Between two consecutive vertebrae, there is a cushion like substance called intervertebral disc. Spondylosis is a common degenerative condition of the intervertebral disc due to subsequent biochemical changes in the bones and soft tissues. This affects the normal spinal structure and function of the cervical (neck), thoracic (chest area), and lumbar (low back) spine.

Hla b27 what does it means in ankylosis spondolysis-
It's hlab27. Again, it's part of the immune system. It's an antigen on the surface of the cell that can be recognized by t-cells, which can recruit things to attack it. So this is the autoimmune component of the disease. This hla-b27 association actually exists for the seronegative spondyloarthropathies in general, but here, in particular, for as you should remember that connection.

Causes of ankylosing spondylitis -
Interesting thing about ankylosing spondylitis is that the exact cause of this condition has not been clearly understood till date though it has been the subject of numerous studies!

Ankylosing spondylitis is considered to be a genetic disorder in which the HLA-b27 gene, in particular, has an important role to play.

The risk factors that predispose a person to ankylosing spondylitis include:

Testing positive for the HLA-b27 marker:- people with this particular gene have a significantly higher risk of developing ankylosing spondylitis. However, it must be noted that not every person who has the HLA-b27 gene develops this disorder. Thus there are some additional factors, probably environmental, that are involved in the development of this condition.
A family history of ankylosis- patients with a family history of ankylosing spondylitis stand a greater chance to develop it as compared to those with no family history.
In addition, gender affinity has been strongly noticed in this condition. Males are almost 2 to 3 times more prone to develop this condition as compared to females. Ankylosis spondolysis is more common in men but occurs in women as well.
As commonly occurs in younger people, between the ages of 17-45. So restrict the life style of these people. However, it can affect children and those who are much older.
Frequent gastrointestinal infections:- the initial inflammation that activates the immune system is thought to be triggered by a bacterial or microbial infection. At a later stage, the immune system is unable to turn off this response which continues to attack the body’s own tissues now, in this case, the joints leading to arthritis. This explains the auto-immune nature of the disease.

Symptoms of ankylosing spondylitis-

  • Backache and stiffness along the spine – usually worse in the mornings or after periods of inactivity
  •  The joint pain (arthritis) experienced by the patient is often relieved by motion, applying local heat or taking a warm shower

  •  Difficulty in bending down (inflexibility)
  •  Stooping posture due to chronic back pain

Diagnosis of ankylosis spondolysis-

  1. History: Individuals typically complain of a gradual onset of low back pain and associated muscle spasms experienced over several months. The pain is usually described as worse in the morning and improving during the day. It may have followed a gradual ascending pattern from the tailbone (sacrum) to the lower back (lumbar spine) to the chest region (thoracic spine) and finally to the neck (cervical spine). Individuals may also report pain and stiffness of the rib cage and/or pain beginning in the larger peripheral joints (e. G, knees, hips, shoulders). Eye pain may be reported as well as a loss of appetite, weight loss, and fatigue. A family history of ankylosing spondylitis may also be reported on questioning.
  2. Physical exam: The exam may reveal tenderness over the area where the sacrum and hipbones meet (sacroiliac joints) and lumbar range of motion may be decreased. Chest expansion may be decreased to well below the 2 inches (5 cm). Evidence of eye inflammation (iritis) may be observed. Peripheral joints will be evaluated for tenderness, swelling, and restricted motion. Limited motion in the lumbar spine means that when the patient bends forward motion occurs by hip flexion, and very little or no lumbar spinal flexion occurs.
  3. Tests: Routine laboratory tests, including complete blood count (CBC) and blood chemistries, may reveal a low-grade anemia, an elevated erythrocyte sedimentation rate (ESR) indicative of active inflammation, and an elevated serum alkaline phosphatase indicating active bone remodeling. Tissue typing (HLA typing) may be done to determine presence of HLA-b27 since as is more likely if HLA-b27 is positive. X-rays of the pelvis may show characteristic fusion or inflammation of the sacroiliac joints (sacroiliitis). Lumbar x-rays may reveal fusion of the facet joints (anterior and lateral osteophytes bridge disc spaces and fuse motion segments into a" bamboo spine"). Mri will show evidence of inflammation in the sacroiliac joints.

