I have a gf I love fingering in her pussy is it necessary to use any lubricant while fingering. Will it cause any harm if I'll not use any lubricant.

Hello- If your partner is feeling uncomfortable during fingering, or feeling cutting pain and burning, you have to use lubrication, if not then there is no need to use any external lubrication. Vagina produces self lubrication to avoid friction during sex, you can also experience it yourself (vagina gets wet during sexual arousal).

Shoulder Pain: Causes, Diagnoses & Treatment

Shoulder Pain: Causes, Diagnoses & Treatment

Shoulder pain can be a persistent dull and nagging sensation in the upper arm or shoulder area. It can be easily differentiated from neck pain as it is related to the movement of shoulders i.e. it will increase and decrease with shoulder movements, especially overhead activities. The other signs to identify shoulder pain is if your shoulder movements are reduced.

If you find it difficult to scratch / wash your back, comb your hair or reach your back pocket, it clearly shows you are suffering from shoulder pain. Shoulder pain can happen due to various reasons, it varies from person to person depending on the activities he/she is involved in, for e.g. sports, manual job etc. It can be a single major injury or multiple small injuries due to repetitive usage patterns. Also, degenerative changes in various parts of the shoulder can lead to shoulder pain.

In case of middle to old age, injuries that can cause shoulder pain are:

  • Biceps Tendonitis
  • Acromio-clavicular Joint arthritis
  • Scapular Dyskinesia
  • Trapezitis
  • Suprascapular nerve entrapment In young age, injury due to sports or gym activity can cause shoulder pain
  • Superior labral anterior/posterior (SLAP tear)lesion

Also, traumatic or repeated dislocations of shoulder can cause shoulder pain due to:

  • Bankart lesion

Diagnose Shoulder Pain

Shoulder pain can be diagnosed only after thorough history taking and clinical examination. However, some imaging studies can be very helpful like:

  1. X-rays: Through an x-ray one can view the shoulder in many different views.
  2. Ultrasound: With an ultrasound, you can also check the damaged caused to the tendons and muscles of the shoulder but the quality of reporting depends upon the expertise of the doctor.
  3. MRI: MRI gives a clear picture of the shoulder as it shows everything related to a shoulder joint that is joints, vessels, tendons and muscles that too from different angles.
  4. Diagnostic Arthroscopy: A surgical procedure often used by orthopaedic surgeons to diagnose and treat issues inside a joint. Arthroscopy has the big advantage that one can use it to diagnose as well as treat at the same time.

Treatment for Various Types of Shoulder Pain

  1. For Shoulder Impingement or Rotator Cuff tear: Give rest to the shoulder for a few days only Dedicated Physiotherapy Protocol Bursal injections If these fail then, (Surgery)Arthroscopic sub-acromial decompression If Cuff tear is found, then one can go for Arthroscopic/Mini-open repair
  2. Labral Lesions: For anterior Labral lesions (Bankart's) following anterior shoulder dislocation then surgery in the form of Arthroscopic Bankart's repair is the only option to reduce risk of recurrence and degeneration.

SLAP Lesions

  1. Pain relief and physiotherapy as first step If this fails then following surgical options are available
  2. Arthroscopic debridement or repair of labrum
  3. Biceps tenotomy or tenodesis

Arthritis

In this case, the joint is already destroyed and has to be resurfaced for pain relief and to maintain ROM. If Rotator cuff muscles are working, then Shoulder resurfacing hemiarthroplasty (preferred in young people), that has a life of 10 to 15 years, Total shoulder replacement (preferred in middle aged to old people) has a life of 10 to 15 years. But, if rotator cuff muscle are not working then Reverse Geometry Shoulder replacement is recommended, that has a life of 10 years.

It is always advisable to get yourself diagnosed properly in case of shoulder pain as management changes depending on clinical situations. Neglecting shoulder conditions can lead to bigger problems later on.

Always remember 'a stitch in time saves nine'

In what time girl gets pregnant (on period time or other normal days)Please tell.
If she is having normal 28 days cycle, the ovulation occurs in the middle of the menstrual cycle. 0-10, 20-28 days are safer as compared to other days. Pregnancy is occurs mostly in ovulation period (day 11- 20).

A week before the cycle and a week after that is comparatively safe but no time is 100%safe that's why it's better to use contraception condoms or pills both are ok remember there is 2%failure with all the contraceptive methods but that much chance everyone has to take.

