Chances of hiv spread through saliva is very very less. The virus can only transmit through contact with blood, semen, pre-seminal fluid, rectal fluids, vaginal fluids and breast milk. If you are not in contact with any of these then you don't need to be worried about it.

It's completely understandable to be worried about this situation. Leaving cotton in for 5 days after a tooth extraction can increase the risk of complications, so it's important to address it right away.
Here's what you need to do:
1. Contact your dentist as soon as possible: this is the most important step. Don't try to remove the cotton yourself, as this could further irritate the extraction site. Your dentist will be able to remove it safely and assess the condition of the wound.

You should get an opg done. This will help evaluate the amount of decay, the position of your tooth roots as well as proximity of the roots to the underlying nerve. Then the decision can be taken whether to get it removed or not. Kindly consult an oral and maxillofacial surgeon for the same.
