

The placenta is a link between you and your baby during pregnancy. It helps your baby to grow normally by providing it with food and oxygen. When the placenta is not able to do its work properly, the condition is called placental insufficiency or dysfunction.

Placental insufficiency is a serious complication of pregnancy and can happen when the placenta does not develop properly or is damaged. If this condition develops, the baby doesn’t get the nutrition and oxygen it needs to grow and can develop complications like low birth weight, premature birth, and birth defects. The mother can also develop dangerous complications if it’s left undiagnosed. Thus placental insufficiency must be diagnosed as early into the pregnancy to avoid these complications.

Causes of placental insufficiency
Actually, placental insufficiency is a blood flow disorder which happens due to a reduction in the mother’s blood supply. It can be triggered by vascular disorders, medications, lifestyle and  the following:         

  1. Diabetes
  2. Chronic hypertension
  3. Blood clotting disorders
  4. Serious anemia
  5. Blood thinners
  6. Smoking
  7. Drug abuse 

Other causes of placental insufficiency are cases where the placenta doesn’t attach properly to the uterine wall or if the placenta breaks away from it.

Symptoms
Unfortunately, there are no symptoms of this dangerous condition in the mother except that she may notice that the size of her uterus is smaller than in previous pregnancies.  She may also notice that the fetus is moving less than before.  As far as symptoms of placental abruption or disruption go, there may be vaginal bleeding or pre-term labour contractions. In case placental insufficiency is not diagnosed, the mother can experience a serious fatal condition called Preeclampsia, which means high BP and protein in the urine as well as disruption of the placenta and pre-term birth. The picture for the baby is pretty dim too. The earlier placental insufficiency happens, the more grave the risk for the baby. The baby can suffer from:

  1. Cerebral palsy and learning disabilities
  2. Low body temperature 
  3. Low blood sugar 
  4. Premature birth
  5. Cesarean delivery
  6. Stillbirth
  7. Death

Placental insufficiency can’t be cured, but it can be managed. Once diagnosed, it can be managed through proper prenatal care like treating maternal high blood pressure or diabetes, education on preeclampsia and ways to self-monitor for the disease, frequent doctor visits, bed rest to conserve energy for the baby and consultation with a specialist. These can improve the baby’s chances of normal growth. 

Proper prenatal care of pregnant women is thus, very important as it can diagnose placental insufficiency early. A simple ultrasound can diagnose this dangerous condition by measuring the baby's size and growth, and assessing the size and placement of the placenta.

Traveling in good road is not dangerous. You can take progesterone pill till you travelling.    take hot saline gargles atleast 5 - 6 times a day, it will help to clear the throat .  steam inhalation also helps to clear respiratory passages.  sprinkling eucalyptus oil on the clothes and pillows while sleeping will help.  elevating the upper body and head by propping up with 2 - 3 pillows at night will help to relieve post nasal drip which worsens dry cough.  hot soup infused with ginger, tulsi, pepper should be taken.  avoid cold food. Eat fresh homemade steaming hot food. take lots of fluids - water, soups, dal ka paani  take decongested and anti allergic drug if cough is more.

Hi, I am 26 years old and I will complete 2 months of pregnancy. In test reports I found t3-total (tri iodothyronine) 1.22 ng/ml t4-total (thyroxine) 12.6 µg/dl tsh (thyroid stimulating hormone) 12.705 µiu/ml as per the test report tsh level is increased. I am taking medicine for thyroid (eltroxin 50 mcg). Please let me inform will it create any problem in my pregnancy. Before conceiving I was under medication of pcos. Please guide me. Hello, thanks for the query. The query says marle & 24 years old. Then talks about pregnancy. Please clarify about whom are you asking the question? Tsh 12.705 mu/l is too high. Thyroid functions should have been checked before getting pregnant by this lady. Because tsh should be maintained around 2.5 mu/l or less even before getting conception. Higher values are not good for fetal development and for outcome of pregnancy. Please consult the gynecologist and endocrinologist for reducing the tsh. )r else come back with following details: age, weight, height, lipid profile, bp readings as there is a need to initiate treatment with levothyroxine immediately. In this communication we are supposed to give only general guidelines, no drugs can be prescribed. I hope the seriousness of the situation is understood. Thanks.

Malaria is a disease that is caused by the bite of a certain mosquito or protozoan parasite. This can attack the red blood cells and is mainly found in tropical as well as subtropical areas. Until recently, there was little chance of recovering from this ailment. Yet, in today's day and age of great medical advancement, it has become possible to cure a patient suffering from this disease with the help of timely and proper diagnosis. This disease becomes even more complicated and problematic when it affects pregnant women.

The ways in which this disease can be treated during pregnancy is as follows:

  1. Medication therapy: If a woman is found to be suffering from this disease in the second or third trimester of their pregnancy, they may be prescribed artemisinin-based therapy. This therapy can be used only if the pregnant lady is not suffering from any other kind of complications like heart conditions, diabetes and high blood pressure, during the pregnancy. This medicine is short-acting, with artemether or artesunate, which can greatly reduce the number of parasites that may have invaded the red blood cells.
  2. Other antimalarials: The treatment of this disease for pregnant women can also be done with the help of other antimalarials, which should typically be prescribed and administered in a clinical setting. These also include quinidine gluconate which can be used in place of the IV quinine in order to bring down the number of parasites in the body. This will lessen the attack on the red blood cells to a great degree. Lumefantrine tablets may also be used in such cases. Also, the doctor will have to first try a test dose to ensure that the patient is not allergic to any of the elements or compounds of these medicines.
  3. Monitoring: Pregnant women suffering from malaria will need to get their blood samples tested on a regular basis. The blood smears will usually be tested daily to check whether the body is responding adequately to the treatment. This will also ensure that the doctor knows when to change the dose or the drug involved.
  4. Diet and exercise: The patient will be asked to stick to the normal diet and activity levels. Yet, care must be taken to avoid any situation where fatigue might set in, as this can aggravate the condition. One must also increase the intake of water to ensure that the body is always well hydrated. This helps the body in bouncing back to normal as the toxins are continuously flushed out, and the immunity system is better equipped to fight the disease-causing parasites.
