I have seen the details given. The main reason for weight gain appears to be uncontrolled hypothyroidism. Ideally in a well controlled patient t3 & t4 are in normal range plus tsh is around 3 to 4 mu/l. It is clear that you have not checked tsh on regular basis, hence there is a tremendous increase.
