ERA indicates the window of implantation (WOI), increasing your chances of successful transferThe endometrial receptivity is the status in which the endometrium is ready for embryo implantation to take place. This occurs around days 19-21 in each menstrual cycle of a fertile woman. Until now, the only study done on the endometrium was an ultrasound scan, and there was not analysis method available to help doctors in their clinical practice.

Now, during the process to treat an infertile couple, the ERA test (Endometrial Receptivity Array) leads to the evaluation, at molecular level, of the endometrial factor. This molecular tool allows us to diagnose whether the endometrium is receptive or not by analysing the expression of a group of genes related to endometrial receptivity. For that, an endometrial biopsy must be performed at P+5 (hormone replacement therapy cycle) or at LH+7 (natural cycle), and after its shipment to our installation, the expression of 238 genes is analyzed. According with these expression values, the endometrium is classified as receptive or non receptive by a computational predictor.

If a couple is unable to conceive naturally despite having unprotected sexual intercourse for over a year, they may be diagnosed with infertility. IVF is one of the most common ways of dealing with infertility. This is a process where eggs from the female and sperm cells from the male are harvested and combined in a laboratory to form embryos. The embryos are then reinserted into the woman’s uterus. This is a crucial step of the IVF procedure. In order for the treatment to be successful, at least one embryo must attach itself to the endometrial lining of the uterus. This allows the embryo to receive nutrition from the mother. If the embryo is not attached successfully, it will be expelled from the body. In such cases, the woman is said to have an unreceptive uterus.

One of the ways to check the receptivity of the endometrial tissue is through an Endometrial Receptivity Array (ERA) test. This involves taking a biopsy sample of the endometrial tissue on a particular day of the woman’s menstrual cycle and sequencing the genes of the sample. Collecting the sample takes hardly any time but processing it can take up to 3 weeks. This is akin to a personalized genetic test. The results of the Endometrial Receptivity Array test help to determine the ideal time during the menstrual cycle when the embryos should be implanted.

This test is not always recommended for IVF patients. It is usually advised only if the woman is over the age of 37 years and the couple has had 2 or more failed IVF cycles. It may also be advised if the woman has healthy eggs but is still unable to conceive. This is a fairly simple procedure which may be performed in the doctor’s office. It is not painful and there are no serious risks associated with it.

Though this procedure can make IVF a little more expensive, there are a number of benefits associated with it. For starters, it helps reduce the risk of IVF failure and increases the chances of having a successful pregnancy. This helps save time by increasing the probability of a successful implantation in the first cycle of IVF itself. In the long run, it also helps save money. This is because each IVF cycle has a cost associated with it. Hence, undergoing an ERA once and finding the perfect window for implantation can negate the need for further IVF treatment.

What is ECMO?
Like dialysis for unfunctional kidney, Ecmo for unfunctional lung.

Ecmo stands for extracorporeal membrane oxygenation. It is a method of giving oxygen for the body when icu pateint lungs and/or heart are not able to supply oxygen on their own. 

Why ICU pateint put on ECMO?

Doctors place ICU patients on ECMO when patients are not able to supply oxygen to the body.

ICU patients’ lungs fail for a number of reasons including pneumonia, lung cancer, pulmonary edema, pulmonary embolism and COPD.

 When a patient’s lungs fail, he/she first is intubated (breathing tube) and hooked up to a ventilator (breathing machine).
 However, sometimes lungs are so damaged that providing oxygen through intubation is not enough.

This is when doctors turn to v-v ecmo.
A heart can fail for many reasons including heart attack, pulmonary embolism, bad valve disease, or worsening heart failure. When a heart fails, doctors try to fix the underlying problem. They may also start medications (called ionotropes) to help improve the pump function of the heart. If medications are not enough, doctors will turn to v-a ecmo.

How long can someone stay on ecmo?

That is a complicated question. Due to the risks of ecmo discussed above, doctors try to keep patients on ecmo for as short a time as possible. Often patient will be on ecmo for several days up to 1-2 weeks. Every day, several blood and imaging tests are done to determine if a patient is ready to come off ecmo. As the technology of ecmo improves, hopefully side effects will decrease and patients can remain on ecmo for longer periods of time.

What is the difference between ecmo and a ventilator (breathing machine)?

