Dear lybrate-user,
as you have developed symptomatic gouty arthritis with high uric acid levels it is likely that you have a genetic tendency for high uric acid levels.
You need to avoid nuts, wine and red meat and drink 3-4 litres of water every day. If in spite of dietary modification the problem persists you may have to adjust the dose of febutaz to optimize uric acid levels.

Hello, Thanks for the query.
I have seen the details given. I do not know from where you have gathered the information about Telmisartnan causing "kidney damage". In fact these group of drugs (ARBs) and ACE- Is are considered safest of blood pressure reducing drugs. Since your BP is well controlled, there is no reason to reduce the dosage. Telmisartan is shown to reduce proteinuria and help improve Glomerular Filtration Rate (GFR), which an indication of kidney function. Thanks.

Acute rheumatic fever can be treated, but it leaves behind many sequelae including heart diseases. Has a doctor diagnosed you with rheumatic fever. It needs a panel of tests to diagnose. Similarly goes for rheumatoid arthritis.
I would need to ask further history to assess the magnitude of the problem and to give suggestions accurately. Consult me, privately.
Recommendations:
1) hot water fomentation and diclofenac 100 mg for back pain.
2) warm saline gargles.
