MBBS
General Physician, Faridabad
I am a 49 year old male suffering from high BP, diabetes, acidity and piles. Kindly advise.
after needful investigation, you need medicines for bp, diabetes, acidity and treatment of piles can be done In the majority of cases, piles resolve on their own without the need for any treatment. Treatments can help significantly reduce the discomfort and itching that many patients experience Diet - piles can be caused by too much straining when doing bowel movements, which is the result of constipation. A change in diet can help keep the stools regular and soft. This involves eating more fiber, such as fruit and vegetables, or switching your cereal breakfast to bran. Water is the best drink, and the patient may be advised to increase his/her water consumption. Some experts say too much caffeine is not good. Body weight - if the patient is obese, losing weight may help reduce the incidence and severity of hemorrhoids. Simple things you can do yourself to help prevent piles: Try not to strain when you go to the toilet Avoid laxatives Exercis take tab pilex 1tab tds, anovate cream locally inside anus, siltz bath [hot water in a tub, add kmno4 in it, sit 1/2 an hr in tub, it will help.Avoid spicy ,Drink 3- 4 Litre of Water Daily. welcome for further help. or go for surgery

General Physician,
Heart Patients Beware of Summer
Summers have arrived and the temperature rises every day. Summer brings with it, its own set of problems. Most common summer disorders are dehydration, heat cramps, heat exhaustion and heat stroke on one hand and acidity, infections, diarrhea, cholera, Typhoid and Jaundice on the other hand.

Dehydration can precipitate heart attack in susceptible individuals. The normal fluid requirement is 30 ml per kg weight, but the same needs to be increased in the summer because of the loss of fluid from sweating. Apart from water, sodium or salt is also lost. A person, therefore, needs to take more fruits during summer period. Not passing urine in eight hours, dry armpits, feeling exhausted or feeling weak are suggestive of underlying dehydration.Dehydration can make the blood thick and precipitate heart attack in patients with uncontrolled blood pressure or diabetes.

Walking is a necessity for heart patients and the same should be continued even during peak summer but the timing should be so chosen that peak heat periods are avoided. One can walk early in the morning or late in the evening. People taking anti-allergic pills should take special precautions as they are more likely to get heat stroke. Heat stroke is a medical emergency leading to charring of organs because of extreme internal heat. A person?s temperature may be more than 105°F.

Heart Palpitations May Get Their Start in Perimenopause While there are no hard statistics on how many menopausal women experience heart palpitations, Portman estimates that 25 to 40 percent of women going through menopause will develop them. They tend to be even more common in women going throughperimenopause, the four years or so preceding menopause. “That’s when hormones seem to fluctuate the most,” Portman says. However, it’s unclear whether the hormonal changes that come with menopause are directly related to the development of heart palpitations. Portman says studies have shown that epinephrine and norepinephrine, the neurotransmitters that regulate heart rate and blood pressure, tend to fluctuate in menopausal women. These neurotransmitters might one day be a target for specific treatment of heart palpitations associated with menopause.

If LDL and HDL cholesterol level is found at normal and ideal level in our fasting blood test, then should our arteries be also mostly normal without any obstruction of cholesterol?

Having low cholesterol is a sign that your arteries are healthy. However, atherosclerosis, the buildup of cholesterol on the walls of arteries, happens naturally throughout a lifetime. Ways to reduce the speed of buildup is to reduce overall saturated fat intake. Saturated fats have been shown to increase LDL (low density lipoprotein) levels, whose function is to carry fats and cholesterols through the blood vessels. This is necessary because fats and cholesterol are lipids, and cannot dissolve freely in water, or in this case, blood. During times of stress, the cholesterol becomes sticky due to cortisol and sticks to walls of arteries, over time, this buildup can clog arteries. (this is one reason stress is affiliated with poor cardiovascular health). In contrast, regular exercise and a healthy diet have been shown to increase HDL (high density lipoprotein) levels. Their function is to clean cholesterol from arteries for various uses, such as producing bile in the liver. So to sum it all up, having good cholesterol levels ie. high HDL to LDL ratio, is a good indicator that your arteries are in good condition, however it can't tell you for certain that there isn't some atherosclerosis taking place.
