as you said TCC already applied and got relived. That was the first step of medical Mgt of Charcot foot. As its a complex phenomenon and is due to underlying neuropathy as well as osteoarhropathy. One should be careful while treating it. Please do have your Xray followed by MRI. Management is based on medical as well as surgical one. Some times peripheral neuropathy can occur without diabetes or any other disease. This can be due to certain vitamins deficiency, like B1, B6, B12, Folic acid etc. So get your vitamin estimation done or take supplements. Thanks.

Absolutely, smoking will harm the nerves too and he can aggravate the neuropathy. Quitting alcoholism is a tremendous achievement but he has to give up the smoking too. Get him to an addiction counselor and that help may resolve it for him.