Both ecmo and a ventilator aim to provide oxygen to the body when the patient’s own lungs and breathing are failing. The ventilator assists the patient’s own lungs by pushing oxygen with pressure into the lungs. Ecmo instead provides oxygen directly via a catheter placed in a patient’s vein or artery. We almost always try oxygenating a patient with a ventilator first. However, when a patient’s lungs are too sick for this, we turn to ecmo to assist in providing oxygen to the body. V-v ecmo provides oxygen through a vein. This blood then has to travel to the heart and be pumped around the rest of the body through arteries. Therefore, with v-v ecmo or with a ventilator, a patient must have a well-functioning heart to get the oxygen pumped throughout the body. V-a ecmo has the additional advantage of pumping blood directly to arteries. This “by-passes” the heart and is therefore the method of ecmo we use when a patient’s heart is failing.

Cancer that originates in the uterus is termed as Endometrial Cancers. The cancer is known to originate in the endometrium i.e. lining as well as the cells of the uterus. Abnormal vaginal bleeding causes the cancer to be detected early and a consultation with the doctor should be done immediately. When other cancers originating from the uterus are considered, endometrial cancer is found to be the most common.

Causes: The exact cause for endometrial cancer is not known clearly. The cells in the endometrium tend to become abnormal due to the occurrence of a genetic mutation, which causes rapid multiplication of cells that ultimately lead to the formation of a tumour like mass. They begin to spread to other parts and proceed to affect the tissues in the surrounding areas.

Symptoms:
The typical symptoms of endometrial cancer include:

  1. 1. Irregular bleeding in between periods
  2. 2. A watery discharge from the vagina
  3. 3. Bleeding from the vagina postmenopausal
  4. 4. Pain in the pelvic floor muscles

Risk factors that might increase the chances of being affected by endometrial cancer include:

  1. 1. Aging increases the chances of this disease, especially post menopause.
  2. 2. Obesity can cause hormonal imbalance.
  3. 3. Certain drugs used in hormone therapy
  4. 4. Early onset of menstruation, especially before 12 years of age

Treatment for Endometrial Cancer Includes:

  1. Radiation therapy: This treatment procedure involves using radiations, such as X-rays to target and destroy the cancer cells. The two forms of radiation therapy are external radiation therapy (external machine is used to direct radiation on the cancer cells) and internal radiation therapy (radioactive substance enclosed within small and consumable items is placed near to or directly at the spot of cancer).
  2. Hormone therapy: Medications are administered to eliminate imbalances in hormone levels and prohibit the growth of cancer cells in the body. This remedy is preferred if the cancer progresses to an advanced stage.
  3. Chemotherapy: This form of treatment uses oral administration or injection of chemicals into the veins so that the growth of cancerous cells is curbed, either by elimination of the cells or by prohibiting cell division.
  4. Surgery: In this surgery, the areas affected by cancer are surgically removed; i.e. the uterus in this case. The surgical procedures that might be used include total hysterectomy (total removal of uterus), radical hysterectomy (removal of the cervix, uterus and part of the vagina) and Bilateral salpingo-oophorectomy (removal of both the ovaries and the fallopian tubes). There is no real female viagra and if anything is used as female viagra it is not safe before sex. Flibanserin which was tired as female viagra was banned due to its uselessness in increasing sexual desire and its side effects

What Causes Urinary Tract Infection?
Kidney Tract Infection is a form of UTI (Urine Tract Infection) that generally begins in Urethra or Bladder and spreads to one or both of the Kidneys. This Kidney infection is caused by bacteria which enters the Urethra and multiplies in the Bladder, then infection spreads to Kidneys.

  1. Endometrial scratching optimises the uterine lining and thereby, makes it easier to continue the embryo implanting procedure. Normally, before the endometrial scratching procedure, the gynecologist may suggest endometrial biopsy. 
  2. The endometrial procedure is usually suggested for those women who had two or more unsuccessful IVF procedures, despite having good quality embryos. However, in some cases, depending on the health of the patient and various other factors, the gynecologist may suggest this endometrial scratching at the very first IVF procedure. This procedure can be availed either for fresh embryos of the couple or frozen embryos or for the donor egg. 
  3. Although this is a part of a surgical procedure; endometrial scratching is very simple and painless and it is done without administering anesthesia. The procedure does not require any hospitalisation. In fact, the patient can go home soon after the procedure. Except for a few pain killers, this procedure does not require any extensive medication. 

This is a safe and easy procedure. Neither does it lead to any side effects, nor is the procedure burdensome for your wallet. Many couples have been benefited by this endometrial scratching procedure.
