Kidney Transplant Surgery and What You Should Know About It

MS - General Surgery, MBBS
General Surgeon, Guwahati
Kidney Transplant Surgery and What You Should Know About It

Kidney transplant surgery is a surgical procedure where a healthy kidney is used to substitute a diseased one. The healthy kidney is obtained from either a deceased donor or a family member who has a good blood type match with the recipient. Usually, the recipient receives one kidney if the donor is alive and/or both the kidneys from a deceased donor.

Certain reasons for going for a kidney transplant surgery might include:
1. Kidney failure due to diabetes
2. Infections in the urinary tract
3. Autoimmune disorders such as lupus (an autoimmune disorder)
4. Polycystic kidney disorders
5. Obstructions in the kidney
6. Glomerulonephritis, where inflammation occurs in the kidney

Preparation for the surgery
Before the procedure, certain tests are carried out to determine your suitability for the procedure. The tests start with a psychological evaluation followed by blood tests and diagnostic tests to check for health problems. Once the tests are done, you will be placed on the waiting list for kidney recipients. Once the donor is available, you will be asked to get ready for the surgery.

Procedure: The procedure begins with you being administered general anesthesia. An incision is made in the lower part of the stomach, through which the donated kidney is inserted in the body. The next step involves attaching adjacent blood vessels to the kidney so that it has a proper supply of blood. Finally, the ureter of the kidney is linked with the bladder, facilitating normal disposal of bodily wastes. A plastic tube, known as a stent, is placed in the ureter to facilitate urine flow. This is later removed by a procedure called cystoscopy. Once the surgery is completed, the incision is closed by stitches.

Aftercare: After the procedure is completed, you will be given painkillers to ease the pain. Immunosuppressant medications will be prescribed to prevent your immune system from destroying the cells of the donated kidney. Usually, the duration of stay at the hospital post-surgery is about a week.

All About Kidney Stones

All About Kidney Stones

The pair of kidneys that human beings have, forms an important part in the excretory system. They perform specific and important functions. They excrete urine as well as help in eliminating the toxins, which accumulate in the body. Kidney stones occur when the minerals present in the urine without being discharged, keep on accumulating. This gives rise to an extremely painful and uncomfortable situation. Though sometimes the situation gets revoked all by itself, many a times it requires medical intervention.

Some of the symptoms of kidney stones are

  1. Excruciating pain in the back
  2. Pain while urinating
  3. Tendencies to vomit
  4. Dizziness
  5. Fever

However, there are several preventive measures which you can undertake in order to avert the situation. Some of them are:

  1. Drink lots of water: Kidney stone is one of those rare conditions that once occurred, always carry the latent threat of recurring again. Therefore, drinking water in large quantities becomes essential. The more you drink water, the more frequently you would urinate, thereby curbing the chances of the minerals settling in the kidneys.
  2. Avoid certain foods: Food like beets, spinach, those which are replete with calcium oxalate should be avoided in order to minimize chances of having kidney stones.
  3. Change your diet: If you are trying to avoid the eventuality of developing kidney stones, it is imperative that you change your diet a bit. It is recommended that you exclude animal protein as much as possible and assiduously avoid the intake of salt as much as possible.
  4. Take less calcium supplements: Calcium generally is considered a nemesis to your kidneys and is prescribed by doctors to be avoided. However recent research has thwarted this claim. Calcium does not pose a threat to kidneys. However calcium supplements definitely do and therefore, should be avoided.

From yesterday I am feeling a flesh ball just above the anus layer. Feeling little pain while walking and at the time...
This could be a fissure with sentinel pile. Anal fissure improves with general measures but may require surgery. The general measures I would suggest are 1. Take lot of vegetables & fruits in your diet 2. Avoid chillies & spices 3. Take lot of fluids 4. Avoid constipation 5 periodically do anal exercises (alternately contracting & relaxing the buttocks, anal area & upper thighs) 6. Take sitz bath 3-4 times a day. This means sitting in a tub of warm water & doing anal exercises 7. Atleast once get it examined by a general surgeon. Purulent discharge might be due to a fistula. The surgeon will find it on examination.

Drink water throughout the day. Eat fewer oxalate-rich foods. Choose a diet low in salt and animal protein. Continue eating calcium-rich foods, but use caution with calcium supplements. Recommendations based on the specific type of kidney stone include the following: 1. Calcium oxalate stones= reducing sodium reducing animal protein, such as meat, eggs, and fish getting enough calcium from food or taking calcium supplements with food avoiding foods high in oxalate, such as spinach, rhubarb, nuts, and wheat bran 2. Calcium phosphate stones= reducing sodium reducing animal protein getting enough calcium from food or taking calcium supplements with food 3. Uric acid stones= limiting animal protein. Prostatitis treatments vary depending on the underlying cause. They can include: Antibiotics. This is the most commonly prescribed treatment for prostatitis. Your doctor will base the choice of medication on the type of bacteria that may be causing your infection. If you have severe symptoms, you may need intravenous (IV) antibiotics. You'll likely need to take oral antibiotics for four to six weeks but may need longer treatment for chronic or recurring prostatitis. Alpha blockers. These medications help relax the bladder neck and the muscle fibers where your prostate joins your bladder. This treatment may lessen symptoms, such as painful urination. Anti-inflammatory agents. Nonsteroidal anti-inflammatory drugs (NSAIDs) may make you more comfortable. Prostate massage. This is done by your physician using a lubricated, gloved finger — a procedure similar to a digital rectal exam. It may provide some symptom relief, but doctors disagree about how effective it is. Other treatments. Other potential treatments for prostatitis are being studied. These treatments include heat therapy with a microwave device and drugs based on certain plant extracts
