Get your semen analysis done. If semen is normal and you don't have any pain or discomfort because of varicocele, you may leave it alone. All types of varicoceles do not need surgery. beware! let me clear two acts for you.
1) masturbation is the main cause behind varicocele in young adults, but since you ave consulted a allopathic surgeon, he will not accept masturbation as its cause. 2) surgery is not an ideal treatment for varicocele as it has more than 70% chances of recurrence within 6-8 months. Cutting an affected part will not help, as the tendency to form a varicocele is still there.

Varicoceles present as soft lumps above the testicle, usually on the left side of the scrotum. Right-sided and bilateral varicoceles may also occur. Patients may sometimes complain of pain or heaviness in the scrotum. Often they are found in the course of an infertility workup.

A non-communicating hydrocele usually remains the same size or has a very slow growth. If a communicating hydrocele does not go away on its own and is not treated, it can lead to an inguinal hernia. A hydrocele can develop as a result of injury or inflammation within the scrotum. Inflammation might be caused by an infection in the testicle or in the small, coiled tube at the back of each testicle (epididymitis).

Hydroceles and varicoceles are types of testicular lumps and swellings involving the male scrotum. However they differ because hydrocele is a swelling caused by fluid around the testicle, whereas varicocele is a swelling caused by dilated or enlarged veins within the testicles

Hello regarding you Bp. Telmisartan 20mg is a suboptimal dose... and if you are maintaining Bp below 130 by 90 without Medicinie I would suggest you to give a trial of off medication, along with dietry and life style modifications.
