peyronie's disease is characterized by an extreme curving of the erect penis caused by a hard, fibrous layer of scar tissue (plaque) that has developed under the skin. It never treated without surgery.

But does it problematic then answer is it should be or should not be.

If you have skill how to sex with phimosis then it will never problematic and skill will be gain by doing repeated sex only.
