Hello, the pigmentation are of two types superficial and deep. The superficial pigmentation respond to creams. The deeper ones need chemical peels and lasers and additional depigmentation creams. You can start initially on milder depigmentation creams which contain kojic acid, arbutin, niacinamide like Melaglow at night and use sunscreen (SPF 35 and above) three times a day. Your query makes me believe that your faulty tooth brushing technique. The same reason caused Class IV cavity and now due to same reason the filling is come out.
I usually ask my patients to mend their brushing habits before I venture into tooth filling. It’s you not your dentist at fault.

To help remove a skin tag, apply 6 to 8 drops of a high-quality tea tree oil to a sterile cotton ball and secure to the skin with a bandage. Remove after 15 minutes. Do this three times a day until the skin tag falls off

Use Glycolic acid facewash - Ahaglow. Use a sunscreen in morning and afternoon which is dry and non oily (Facegaurd). Add a very good moisurizer at night like (venusia cream at night) along with kojitin cream (anti pigmentation) once daily at night. If you are sincere, yiu will get a response in 1 month.