Diet Tips for Pregnant Women

Diet Tips for Pregnant Women

Before I start bombarding you with advice, please accept my heartiest congratulations on being pregnant. This is a unique lifetime situation for you, so enjoy and have the fondest memories ever.

Despite having the responsibility to take care of the nutritional needs for your growing fetus, you do not have to eat for two people. All you need is 300 more calories in the second half of pregnancy and 500 more calories when you are breastfeeding.

Try and have 3 main meals and three snacks in a day to give you constant supply of energy. Cornflakes, fruits, Chilla, poha, sprouts and eggs make good snacks and have high nutrient value. Avoid overindulging in cereals like wheat and rice as these result in a high sugar surge followed by a sudden drop, making you lethargic and low in energy.

Include as many colors in your diet as possible; as veggies or fruits and you would not go deficient in any nutrient. For example- yellow color as in mango, ripe banana, pumpkin etc, red color as in tomatoes, water melon, pomegranate etc..

Avoid eating raw salads(unless at home where you are sure that the veggies have been washed nicely), uncooked meat like grilled stuff from outside, which could have remained partially cooked at the core and may cause infection. In parties, it is better to consume the fried stuff instead which ensures that all the germs have been killed.

Certain foods like unripe pineapple, raw papaya, and pineapple should not be consumed in large quantities in the first three months. Avoid belue cheese, sea water fish ( if not cooked properly) and canned food containing monosodium glutamate.

Pregnancy slows down the digestion, so eat your dinner early. Have 2-3 hours gap before dinner and sleep. If you feel hungry at bedtime, you can take a glass of milk or a small snack.

Stay physically active and do not overstuff yourself. A normal healthy person does not require any commercially available protein supplement powders, but if you feel your diet may be deficient or you do not have time to prepare healthy snacks, it is OK to include some supplements in your diet but guard against too much weight gain as these powders are usually high in calories and fat content.

It is OK to have all the stuff your mom lovingly prepares for you, but mind the portion size. After all, you are going to be pregnant only once or twice in your life.

Without wasting time, you should first get your mammography done to rule out any malignancy. Consult your gynecologist after that.

No it is not possible for her to be pregnant. However to reconfirm you can do the home test for pregwith the first morning sample of urine after she wakes up. That way you can be sure upto 99% another suggestion to you is to always have safe protected sex to protect yourself and your partner from sexually transmitted infections which are rampant and from unwanted pregnancy too. All the best.

Hello dr, am 15 weeks pregnant women, I want to know the sleeping positions for this pregnancy time,

MBBS, DGO, MD, Fellowship in Gynae Oncology
Gynaecologist, Delhi
Hello dr, am 15 weeks pregnant women, I want to know the sleeping positions for this pregnancy time,
Hello, The best sleep position during pregnancy is “SOS” (sleep on side). Even better is to sleep on your left side. Sleeping on your left side will increase the amount of blood and nutrients that reach the placenta and your baby. Keep your legs and knees bent, and put a pillow between your legs. - If you find that you are having problems with back pain, use the “SOS” position, and try placing a pillow under your abdomen as well. - If you are experiencing heartburn during the night, you may want to try propping your upper body with pillows. - In late pregnancy you may experience shortness of breath. Try lying on your side or propped up with pillows. Left is best. Right now, side sleeping is safest for your baby. Plus, it's more comfortable for you as your abdomen grows. Is one side of the body better than the other for sleeping? Experts recommend lying on your left side. It improves circulation, giving nutrient-packed blood an easier route from your heart to the placenta to nourish your baby. Lying on the left side also keeps your expanding body weight from pushing down too hard on your liver. While either side is okay, left is best. These suggestions may not sound completely comfortable, especially if you are used to sleeping on your back or stomach, but try them out. You may find that they work. Keep in mind that you may not stay in one position all night, and rotating positions is fine. Also avoid sleeping on your back: This can cause problems with backaches, breathing, the digestive system, hemorrhoids, low blood pressure and cause a decrease in circulation to your heart and your baby. This is a result of your abdomen resting on your intestines and major blood vessels (the aorta and vena cava). Do not sleep on your stomach: When you are farther along in your pregnancy, your abdomen undergoes physical changes and makes it more difficult for you to lay on your stomach.
