If you have not heard, the reshape integrated dual balloon system has recently been approved by the fda. The reshape is the first non-surgical weight loss procedure for people with mild-to-moderate obesity.
Feel uneasy about the idea of having balloons in your belly? rest assured, balloons have been used in medicine--specifically heart and cardiovascular procedures--for decades.

The reshape is a dual balloon mounted on a flexible shaft that is inserted endoscopically in the stomach and filled with saline. It works by restricting food intake and slowing gastric emptying. Insertion is done during a 30-minute outpatient procedure and the balloon remains in the stomach for six months where it mimics the natural curvature of the stomach to enhance tolerability.
In the reduce clinical trials (n=326) looking at the reshape, the fda determined that the device was safe and effective based on a clinical trial involving 326 obese participants aged 22 to 60 years who had at least one obesity-related condition. One hundred eighty-seven participants randomly selected to receive the reshape dual balloon had lost 14.3 pounds when the device was removed at 6 months as compared to the control group, which lost 7.2 pounds on diet and exercise alone. The results showed a 2.3 times greater weight loss in the reshape group compared to the control group.
They had kept off on average 9.9 pounds of that lost weight at the end of the following 6 months. The remaining participants who underwent a sham endoscopic procedure without the insertion of the device lost on average 7.2 pounds after 6 months.
The dual balloon structure is designed to minimize the risk of migration or obstruction in the unlikely event of a balloon leakage or deflation. Comprehensive counseling and support services such as in-office sessions will be provided to patients during the six months of treatment and for six months after it is removed.

Who are reshape candidates?
It is intended for adults with a bmi of 30-40 kg/m2 who have one or more obese-related health conditions such as hypertension, dyslipidemia, or diabetes and have had unsuccessful weight loss with diet and exercise alone.
Patients may experience vomiting, nausea, abdominal pain, gastric ulcers, and feelings of indigestion once the device is in the stomach. Intestinal obstructions have been reported due to deflated balloons passing into the intestines and have required surgical removal. Death due to intestinal obstruction is possible and has been reported with other intragastric balloons.

An effective treatment procedure for the condition called eustachian tube dysfunction is balloon tuboplasty. It is a modern surgical procedure which places a small balloon-like device in the blocked eustachian tube. The procedure offers long term relief to patients and has proven very effective in opening the blocked eustachian tube.

What is eustachian tube dysfunction?
The back of the throat and the middle of the ear is connected via the eustachian tube. This tube is filled with air and opens up while yawning or chewing. It does so to maintain or equalize air pressure on both sides of the ear drum. If the eustachian tube does not perform optimally, it prevents air from passing and obstructs while regulating air pressure. The blockage causes severe ear pain, ringing in ears and/or hearing loss. Common causes of blockage are sinus, common cold or ear or nose infection.

Balloon Tuboplasty procedure
If the occurrence of the obstruction is rare, then it is not a cause of worry, but if it is frequent, then surgical placement of tubes in eardrums might be required. In this procedure, a modified PTA catheter is introduced into a eustachian tube with the help of a special micro endoscope that allows the catheter to be placed in the right position. After the catheter has been positioned properly within the eustachian tube, the balloon present on its distal end is inflated.

The pressure is kept unchanged for 2 minutes, and then the balloon is deflated. After deflation, the catheter is removed under endoscopic vision. The results attained by using this technique show that the procedure for dilation of eustachian tube is very safe, simple and effective. Moreover, it confirms that balloon tuboplasty is favorable for those patients with stenosis of the ear tube. For assessment of objective and subjective parameters concerning postoperative results a specific scale identified as ETS is used.

Success rate of balloon tuboplasty
This modern procedure has proven to have an astounding success rate based on results obtained from clinical studies. Also, tympanogram normalization rate was found to be greater than control subjects treated with medical management alone. The improvement in the quality of life measures is also found to be higher. Moreover, zero risk or reported adverse effects concerning device or procedure was found.

The eustachian tube balloon dilation procedure does have associated risk factors. Associated risks may include tissue and mucosal trauma or carotid artery injury. Angioplasty is referred to as a cardiac procedure which involves widening the narrowed section of a coronary artery. Usually performed by an interventional cardiologist, this procedure is performed in a cardiac catheterization lab, short for a cath lab. An interventional cardiologist takes extra care to review the risks, in addition to obtaining your counsel prior to preparing you for the procedure.

The physician also refers to your medical history in order to determine the amount of anesthesia that should be used.

What happens during the procedure?

Although mild sedatives will be administered to help you relax, the process requires you to remain awake throughout the procedure. 

