When an expecting mother experiences uterine contractions and is getting ready to deliver the baby at 39 to 40 weeks, it is termed as a full-term labor. But due to several factors that can be caused by lifestyle or medical condition, the onset of labor might occur before 37 weeks of pregnancy which is termed as a preterm labor and could lead to an early birth which is not the best possible scenario for the baby. Hence, it is important to understand the factors contributing to the condition and take necessary precautions during pregnancy under the advice of your gynecologist.

Signs of Preterm labor
The typical signs of going into labor such as increased vaginal leak, cramps, and contractions are applicable in the case of early onset of labor as well. Along with this, lower back pain and pressure in the pelvic region might also be felt. Though these are all common during pregnancy and even may be closely related to Braxton Hicks contractions or false labor, it is better to report to your doctor immediately.

Diagnosing a preterm labor
During your routine prenatal checkups, if the doctor observes any change in the cervix or when you notify experiencing the symptoms of preterm labor, an initial pelvic exam will be carried out to check for dilation. This process will be repeated, and your contractions will be monitored. The doctor also would constantly monitor the baby’s heartbeat or rate. Further, the presence of a protein that is linked to preterm labor will be tested to take due care. Based on the findings, the doctor will decide whether you require immediate hospitalization or any other special care.

Should you be worried?
Preterm labor may not always be a cause for concern but in some cases, it may result in premature delivery of the baby. This premature birth can be harmful to both the baby and the mother. Further, if the baby is born premature, then the baby might be underweight, suffer from breathing problems as his or her organs are not adequately developed or face a high risk of infections as the immune system is not well developed. In severe cases, the baby might have long-term developmental disorders as well as behavioral issues.

Though there are definitely risks and complications associated with preterm birth, babies born after 32 weeks of pregnancy have a high rate of survival and in most cases, do not present any long-term complications as well. But it cannot be said the same for the infants born in or before 24 weeks.In such cases, doctors would try to delay the birthing as much as possible for the benefit of the baby. The best way to prevent preterm labor is to have a healthy lifestyle coupled with regular checkups during your pregnancy and act as per doctors’ advice.

  3. Endometriosis: This is a condition where the uterine tissue grows outside the walls of the uterus. This occurs commonly on the ovaries and fallopian tubes but can also develop on the cervix, bladder, bowel or rectum. The exact cause of endometriosis is unknown. Some theories suggest that it is triggered by retrograde menstruation or the backward flow of menstrual blood into the fallopian tubes. The symptoms of this condition include abdominal cramps, pain during intercourse and bowel movements, bleeding between periods and digestive problems.
  4. Poly Cyst Ovarian Disease (PCOD): PCOD is a condition where the ovaries produce follicles that develop into cysts instead of eggs. This leads to a lack of viable eggs and thus affects a woman’s fertility. This can also cause mood swings and depression. PCOD is triggered by hormone changes and exhibits symptoms such as irregular periods, acne, hair loss and breathing problems while sleeping.
  5. Urinary Tract Infection (UTI): UTIs are usually caused by bacterial infections and can be triggered by pregnancy, frequent intercourse, diabetes and as a side effect of certain types of medication. The symptoms of this condition include stinging and burning sensations while urinating, abdominal cramps, painful intercourse and an increased urge to urinate frequently.

A menstrual cramp is something which every woman complains of for a day or two every month. Menstrual cramps are usually the pains felt in the lower abdomen which can occur both before and during a woman's menstrual period. These cramps are caused by a hormone called prostaglandin that causes the uterine muscles to contract during menstruation. When the uterus contracts to shed its natural lining during your period it releases a hormone called prostaglandins which causes pain and inflammation. So higher the level of prostaglandins more severe the menstrual cramps.

It might be just a minor discomfort for some, but for others it can be severe. The symptoms of menstrual cramps include pain in the abdomen, pressure in the abdomen and pain in the hips, lower back, and inner thighs as well. However, if the cramps are severe you might have an upset stomach, loose stools and vomiting too. Three out of four women experience menstrual pains and every one out of 10 women experiences severe cramps.

Here are few remedies that might help in relieving the pain:

  1. Exercise, brisk walking or any type of physical activity can help to ease your belly pain as by being physically active your body pumps more blood, which helps to release endorphins to counteract the prostaglandins and reduce your cramps.

Dilation and curettage procedure which is commonly referred to as D&C is a minor surgical procedure where the cervix is dilated while a special instrument is used for scraping out the lining of the uterus. It is important to know what you can expect before, after and during the process so that you can stay ahead of unnecessary worries and help the process to be smooth and fruitful.

When do doctors recommend dilation and curettage process?
You may be required to undergo the dilation and curettage procedure for one of many reasons. It can be used for removal of tissues in the uterus during or after an abortion or miscarriage or to remove little pieces of placenta after delivery. This process aids in preventing infection as well as heavy bleeding. On the other hand, it can help in diagnosing and treating abnormal uterine bleeding including polyps, fibroids, hormonal imbalances and even uterine cancer. A sample of the tissues in the uterus is tested under a microscope to check if there is any abnormal cell present.

What can you expect during the dilation and curettage process?

Curettage involves scraping of the lining and removal of the uterine contents with the help of a spoon-like instrument known as a curette. This may cause some amount of cramping, and a tissue sample would be taken out for examination in the laboratory.

After the completion of the procedure, you may experience slight bleeding and cramping. In some rare cases, adhesions or scar tissues may start forming inside the uterus, and this condition is termed as Asherman’s syndrome which can cause changes in the menstrual cycle along with infertility.
