I have an acute middle back pain. What could be the cause and what should I do to relieve me of this pain?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Acute middle back pain with sudden onset could be due to small disc protrusion at the back but it can also be due to other reasons as well. If you have only a localized kind of pain with no other associated symptoms like radiating leg pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc let us observe you for a period of 2-3 weeks by which a lot of patients actually improve, if your pain still persists, then we will get some investigations done. Calf stretching exercise or heat fermentation with warm water and apply pain relief ointment avoid long sitting 2nd option physiotherapy treatment wax therapy and ultrasonic therapy. Wear sole inside of shoes seems pf.

Get a cbc blood count and also rule out if there is a deficiency of calcium or vitamin d. If everything is normal and the pain persists then you can take homoeopathic medicines.

You can feel tired due to lack of essential nutrients, not getting enough sleep, a weak immune system, excessive drinking, skipping meals, emotional stress, and too much physical labor. Shoulder pain and injuries

The shoulder is a ball-and-socket joint that has three main bones: the humerus (long arm bone), the clavicle (collarbone), and the scapula (also known as the shoulder blade). These bones are cushioned by a layer of cartilage

The shoulder joint is a very mobile joint which makes it very susceptible to injury. It moves the shoulder forward and backward. It also allows the arm to move in a circular motion, and to move up and away from the body.

Shoulders get their range of motion from the rotator cuff. The rotator cuff is made up of four tendons. Tendons are the tissues that connect muscles to bone. It may be painful or difficult to lift your arm over your head if the tendons or bones around the rotator cuff are damaged or swollen.
You can injure your shoulder by performing manual labor, playing sports, or even by repetitive movement. Certain diseases can bring about pain that travels to the shoulder. These include diseases of the cervical spine of the neck, as well as liver, heart, or gallbladder disease.
You're more likely to have problems with your shoulder as you grow older. It is especially common after age 60. This is because the soft tissues surrounding the shoulder tend to degenerate with age.
Most shoulder pain problems are:

  •   tendonitis/bursitis, injury or instability of the joint, or arthritic disease.
  •   rotator cuff syndrome is a strain or sprain or tear of the tendons that make up the rotator cuff.
  •   rarely, tumors, infections, and nerve damage cause shoulder pain.
  •   much neck pain is caused by soft tissue damage due to injuries such as whiplash or wear and tear due to overuse.
  •   damage to spine joints and bones, such as cervical disk degeneration, arthritis, or traumatic injury can cause severe pain and disability.
  •   fibromyalgia (whole body pain syndrome) can contribute to neck and upper back pain. Poor posture while performing everyday activities can also cause significant neck pain. Rarely, infections or tumors will cause neck pain.
  •   pinching a nerve in the neck or shoulder, or breaking a shoulder or arm bone, are also causes of pain.
  •   a frozen shoulder is when tendons, ligaments, and muscles stiffen and become difficult or impossible to move.
  •   a dislocated shoulder is when the ball of the humerus pulls out of the shoulder socket. An injury due to overuse or repetitive use can cause injury

Hello.... You can try physiotherapy. Most cases do recover very good with physiotherapy. You can also avoid surgery. Just meet a physio nearby,follow all the instructions. You will have to continue therapy for a few days.

Fracture is a medical emergency.It can't be treated on line. Kindly go to a hospital. For pain you may take : Paracetamol 250 mg SOS.

Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Sir I have pain in back from 2 years I have mri report but no nuro problem but I lift some more weight so Dr. Say stretch in muscles. Sir there is continuous pain in back and when pain is more then pain in leg bone difficult to walk sometimes. I consulted Dr. Again he advised for exercise. what is your opinion ?

Dear patient you've written very less regarding your problem though but try doing the isometrics of the back muscles and active bridging exercises. Initiate bridging bilaterally without resistance and progress to resisted bridging bilaterally followed by unilateral resisted bridging. Cat'n'camel exercise will help you to self mobilize the spine which will surely help reducing the pain. Avoid core strengthening and weight bearing exercises of legs. And do use ls belt all the time when you" re walking or standing.