The procedure of angioplasty involves cardiac catheterization. The doctor numbs the intended site through medications and inserts the catheter. Following such, the doctor inserts a thin sheath in your artery, oftentimes in your groin or arm. A long, narrow, empty tube known as a catheter, is then carefully guided through the sheath, through the blood vessels and into the arteries near the heart. 

A little amount of liquid is then injected through the catheter which is moved through the chambers, valves and vessels of the heart. This movement is then pictured with the help of X-rays, with which the doctors can diagnose the condition and working of the coronary arteries and the heart valve.

The surgical procedure lasts for around 1.5 - 2.5 hours but the preparation and post - surgical procedures may take several hours. A patient is usually made to stay overnight to be monitored by medical personnel.

What to expect before an Angioplasty?

Most people are made to undergo a blood test and electrocardiogram before having an angioplasty. Usually scheduled a day before the procedure, such tests usually require separate appointments. Usually patients are restricted to eat since 12 hours before the exam.

What happens post-procedure?

After the procedure, you will be instructed to remain flat for 5-6 hours without bending your legs, so as to prevent bleeding. The nurse will instruct you as to when you can get up from your bed. You may not be permitted to drink or eat until the groin sheath is removed as such can cause nausea.

According to the procedure, the doctor will provide you with adequate after care advice.

5 Treatment Methods For Dysphagia

Dysphagia is the difficulty in swallowing food or liquid. Most people suffering from dysphagia are unaware of their medical condition and only realize it in later stages. When not diagnosed and treated in time, it may lead to aspiration pneumonia (lung infection occurring due to food particles or saliva entering the lung), weight loss, dehydration and malnutrition. Some of the symptoms include:

  1. Heartburn
  2. Choking
  3. Drooling
  4. Having the feeling of food getting stuck in the chest or throat.
  5. Food or stomach acid coming back up into the throat
  6. Regurgitation
  7. Hoarseness of the throat
  8. Gagging and coughing while swallowing
  9. Experiencing pain while swallowing

There are two types of dysphagia:

Esophageal Dysphagia:
This is also known as low dysphagia and is the condition in which one can sense food getting stuck in the throat or chest area. The problem lies in the esophagus. Generally, the surgical procedure is required to solve the problem. Some of the causes of low dysphagia include-

  1. Achalasia- lower muscles in the esophagus do not allow food to enter the stomach.
  2. Diffuse spasm- Uncontrolled contractions of the lower esophagus after swallowing food.
  3. Esophageal stricture- Narrowing of the passage of esophagus making it difficult for food to pass. It often leads to GERD (Gastroesophageal Reflux Disease).
  4. Esophageal Ring- A narrowing of the lower portion of the esophagus
  5. Scleroderma- Autoimmune disease where it leads to hardening and stiffening of body tissues.

Oropharyngeal Dysphagia:
This is also known as high dysphagia and is the condition where the throat muscles become weak making it difficult to swallow food. Some of the causes for high dysphagia include- stroke, Parkinson’s disease, Zenker’s Diverticulum (a condition in which food particles get accumulated inside a small pouch in your throat) and cancer.


Treatments:
Depending on the type of dysphagia, you will be advised any one of the treatment courses available. It can range from swallowing therapy, dietary changes, feeding tubes and medications to even surgery. The treatment is carried out by a multidisciplinary team. A multidisciplinary team includes- a speech-language pathologist, primary physician, gastroenterologist, dietitian, therapist, radiologist and pharmacist. The treatments for dysphagia include:

 

  1. Swallowing Therapy- If you are suffering from high dysphagia, you will be referred to speech and language therapists. You will be taught a number of exercises to make the process of swallowing easier and simple.
  2. Dietary Changes- In this method, you will be referred to a dietician or nutritionist. You will be advised a well-balanced and healthy diet that may include more liquid and semi-liquid food.
  3. Feeding Tube- When swallowing becomes extremely difficult and the patient is at risk of suffering from malnutrition and dehydration, the patient is usually fed through a nasogastric tube which passes through the nose and into the stomach. PEG (percutaneous endoscopic gastronomy) tube which is directly implanted in your stomach.
  4. Dilation- People suffering from low dysphagia have to undergo surgery. One of the methods is dilation. Through endoscopy, images of inside the throat is taken and a small balloon is inserted.
  5. Botulinum Toxin (Botox) - One of the causes for low dysphagia is achalasia. In such cases, botulinum toxin is used to relax and weaken the stiff and hardened muscles of the esophagus.

These were some of the treatment options for dysphagia that prove it is treatable and can be cured easily. Generally, dysphagia occurs in older people and babies. Whatever may be the cause, the main purpose of the treatment of dysphagia is to make the patient go back to oral feeding.

