What r other symptoms that you r experiencing?
By doing what like walking, exertion, eating or empty stomach- it increases or decreases in intensity. For how much time the pain remains?

Dear User, it looks like you have a localized gum infection which could be due to an untreated Gingivitis. You will have to start antibiotics to prevent the infection. Dear user, that is decided based on the type of swelling. It could be a simple goitre or a malignant condition which would require a thyroidectomy which are of many types. This is decided by the surgeon after a proper diagnosis is made with all the required tests done. However the surgery will be done inly if needed and you will have to maintain your thyroid levels at normal for sometime.
