You need a surgery for hole in eardrum with recurrent infection. You can confirm your hearing level with help of a test called audiometry. You are young person you should not carry on such problem for prolonged period. In rare types of ear drum hole a nonsurgical procedure called chemical cautery can be tried but if it works for you that has to be decided by your ent surgeon. Success rate of such procedure is around 50% so the best option is surgery only.

Stop all the medicines that you are taking. There could have been a traumatic perforation of the ear drum or the nerves in the inner ear might have got temporarily damaged.
Get an ent consultation done to rule out if there is any perforation of the ear drum. Do not allow any water to enter the ear. Put cotton inside the ear & keep the ear dry for about 15 days. The ringing sensation should go on its own.

First we have to determine the cause for your tinnitus. It may be pathological or simply physiological. If we cannot find the cause for your tinnitus then we have to make you habituated to it l, so that you will feel no distress or anxious about it. Remember tinnitus is one in thousands of small minute sound produce in various parts of our body with magnified due to its close proximity to auditory system. Try ear drop erase wax 4 drops in both ear 4 times a day.
If any wax impacted, it will improve the pain.
