As your kidneys are not able to remove urea from the blood normally, your BUN level rises. Heart failure, dehydration, or a diet high in protein can also make your BUN level higher. Liver disease or damage can lower your BUN level. A low BUN level can occur normally in the second or third trimester of pregnancy.

The normal BUN level is between about 7 and 21 milligrams per deciliter (mg/dL). Normal levels of creatinine in the blood are approximately 0.6 to 1.2 milligrams (mg) per deciliter (dL) in adult males. A better measure is the ratio of BUN to creatinine found in your blood. Typically, the ratio of BUN to creatinine should be between 10: 1 and 20: 1. If it's lower or higher than that, it may mean you have a problem with your kidneys or you may not be drinking enough water. In your case it’s 23: 1<...more

Creatinine is high
what is the cause for that should be evaluated first.
There is no hurry in iniating ATT.
But of the patient is so sick of TB, then ATT should be initiated immediately. In such a case Ethambutol should be given on alternate day with complete dose. So it would be better to give ATT basing on an alternate day regime. Once creatinine becomes normal switch over to daily regime.
