The best option is to start physiotherapy treatment for posture correction and pain relief.

Well as your are 17 years old your bones are still somewhat growing what we call diaphysis so you can work on strengthening infraspinatua and mostly lattisimus dorsi strength push against wall slowly with 12 times and repeat in 3 sets do it for a month any queries you can consult in private section.
