Mild pleural effusion can not be detected in xray chest.Need ultrasound chest. May be just pleural thickening as effusion should not persist for 4 years

Pleural effusion from TB are normally non contagious as long as there is no pulmonary TB. It is still recommended to maintain caution till 2 weeks into ATT.

If there is a liver abscess the pleural effusion is likely to be secondary to that. Until the infection there resolves fluid drainage can continue. It can take many days. The drain needs to be there until the infection is cleared.
