Extra sound may have multiple reasons and as you mentioned hole is the most common cause. Treatment depends upon the size and site of the defect, is small can be closed with minor procedure otherwise surgery may be needed.

Radial route is better as it leads to less complications and less hours in hospital and less aftercare with minimum risk of infection or bleeding from site. Regarding constipation, take the person clean by enema in the morning & then take to ot, give some soft nourishment as ga is not required.

Radial requires skill and is less traumatic compared to femoral which too requires skill but it has been there for a lot more years, the side effects of femoral are 1) bleeding through a malor vessel, there fore the leg has to be kept at a higher level with head side low for at least 6 hours & post procedure care takes longer to heal though in both the vessels it is just a puncture caused by a thick needle to inject dye to reach the heart vessels.
