One need not see the opening to guide inside.
While not having sex just see lower part of wife to observe three different openings- 1) small through which urine passes 2) vagina- where you have intercourse and 3) anus- stool comes out from.
At that time put a finger in vagina and let it slowly go inside for you to get idea about direction etc.

thanks for your query. At 45 if you already hve kids, then you can continue with barrier protection methods like using a condom. The next sure method is vasectomy in which part of your spermatic cord is cut and tied a knot so that the sperm doesn't pass through the urethra. You need to practice caution (using a condom during intercourse) for a minimum 2 months post vasectomy as well. The chances are high especially if you had unprotected intercourse on 27th - 29th Nov as your fertile time is between 29-1st Dec.

Temporary or permanant?
There are many temporary family planning methods like oral pills, injectable hormones, intrauterine devices, condoms, diaphragms, safe period, coitus interruptus, etc. You must know about the success and failure rates of all as well as the advantages and disadvantages of all to decide which suits you the best. Maybe consult gynecologist as a couple.

Any is ok for family planning.
In laproscopy (button hole) may be little early discharge from hospital and may be less painful on 2nd and 3 rd day after operation but also theoritically extra risk.
Not much difference between two.
