Migraine problem since 3 years. I think its due to some food stuff like biscuits and food made from maida. Mostly pain occurs on right side of head . How can it be cured ? Can't take any medicine because I am breastfed mom .

Migraine is a kind of relapsing headache. Its symptoms include - episodic unilateral or bilateral, severe and pulsating headaches, associated at times with nausea and vomiting, relieved momentarily on rest or taking medications and aggravated by precipitating factors like sleep deprivation, loud noises, continued sun exposure, allergy to some agents etc. The treatment of migraine is mainly done for relief from acute headache attacks and later treatments to prevent future relapses. Also treatment of factors which tend to cause relapses is also important. Consult a headache specialist for treatmnet. You may contact me online for an further queries and management.

Brain Tumor Surgery

IFAANS, DNB (Neurosurgery), MBBS
Neurosurgeon, Ghaziabad
Brain Tumor Surgery

Although surgery is considered to be the first step of treatment in any form of tumor, but its feasibility depends upon the type, size and location of the tumor. It is not necessary that every kind of brain tumor might require surgery.

Indications of Brain Tumor Surgery

  • Diagnosis of the type of tumor by taking a sample of the tumor for laboratory examination
  • Complete resection of tumor
  • Removal of the tumor as much as possible to slow down its growth and improve the symptoms
  • Remove the tumor in order to help other treatment
  • Provide direct access for other forms of treatment like chemotherapy, radiotherapy etc.
  • To relieve pressure caused by tumor on surrounding structures

Types of Brain Tumor Surgery: The different types of surgery that are performed in cases of brain tumor include the following:

Craniotomy: Craniotomy is the most commonly performed procedure to remove brain tumor along with a piece of bone. The removed bone is replaced and the tumor is sent for histopathology (biopsy). The surgery is performed using a high end microscope by any of the following techniques:

  • Extended bifrontal craniotomy
  • "Eyebrow" craniotomy (supraorbital craniotomy)
  • "Keyhole" craniotomy (retro-sigmoid craniotomy)
  • Orbitozygomatic craniotomy
  • Translabyrinthine craniotomy

Shunt: A narrow piece of flexible tube with a pressure regulated valve in between is called a shunt. This is used to relieve the intracranial pressure caused by obstruction of the natural brain fluid, Cerebrospinal fluid (CSF) pathway by tumor mass. The procedure involves insertion of a drainage system into the brain to remove or drain excess of blocked fluid into the peritoneal cavity.

Stereotactic surgery: This surgery is done by creating a three dimensional image called stereotaxy using computers. It aims to find out the location and position of the tumor. In fact it can also be done to aid tumor removal, implant radiation pellets or for other treatments.

Embolization: It is a procedure used to reduce the amount of blood supply to a tumor by blocking the blood flow in the selected arteries. It is generally performed before the main surgery.

Endoscopy and endoscope assisted surgery: Endoscope is a tool, which helps to closely visualize the tissue through small openings in difficult to reach areas. This tool can be used in various brain surgeries to precisely reach the target location without damaging the adjacent structures.

Adjuvant modalities to improve outcome: In addition to above mentioned surgeries, following surgeries may also be performed in relation to brain tumor treatment:

  • Laser surgery
  • Photodynamic laser surgery
  • Ultrasonic aspiration
  • Fluorescence guided surgery (FGS)
  • Electrophysiological monitoring

There can be many other causes of tremors also. Get his thyroid function tests, sugar test and also it can be a possibility he is having anxiety issues. Parkinsonism also has various other symptoms such as stooped posture, short steps, rigidity and stiffness.

My son about 22 years of age having slight vibration in hand when took glass in hand since last month. What may be the cause and what is the line of treatment.?

MBBS
General Physician, Cuttack
. 1. It could be due to essential tremor. The cause of the tremor is unknown. In some cases it is hereditary and runs in families. There is no definite treatment unless it is severe 2 other causes of tremor could be stress, anxiety, panic, nervousness, inadequate sleep, physical exhaustion, strong emotion, hyperthyroidism, hypoglycemia, smoking/tobacco/alcohol abuse/withdrawl, excess tea/coffee, drugs like steroid/amphetamine, neurological disorder, cerebellar dis order 3. Investigate yourself to exclude the above causes of tremor after consulting neurologist 4. Avoid stress/anxiety/nervousness/ panic etc 5. Avoid excess tea/coffee/ smoking/alcohol if you have 6. Take good nourishing diet 7. Have adequate sleep. 8. Do a thyroid function test to exclude thyroid problem 9. Consult neurologist in person for further advice.

General Physician,
EPILEPSY OR FITS (As laymen say )
***********************************************
Seizure is a sudden change in behavior that is the consequence of brain dysfunction.
Approximately 0.5–1% of population has epilepsy.
ome seizures are provoked i.e. that occur in the metabolic derangement, drug or alcohol withdrawal and in situations like acute paralysis or acute encephalitis. Such patients are not considered to have epilepsy because these seizures would not recur in the absence of the provocation.
Less than 50% of epilepsy cases will have an identifiable cause such as head trauma, brain tumor, paralysis, infection, brain malformation etc.
Having one seizure does not always mean that the patient would always get a seizure.
One episode of seizure may not require treatment.
Hospitalization is required in the first seizure only if it is associated with prolonged post seizure altered level of consciousness.
Patients with unprovoked seizure may not be allowed to drive for some time.
In children, seizure can occur with high grade fever.
In adults, the first episode of seizure may be due to worms in the brain. In such a situation, it may be necessary to do an MRI test.
A patient with seizure can get married, live a normal life and produce children.
It is a misnomer that during a fit you need to make the person smell a shoe.
During epilepsy, never put the fingers inside the mouth of the patient as you could be bitten. Use a spoon instead to prevent tongue bite.
A patient with epilepsy fall will have stiffness in the body; on the contrary, patient with cardiac loss of consciousness will fall loose.
A seizure that lasts for more than 5–10 minutes requires specialized attention.

If you had an EMG/ NCS that diagnosed you with carpal tunnel, you can decide further management based on its results- for mild to moderate cases, hand therapy is the key, using splints as well; for severe cases surgery is better- a hand surgeon should be able to help.

It is a type of facial palsy called bells palsy. It is good that 90% has improved in 1 month. It will improve further over time. No specific medicine is required for that. Do regular facial muscle exercise as advised by your physiotherapist. But do it repeatedly and frequently you may continue facial nerve stimulation therapy and physiotherapy for few more days.