The body receives its source of energy and nutrition through the blood, which is the transport medium. The blood runs through a highly complex network of blood vessels, with the aorta being the largest one. It originates in the heart, the left ventricle precisely and supplies oxygenated blood to all parts of the body. Coarctation is a condition where there is a narrowing of the aorta. The heart, therefore, has to exert extra pressure to push out blood so that blood supply to the target organs is not affected.

The exact cause of aortic coarctation is not known, though it often develops at birth and so is known as a congenital disorder. Depending on the severity of the narrowing, symptoms develop at various times in life. While some infants demonstrate symptoms, some may not show any symptoms well into adulthood. Infants with severe narrowing may manifest the below symptoms quite early in life.

  1. Difficulty in breathing may gasp
  2. Difficulty in feeding
  3. Be extremely irritable
  4. Profuse sweating
  5. Pale skin

When these symptoms are not noticed and treated in time, the child may have heart failure or even die. If the symptoms do not manifest early in life, the infant can grow into childhood quite normally. Symptoms may then manifest which would be different, as below.

  1. Increased blood pressure
  2. Headaches (frequent)
  3. Weakness in the muscles
  4. Nosebleeds
  5. Chest pain
  6. Frequent fainting spells

These are generic symptoms and it is difficult to suspect coarctation of the aorta. Therefore, further imaging would be required to rule out coarctation of the aorta. There are some risk factors, most related to the heart, which puts one at increased risk of developing aortic coarctation.

  1. Bicuspid aortic valve
  2. Aortic valve stenosis and/or regurgitation
  3. Mitral valve stenosis and/or regurgitation
  4. Patent ductus arteriosus

Treatment: Diagnosis is the first step in treating coarctation of the aorta. This would include a series of imaging tests including an echocardiogram, electrocardiogram, chest x-ray, CT scan, and MRI.
Surgery is often the most definitive mode of treatment, and the approach chosen depends on the severity and stage of identification. The most common approach is to open up the narrowed portion and keep it from collapsing again, so that blood flow is restored. This is known as a balloon angioplasty and stenting. Some of the other procedures used include:

  1. Resection with end-to-end anastomosis
  2. Bypass graft repair
  3. Subclavian flap aortoplasty
  4. Patch aortoplasty

In all affected individuals, after surgery, medications are prescribed to maintain blood pressure levels, which is often increased. In addition, lifestyle changes including exercise and healthy eating habits have to be developed.

Coronary artery disease is one of the major killer diseases of the modern society. It is not a solitary problem but brings with it a multitude of issues including obesity, diabetes, stroke, and other metabolic disorders. A thorough understanding of what causes it and how to manage it can help save thousands of lives.

Causes: The circulatory system is mainly made up of the heart and a complex network of arteries and veins. The inner walls of these are lined with smooth muscles, allowing for free flow of blood. Gradually, over a period of time, given the density, the fat from the blood flowing through these vessels settles along the walls of these vessels. This attracts more fat, lipoproteins, and other inflammatory cells and so the process continues. This reduces the diameter of the blood vessels, therefore reducing the amount of blood supply to the target organs. If the target organ is a vital one like the brain or the heart, then it could lead to stroke or heart attacks.

Main causes for coronary artery disease include high blood pressure, high cholesterol, sedentary lifestyle, obesity, smoking, diabetes, and stress. Men are more prone than women, and family history and age puts them at higher risk.

Symptoms: Reduced blood supply to the target organ causes the following symptoms:

  1. Shortness of Breath: A person with coronary artery disease will feel short of breath and tired with most activities, even like walking a few meters.

  2. Chest Pain: A strong pressure sensation on the left side of the chest is an indication of coronary artery disease. Known as angina, it comes with stress and goes away once the stress is removed. It could sometimes radiate to the shoulder, down the arm, or up into the jaw also. These are classical symptoms of angina or heart attack and is considered an emergency. Of note, this is often mistaken for indigestion.

  3. Palpitations: A sensation where you are able to hear your heartbeat.

  4. Nausea and Excessive Sweating: Nausea and excessive sweating are also seen during angina.

Treatment: There are 3 modes to manage this.

  • Medications:

  • Surgical Procedures:  Surgical procedures including balloon angioplasty followed by placement of drug-eluting stents and coronary artery bypass surgery are done to manage more severe cases.

  • Lifestyle Change: Lifestyle changes including reduced fat consumption, decrease body weight, stop smoking, increased physical exercise, and reducing stress are highly important in preventing further damage.

Once suspected, coronary artery disease can be effectively managed and the extent of damage controlled using the above techniques.
