Hi I have de quervian's tenosynovitis on my right hand. It's fine the whole day but it hurts in the night if I don't put on the wrist splint. Is this how it will have to be for the whole life? I have gone to a foctor who gave me some pain killers for a week and suggested complete rest of the hand for a week or so. That is not an option for me. Is there a way out of this other than a surgery?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. De quervain's tenosynovitis is inflammation of sheath of 1st extensor tendons due to which there is pain at the level of styloid process. The treatment depends upon the symptoms you are having, normally ice packs for local application, thumb spica splint for 3-4 weeks not only at night but all throughout, anti-inflammatory medication as per your suitability, local analgesic spray for application but no massage, ultrasonics and laser as physical therapy modalities are useful in many cases. But if it does not get better, then release is better, it is actually a daycare procedure which take about 8-10 minutes to do in ot under local anesthesia, so it is very safe procedure. Shoulder advice. Regular stretching of shoulder muscles. Warm up and cool down is a must. Hot fomentation.

My age is 18. My right knee has swelled & it painful also. From last 8 days. That day I cycled 20 km. Dr. Gave medicine & suggested to wearing a knee cap. When taking medicine it gave relief & after 4-5 hours it backs again. Before it happened I had a badly fever. Now fever has gone but Knee gets pain. please suggest medicine.

Apply hot fomentation twice daily. Avoid bending in front. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang asana -- lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core strengthening exercises- straight leg raised with toes turned outward, repeat 10 times, twice a day.

Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Inform after 5 days.

Hello, there are many reasons for this type of pain- 1. Overuse of muscle, 2. Deficiency of vitamin d3 or calcium, or lack of water in the body, 3. Postural stress, 4. Muscle strain, etc. Please send me all investigation reports if available. I can help you by teaching correct postural habits and ergonomics, do consult me privately. Till then follow these advises:- 1. Take calcium supplements and calcium rich diet like milk, paneer, buttermilk, cheese etc and sun bath daily to prevent further damage to your joints. 2. Avoid heavy lifting. 3. Avoid squatting and weight bearing activities. 4. Use western toilet. 5. Wear shock absorbing, good quality of shoes while walking. 6. Keep roll of a towel under your knees while sleeping. 7. Perform some gentle exercises and stretching of your all joints before getting out of your bed. 8. Walk straight with shoulder protracted. 9. Perform deep breathing exercises or yoga for proper functioning of lungs. 10. Perform calf pumping exercises, which i'll explain you in the next session. Thanks.

Hello lybrate-user, have undergone xray for the knee joint? there are many reasons for the cause of knee pain. If you stand for a long time or sitdown for long duration and deficiency of calcium can cause knee pain. I suggest you to diagnose the cause of knee pain for perfect treatment. You can take ultrasonic therapy for faster relief from pain.

I am suffering from lower back pain from last 1.5 years. I got injured in the gym while lifting heavy weights inappropriate manner. Pain is consistent but very less! in MRI reports shows that their is a Diffuse Posterior bulging L5-S1 disc. Plz help me with this am very uncomfortable with this pain! i cant go for sports m a sportsman.

For young males with single disc, the best treatment is endoscopic dics removal, it will relieve you of symptoms, will not cause structural damage to your surrounding structures, less post operative stay and early return to sports. Since you are having severe pain, no amount of rest of therapy may relieve your symptoms.

Get yourself checked with a physiotherapist for a detailed postural assessment. Forward bending, slouched sitting, unsupported sitting posture for prolonged duration can lead to backache. Long working hours, abnormal sleep-wake pattern can also lead to backache. So, check whether your postural habits have lead to this problem. You can change your sitting posture- to supported sitting. You can also avoid bending forward to avoid backache. And consult a rheumatologist, if the pain doesn't subside with postural correction.

Dear you can follow these methods for relief from backache (back pain): * first correct your sitting posture. Do not bend your lower back. You can use lumber belt for support. * do regular back strength exercises. Move your clockwise and anticlock wise. * do not sleep on very soft bed, sleep on slight hard cotton mattress. * do tadasana and bhjangasana for back strengthening. If you can not understand these asans you can consult me. Surya namaskar is also best asan in back pain. * take calcium rich diet and calcium and vitamin d supplements regularly. *do not take any heavy weight or heavy work. * if you are over weight then follow a good and effective diet plan to loose some kilos of weight. You can consult me to get a diet chart. * you can take guggalu tablets of himalaya for fast relief.

To loose weight: step 1-stop taking sugar & fat e. G. Ghee/fried stuff. Step 2 take a plat of salad before meals reduce no. Of chapatti gradually. You are most probably suffering from oa of knee. Knee surgery-total knee replacement (tkr) is quite safe & gives uniformly good results, in our hands. We have well equipped centers in delhi. Do ask for detailed treatment plan. Surgery should be done only when patient is still active & will be able to take benefit of surgery. Do not ignore. It could be beginning of a serious problem. Kindly show me x rays before a final decision can be taken. You will be thoroughly investigated before surgery is under taken.

Apply pranacharya restopain oil or prasarini oil on your affected part then give hot fomentation. Take maha rasnadi kwath 2-2 tsf twice a day. And agni tundi vati and maha yograj guggul 1-1 tab twice a day. Avoid junk food. Spicy food. Fermented food. Tea. Alcohol. Avoid alcohol consumption. Do pranayama early morning.

Yes it could be vitamin d deficiency. You should take calcium and vitamin d supplements. Fatigue can also be due to heart problem. Get yourself checked.

Since from the child birth, my wife is suffering from pain in her right side upper outside bone of leg near to butt. She says she can not sit in a single posture for long time. If she sits on right side butt. She has to get-up slowly. We don't know the reason of pain. But it is there for last 3-1/2 years. We got our baby by her cezariane operation. Our query is. It requires detailed investigation and examination. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

You are too young. These symptoms should not be ignored. Stiffness & pain in back & other joints is quite a common condition. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Any way take ibuprofen 200mg od & sos x 5days you will need other supportive medicines also. Take multivitamins & other antioxidants along with. Do back (spine) exercises make sure you are not allergic to any of the medicines you are going to take do reply back for private consultation and detailed treatment plan. Do not ignore. It could be beginning of a serious problem.
