Tacrolimus drug level testing is done just before the next dose ie trough that is usually in the morning hour before taking the tablet of 1.5. Testing should be repeated according to the timing post transplant. In the first year, repeated testing is to be done, in the later years, testing is done only after dose modification or any clinical event. Yes please continue both clopidogrel and ascard 75 mg. These are blood thinners required lifelong after cabg surgery. If discontinued then the risk of graft blockage is high.
