In the early stages of the disease a flattening of the lumbar curve may occur. Then, with increasing pain and a flexed posture, the thoracic curve can become more accentuated. If unchecked, this can be followed by a loss of cervical curve and a protracted cervical spine develops as an attempt to compensate for the forward flexion. As the hips become more flexed, hip flexion contractures can occur and the knees flex to improve balance and vision. Poor posture results in muscle fatigue, weakness, disability and a poor body image.Good management of the disease should prevent poor posture from occurring,although any fusion may be delayed and not halted. Note especially the knee flexion and hip flexion deformities. The reason for these deformities is that,if the spine is constantly flexed, the patient would fall over forwards. By flexing the knees and hips, the person shifts the line of gravity back within the base to become more stable.

AIMS OF PHYSIOTHERAPY MANAGEMENT:

The overall aims are to minimize deformity and disability and to improve well being, thereby maintaining normal function and improving the person’s quality of life.

PAIN  IN  ANKYLOSING  SPONDYLITIS

Pain may vary from low grade discomfort to intense pain. 

Typically, pain is aggravated and is often reduced by exercise. The perception of pain can be influenced by several factors, including fatigue. 

Physiotherapy modalities such as ultrasound, dry needling, and transcutanous nerve stimulation, mega pulse and gentle spinal mobilization can be effective, especially for localized pain.

If the person has not exercised recently, exercises will need to be commenced gradually and the person warned that initially they may provoke pain. 

Wearing supportive shock-absorbing shoes can decrease the jarring effect of walking on hard surfaces and reduce the likelihood of aggravating the pain. Hot packs can reduce pain.

The spine or the backbone is responsible for the fluid movement of the body, especially standing, bending, and sideways movement. It also is an important part of the nervous system and carries multiple nerves to the lower part of the body. The lower spine has sacral and iliac vertebrae, which have disks between the vertebral bones where these nerves are located. With age, infection, tumors, or injury, this soft tissue becomes hardened and get the bones to fuse. This is a type of arthritis known as ankylosing spondylitis and limits movement between the disks.

Causes: With age, the soft vertebral tissue gradually hardens. There is a genetic component also to this, so it runs in families. In addition, infection or tumors of the spine and injuries involving the spine can also lead to this condition.

Symptoms: When AS ankylosing spondylitis (AS) onsets, there is usually morning pain and stiffness in the low back and upper buttocks. The intensity of this gradually reduces over the course of the day. The range of movement also is reduced, with bending, stooping, and sideward movement becoming gradually difficult. It is believed that people with AS have an overactive immune system, and homeopathy helps by suppressing this activity. A homeopath will have a detailed discussion with the patient and based on the presenting symptoms, an appropriate product would be prescribed.

  1. Aesculus: The patient would have extremely painful and stiff low back and hips. It would be constant ache with walking and stooping, and it is not easy to rise from a sitting position. The patient also feels that the legs would give way, making them hesitant to walk.
  2. Rhus Tox: When the patient presents with low back pain that is present at rest, with sitting and inactivity, Rhus is usually preferred. The pain improves with activity and movement and radiates down the legs all the way to the foot.

Factors That Can Cause Ankylosing Spondylitis!

The human spine has 33 vertebrae. However, some conditions can fuse these vertebrae. Ankylosing Spondylitis is one such condition. This disease may also be known as AS or Bechterew's disease. It is an inflammatory disease that can make the spine less flexible by fusing the vertebrae of the lower back together. In some cases, it can also affect the rib cage and make it difficult to breathe. This disease typically affects more men as compared to women. Most patients begin showing symptoms in early adulthood.

Ankylosing Spondylitis affects the sacroiliac joints. These joints are located just above the tailbone. It causes inflammation of the spinal bones that in turn cause pain and stiffness. With time, this inflammation spreads to the entire spine and the vertebrae begin fusing together. This can make movement difficult and painful. In severe cases, it can also lead to the development of a hunchback. This disease also affects the other tissues of the body. For example, it can affect other joints and aggravate arthritis or affect organs such as the kidney, heart, lungs, and eyes.

A specific cause has not yet been identified for Ankylosing Spondylitis. However, studies show that genetic factors can be a trigger. In particular, the presence of the HLA-B27 gene increases a person’s risk of developing symptoms pertaining to this condition. However, it is important to note that merely the presence of this gene does not make Ankylosing Spondylitis inevitable. Other genes that are associated with this disease are ARTS1 and IL23R. These genes influence the functioning of the immune system. While it can affect people of all ages, adults are at a higher risk of suffering from this condition.

In many cases, the initial inflammation of the spine is due to a bacterial infection of microbial infection. Though the infection itself may be treated and cured, it may cause the immune system to react and trigger inflammation. Once activated, if the immune system cannot be turned off, this inflammation will continue and can trigger Ankylosing Spondylitis. In each case, the disease presents a unique pattern of progression.

There is presently no cure for this condition, but with an early diagnosis and treatment, the symptoms can be managed and progression can be slowed. This treatment usually takes the form of medication to relieve pain, inflammation and discomfort and physical therapy. Surgery cannot be used to treat Ankylosing Spondylitis but in cases where it has caused severe joint damage, surgery may be advised to replace the damaged joint.
