Hi lybrate-user. Quite likely, as you have written that transplant has happened 9 years back, it would be an irreversible disease progression. But it would be important to know about the previous values of creatinine and any renal biopsy. If there are no acute events leading to recent deterioration of kidney function then quite likely the options before him would be dialysis or transplant.

Hello...
BK virus can cause renal dysfunction in post transplant patients. However, from your biopsy finding it is not very clear whther those findings are due to the virus or the Immunosuppressant medications which you are taking. The best approach would be to taper your immunosuppressant medications and regularly monitor the bk virus level to see for response

Appears kidney cancer with spread to lung. PET/CT scan will show all the body parts affected with it, so that it can be analysed and managed better. Surgery is not required when there is already metastasis (spread). Oral medications are available to keep a control over the growth of tumor so that life can be extended. Please consult a qualified medical oncologist near by for further analysis and prescription.

He would need a CT thorax to confirm the size and number of metastasis. Also a bone scan would be advisable if bone pain or raised alkaline phosphate level in blood. PET CT scan can be done instead of both the above investigations. MRI brain if symptomatic.