Fear of death with great suspicion. Sensation as if a heavy, black cloud settled all over her and enveloped her head. There is an illusion of a mouse running under her chair. Bad effects of anxiety, fright, disappointed love, business failure, over-exertion. Severe pain in the uterine region, darting from side to side, sharp, lancinating, electric-like pain. Persons who are fair and beautiful-looking but inclined to be fleshy with fine hair and blue eyes, soft and lax muscles, anemic and chlorotic with pale face. Full of tears, slow and indecisive. It is almost impossible for them to tell their symptoms without weeping. The patient likes open air, feels suffocated in a closed, stuffy and heated room. Wants sympathy, thirstlessness is well marked.

Silicea- Soreness in feet from instep through to the sole in patients of ankylosing spondylitis, with cramps in calves and soles. Person is cold, chilly, hugs the fire, wants plenty of warm clothing, hates drafts, hands, and feet cold worse in winter. Great sensitiveness to taking cold. Intolerance of alcoholic stimulants. Offensive sweat on feet, hands, and axillae. The parts lay on go to sleep.

Sulphur- Ankylosing spondylitis in hot patients, who cannot walk erect, stoop-shouldered. Stiffness of back, sensation as if vertebrae glided over each other. Immobility. Factors That Can Cause Ankylosing Spondylitis!

The human spine has 33 vertebrae. However, some conditions can fuse these vertebrae. Ankylosing Spondylitis is one such condition. This disease may also be known as AS or Bechterew's disease. It is an inflammatory disease that can make the spine less flexible by fusing the vertebrae of the lower back together. In some cases, it can also affect the rib cage and make it difficult to breathe. This disease typically affects more men as compared to women. Most patients begin showing symptoms in early adulthood.

Ankylosing Spondylitis affects the sacroiliac joints. These joints are located just above the tailbone. It causes inflammation of the spinal bones that in turn cause pain and stiffness. With time, this inflammation spreads to the entire spine and the vertebrae begin fusing together. This can make movement difficult and painful. In severe cases, it can also lead to the development of a hunchback. This disease also affects the other tissues of the body. For example, it can affect other joints and aggravate arthritis or affect organs such as the kidney, heart, lungs, and eyes.

A specific cause has not yet been identified for Ankylosing Spondylitis. However, studies show that genetic factors can be a trigger. In particular, the presence of the HLA-B27 gene increases a person’s risk of developing symptoms pertaining to this condition. However, it is important to note that merely the presence of this gene does not make Ankylosing Spondylitis inevitable. Other genes that are associated with this disease are ARTS1 and IL23R. In many cases, the initial inflammation of the spine is due to a bacterial infection of microbial infection. Though the infection itself may be treated and cured, it may cause the immune system to react and trigger inflammation. Once activated, if the immune system cannot be turned off, this inflammation will continue and can trigger Ankylosing Spondylitis. In each case, the disease presents a unique pattern of progression.

There is presently no cure for this condition, but with an early diagnosis and treatment, the symptoms can be managed and progression can be slowed. Ankylosing spondylitis (AS) is a type of arthritis that affects the spine. This chronic disease is caused due to inflammation of the joints in the spine and usually affects young men. It causes the vertebrae to grow or fuse together, causing a rigid spine. Actually, it affects the discs in between the vertebral column. These discs get swollen, causing a limitation of spine motility. It is an autoimmune disorder and its symptoms are lower back pain and stiffness, which usually moves to the legs.

  • The deformity due to AS can be mild or severe. In extreme cases, the patient gets a stooped spine. 
  • Early diagnosis can prevent significant deformity as well as help in controlling the pain and stiffness.

  2. Avoid getting constipated and ensure proper evacuation of bowels.
  3. Include ghee in your diet for lubrication and for lowering the vata dosha.
  4. Apply heat on the affected areas to control the pain and stiffness.
  5. Eat light foods and strictly avoid oily foods and curd which increase ama.
  6. Avoid sleeping during the daytime.

However, due to various reasons, some identified and others unknown, there could be inflammation of the areas where the bones join the muscles leading to impaired movement. The range of motion could be reduced or it could be painful, thereby limiting movement.

