Hernia occures due to weakness of the anterior abdominal wall. Post op one should avoid weight lifting or the chances of recurrance are very strong. Please don't do work up in the gym lest you can do walking and light aerobic exercises. Regards.

Large size hernia in scrotal sac means grade 3 inguinal hernia. It needs operative repair of the inguinal ring with reinforcement procedure called hernioplasty. If not done in time , the hernial loop mau get obstructed or even strangulated, creating an emergency. It is always better to get operated as a cold case instead of in an emergency.

What is the size of abdominal scar. What is the size of hernial protrusion, reducible or not, etc are things which will decide the needful action. Small hernia with small swelling, reducible with slight pressure can be controlled by pressure belts around abdomen. Anything which increases abdominal pressure like coughing, constipation, weight lifting must be avoided. Advanced gall bladder cancers can cause liver dysfunction jaundice and poor digestion. Since he seems to be terminal and not fit for chemotherapy. You can provide best supportive care, means treat symptoms. For vomiting you can give a good antiemetics, antacids, pain killers. Etc. There are organization which provide hospice or terminal care at home at economical cost. In liquid diet you can add some nutritional supplements to provide extra energy.
