Asked for male, 27 years old from Dibrugarh

My father age 62, non diabetic and no bp, but from dec 2022 he has hypothyroidism and his bp was 190/100, symptoms was swelling face legs hands. Dr. Prescribed thyronorm 100 and telmikind h. After 6 weeks his symptoms was not reduced. Then we go for another lab test where his tsh reduced from 40 to 12, but creatinine level jumped 1.3 to 3.02, next day 4.11 and then 5.22. Serum albumin is below range. Urine albumin ++++. He was admitted to hospital and got 3 bottles of albumin. Medicine prescribed- nefrosave keto, atorvastatin 40, clonodil 100 mg, iron-calcium tablets, lasix 40 tab. Taking this medicine from 1st feb. Steroids was not prescribed. Recent reports (20/02/2023) is creatinine 5.02. Albumin is 2.0 total protein is 4.4. Usg finding- bilateral pleural mild effusion and ascites. Bilateral early nephropathy. Please suggest medicine for protect his kidney.

I had total thyroidectomy back in 2017 and after that I take 225 mg thyronorm. I repeat blood test in 3 months or 4 months of thyroid some time report of thyroid comes in normal range and some time tsh high in december it was 35 and in july it was normal. Sometimes I misses the doses also.

Hello, thanks for the query.
I have seen the details mentioned. Please understand with thyroidectomy, you will have to be on adequate dosage of levothyroxine so that tsh is maintained around 3 to 4 mu/l. Missing the dosage is not good. Plus you need to keep checking the thyroid profile at least once I 3 to 4 months so as to know the exact situation. If you have any other queries please do come back. From 4-5 year I have 24 hour full body pain and fatigue and all tests are normal. 4-5 months ago aiims doctor diagnosed fibromyalgia. 1 month ago I have an hypertension emergency and I admitted in aiims and there doctor tell that my left ventricular ef is 25%.my treatment in cardio is going on. My next appointment in cardio is after 15 days. But I want to know that I have fibro and heart failure both or my fibromyalgia was a misdiagnosis. If 25% lvef cause 24 hour body pain and fatigue and is it possible that I have heart failure from 4-5 year but doctor can't diagnosed that because no one do my echo or ecg before hypertension emergency.

every time I test tsh it's 6.5 and t3 t4 are normal. I consulted endocrinologist she suggested me to start 25 mcg thyronorm. During pregnancy it was changed to 50 mcg. Dint take thyronorm for 4 months after delivery. Now I tested again tsh is 6.55 and t3 t4 are normal. What should I do?

Madam I have seen the details. The tsh levels mentioned are definitely higher than what should be normally seen at your age. This is suggestive of sub-clinical hypothyroidism. In many cases of sub-clinical hypothyroidism no treatment is advised. Hi, my gynecologist prescribed 25 mg thyroid medication at the time of my pregnancy. After 3 months from delivery, I stopped that medication. I have seen the question. Madam let clarify for hypothyroidism (if well confirmed with high tsh) only medication is to take adequate dosage of levothyroxine. There is no medicine in any other alternative branch or therapy. This is because hypothyroidism occurs due to insufficient or some times nill production (synthesis) of thyroxine (t4) by our thyroid gland. I have seen the details mentioned. From the given information I am unable to understand what is the current situation. Because only the dosage of tablet (thyronorm 50) is mentioned. Nothing about patient's t3, t4 & tsh levels as well as weight, height or any other details like her age is mentioned.