Spondyloarthritis includes a group of inflammatory diseases that involves the bones and joints and enthuses which are areas where the ligaments and tendons attach to the bones. This causes conditions like plantar fasciitis, ankylosing spondylitis, psoriatic arthritis and reactive arthritis. The term spondyloarthritis is a blanket term to describe any of the above-mentioned conditions.

Causes: While the exact etiology is not identified for sure, most patients with spondyloarthritis have an autosomal dominant gene (HLA-B27). Chronic infections could trigger the onset of the disease too.

Symptoms:

1. In all forms of spondyloarthritis, there is always inflammation of the spine, joints and entheses. If the fingers or toes are involved, there could be swelling leading to what are typically known as 'sausage digits'.

2. There is sacroiliitis which is inflammation of the joint between the pelvis and the spine, and spondylitis which is inflammation of the joints between the spines. 3. Family history and autosomal dominant gene.

Diagnosis:

A detailed medical history and physical examination form the cornerstone of a confirmed diagnosis. In addition, radiographic examination and diagnostic blood tests also could be done to confirm the diagnosis. Radiographs, especially, of the spine and the low back show typical changes including sacroilitis in suspected cases of spondyloarthritis. If the spine does not show these changes, but there is a high likelihood of spondyloarthritis, then MRI (magnetic resonance imaging) can be used to confirm the diagnosis.

As a final step, a blood test for the HLA-B27 gene can also be confirmatory. However, it is not true that a person with the gene will definitely develop the disease.

Treatment:

  1. The most important aspect of treatment is the use of pain medications and anti-inflammatory drugs (NSAIDs) which help manage pain and inflammation.
  2. Corticosteroid injections may sometimes be given directly into the joint spaces or the membranes to provide quick relief of these symptoms.
  3. ESWT / RSWT has a definite role to cure enthesopathy. Plantar fasciitis, and achillodynia can be cured with ESWT. Even small joints of hand and feet are amenable to treatment with RSWT. It is nonoperative and noninvasive treatment and has no side effects.
  4. Very rarely, stronger drugs such as methotrexate or sulfasalazine may be used.
  5. Another set of drugs known as TNF-blockers (tumor necrotizing factor blockers) are also shown to be highly effective in controlling the pain of the spine and joints.
  6. In very severe cases, surgery may be required to correct joint damages or correct severe degenerative changes.
  7. Exercise plays a very vital role in controlling the issue and improving range of motion.

Ankylosing spondylitis (AS) is a type of arthritis that affects the spine. This chronic disease is caused due to inflammation of the joints in the spine and usually affects young men. It causes the vertebrae to grow or fuse together, causing a rigid spine. Actually, it affects the discs in between the vertebral column. These discs get swollen, causing a limitation of spine motility. It is an autoimmune disorder and its symptoms are lower back pain and stiffness, which usually moves to the legs.

  • The deformity due to AS can be mild or severe. In extreme cases, the patient gets a stooped spine. 
  • Early diagnosis can prevent significant deformity as well as help in controlling the pain and stiffness.
  • Allopathy doesn’t really know the real reasons for this disease and here is where Ayurveda scores.
  • According to Ayurveda, AS is caused due to aggravated vata or wind, which causes an imbalance in vata dosha.

Ayurvedic treatment

  2. Avoid getting constipated and ensure proper evacuation of bowels.
  3. Include ghee in your diet for lubrication and for lowering the vata dosha.
  4. Apply heat on the affected areas to control the pain and stiffness.
  5. Eat light foods and strictly avoid oily foods and curd which increase ama.
  6. Avoid sleeping during the daytime.
  7. Yoga asanas like pawana muktasana, bhujangasana, dhanurasana, paschimottanasana and vakrasana should be practised regularly.
  8. The practice of pranayamas such as and shodhana, chandrabhedi sheethali and bhramari also helps.
  9. Chewing 2–3 raw garlic buds on an empty stomach in the morning with water helps in alleviating lumbar pain in AS.
  10. Use a powdered preparation of dried ginger root, celery seeds and cumin seeds in equal quantities with rock salt for taste, and have one teaspoon of this mixture with warm water at bedtime to reduce flatulence as well as lumbar stiffness.
