It is not only your hygiene which matters here, oral hygiene of her is more important. If she is not diabetic, she is on lesser risk of infections than you as diabetics especially uncontrolled diabetics are on higher risk of infection. So keep your blood glucose good control. Ensure there is no cut or bruise on your penis which can be portal of entry for infection and last but not the least good oral hygiene of her to be ensured.

there are various reasons of bad breath, like impurities in blood, inefficient digestive system, dry mouth (lack of saliva production), gum disease, cavities, plaque and tartar formation around teeth and gums, kind of diet, improper brushing / cleaning of mouth etc. I would advise you to get a thorough physical and dental examination to rule out the cause of bad breath and to solve it permanently.
