Things You Should Know About Spine Deformity

Spine deformity is a medical condition that refers to any type of abnormality in the shape, alignment, or formation of the vertebral column. The human spine, in its normal state, has a gentle curvature but when these curves become extreme or move out of place, it leads to deformities. Some deformities are mild in nature while some are serious. The former are not recognizable but the latter mark the personality of the individual and also cause physical problems. Spine deformity is also known as a misshapen spine or curvy spine and can affect anyone – child, adolescent, adult or old.

The signs and symptoms of spine deformity include low back pain, weakness, pain in legs or thighs, muscle spasms, breathing difficulty, uneven shoulders or hips or both, misalignment of the head, protruding shoulder blade, and poor gait.

  1. Scoliosis, Kyphosis, and Spondylolisthesis - Spinal deformity is mainly categorized into three conditions that commonly affects people: These spinal deformities include the following:
  2. Scoliosis – In this type of spinal deformity, the spine becomes curved from side to side forming an “S” or “C” shape. It mostly affects teenagers in their growing years but can also affect children and adults. It can be caused due to birth defects, genetic reasons, or neurological defects. It can be structural or nonstructural. The former is caused by disease, injury or birth defect and it is permanent in nature. The latter is temporary and can be treated with surgery.
  3. Kyphosis – Also known as a hunchback, kyphosis is a condition marked by excessive curvature in the upper part of the spine. This is a very visible condition and the person has a hump on their upper back. It can affect anyone and is generally known to result because of poor posture but an infection in the spine, injury, slipped disc, muscle weakness, arthritis, and aging can also trigger kyphosis.
  4. Spondylolisthesis – In this condition, a bone in the spine moves out of position, either forward or backward, causing a deformity in the spine. It is caused by aging, arthritis, or general degeneration caused by wear and tear.

Treatment: Spinal deformity, in most of the cases, is treatable if it is detected on time. In cases of kyphosis or where the patient is experiencing symptoms such as pain or other tangible signs, physical examination by an orthopedist is necessary. Upon further screening through an X-ray or MRI scan, the degree of deformity is determined and then the doctor starts treatment. Some cases can be treated through braces or physical therapy or exercise. They correct the abnormal curvature of the spine and the individual can get a normal curvature after the treatment but in some cases, surgery is the only option left as only surgical intervention can rectify the deformed curvature. The surgical techniques include spinal fusion and insertion of implants.

Prevention: Spinal deformity is painful as well as personality diminishing condition and hence, needs timely treatment but it can be prevented as well. For it to happen, causes such as poor posture, arthritis, and slip disc need to be addressed in time. Osteoporosis is a condition wherein the bones become brittle and weak; so much so that even mildly stressful activities such as coughing, bending over or even a slight fall (such as the one from a high rise chair) can result in fractures. Osteoporosis-related fractures commonly occur in the spine, hip or the wrist. The human body has a continuous mechanism of bone absorption and removal. In case of osteoporosis, the creation of new bones doesn’t happen in accordance with the removal of old bones.

Spinal Fractures:

The bones of the spine get extremely vulnerable to breakage and even cracking open. The fractures in the spine, also known as vertebral compression fractures can cause a sharp stinging pain in the back that may make sitting, standing, or even walking a very tardy task.

Risk Factors:

  1. Increasing age

  2. Abnormally less or high body weight

  3. Smoking

  4. Menopause or low levels of sex hormones

  5. Gender: This disorder is more likely to affect women as compared to men. Also women, who are above 50 are more likely to suffer from this debilitating disorder.

  6. Race is a significant risk factor of osteoporosis. If you are of Asian descent, you are more likely to be affected by it.

Symptoms:

Diagnosis and Treatment:

Firstly, an X-ray or a computerized tomography (CT scan) will be done to have a closer look at the bones. A bone density test is another commonly used method of diagnosing osteoporosis.

  1. Steroids and medications: Some medications may be used to prevent or combat osteoporosis. These include alendronate, ibandronate, risedronate and zoledronic acid.

  2. Physical therapy: Just like muscles, bones get stronger too when you exercise. Weight-bearing and muscle- strengthening exercises are the most helpful in this regard and are considered best for the treatment of osteoporosis. Cardiovascular exercises such as walking, jogging or even swimming can prove to be immensely beneficial.

  3. Diet: Make a diet chart that includes high-calcium food items, dairy products such as yogurt, cheese, and low-fat milk, tofu, green vegetables such as collard greens and broccoli, sea fish such as salmon and sardines.

  4. Salt: Limit salt intake

A few decades back, spinal tuberculosis was a dreaded disease. It is not so, anymore. With advancements in the medicinal field, when diagnosed in the early stages, a complete cure is possible.

Tuberculosis of the spine is one of the most common spine ailments in our country. Though there was no treatment when the disease was first reported, recent improvements in medical treatment and surgical procedures have made the condition treatable. In many cases, the disease is cured completely.

Molecular genetic techniques, stronger antibiotics and other medicinal discoveries and inventions have been able to combat the condition, which is resistant to a huge number of drugs. There are many surgical procedures as well, which has changed the way spine TB is looked at. How is it caused?
Tuberculosis affects people living in overcrowded, dirty and poorly nourished environments. It grows only intermittently or remains dormant for a long period to then re-grow whenever the immunity system of the host weakens.

What are the different types of TB spine?
Spinal tuberculosis takes two forms. In the first case, the symptoms are mild. The infection starts in the anterior part of the disc and slowly spreads to the nearby surface of the body. The neural arches are very rarely involved here. This results in a vertebral collapse. The spine bends forward to produce kyphus. In the second type, several vertebrae are involved, but the disc space might not become narrow.

What are the symptoms of TB spine?
The first symptom is an excessive pain in the back, which manifests as kyphosis increases. Kyphosis is the condition wherein there is an increase in the normal convex curve of the spine. Later, pus forms at the infected vertebrae and this comes down to the tissue planes and forms a cold abscess in uncommon places. This happens in the groin region, and the person may become paraplegic.

In a child, spinal tuberculosis will cause them to constantly complain about back pains. They will not feel well for a long period and will gradually lose weight. They may not want to sit for a long time and resent any physical activity. The child will be vulnerable in the low thoracic or upper lumbar spine and can show any of the above-mentioned signs that are seen in adults.

The most common symptoms of spinal tuberculosis are kyphus, reduction in the movement of the lumbar spine, cold abscesses in the paraspinal area, groin and loin, and sinuses. The doctor always tests the reflexes in the patient’s legs, sensation, power, and tone before diagnosing this condition.

Though a rare disease, tuberculosis in the spine can occur in people from both developed and undeveloped areas. Proper diagnosis and treatment of tuberculosis with or without surgery at the right time is the key to cure the disease.
