Post-Menopausal Bleeding

MD - Obstetrtics & Gynaecology
Gynaecologist, Bangalore
Post-Menopausal Bleeding

Postmenopausal bleeding
Post menopausal bleeding is vaginal bleeding that happens at least 12 months after your periods have stopped.
About 4 % of women suffer with the above problem. It is not normal to bleed after menopause and it has to be investigated.

Causes of post menopausal bleeding
Most common cause of post menopausal bleeding is atrophic vaginitis and atrophic endometritis. After menopause due to decrease in the level of harmones in the body there can be thinning and inflammation of the inner lining of the uterus.
Other causes of post menopausal bleeding are
polyps they refer to the extra growths of the inner lining of the uterus as well as the neck of the uterus(cervix).
PMB can also be because of thickening of the inner lining of the uterus, also called as hyperplasia.it can be because of extraneous harmones or obesity.
Less than 10% of the woman who suffer from PMB may have cancer of endometrium ,cervix and vagina.

Diagnosis
1)Pelvic examination
your doctor will do an internal examination to examine the cervix and do a papsmear if it is due.
2)USG scan is done to note down the size of uterus and the thickness of the endometrium.based on these reports the doctor might decide about the further tests.
3)Endometrial biopsy a small tube will be inserted into the uterus and cells are collected and sent to lab to note if there is any thing abnormal .This is an office procedure.and ususlly does not require admission
4)Hysteroscopy it is a procedure where a small camera is put inside the uterus and a targetted biopsy taken from the suspicious area.

Treatment
The treatment of PMB depends upon the cause.
If endometrial atrophy was the reason it can usually be traeated with estrogen creams and pessaries
Cervical and endometrial polyps require surgery to remove them.Hysteroscopic polypectomy refers to the surgery where in the polyp is removed under the guidance of the camera which is inserted into the uterus.
Endometrial hyperplasia can be treated with progesterone tablets or progesterone impregnated intrauterine devices.
Endometrial cancer will have to be treated with surgery for the removal of uterus and ovaries. Some patients may require chemo or radiotherapy as the stage of cancer may demand.
All in all postmenopusal bleeding doesnt mean the patient has cancer 90 percent of the time it is because of the benign causes but it should not be ignored but investigated. Its betTer to be safe than sorry.

Three Ways to Protect Your Knees

M.S. Orthopedics, MBBS
Orthopedist, Ahmedabad
Three Ways to Protect Your Knees

The knee is known as the largest and one of the most complex joints in the human body, and also one of the most vulnerable, as it supports the weight of the entire body. The knee joint is made up of several elements including the knee cap, meniscus, connective tissue, tendons, ligaments, cartilage, and muscles. Damage to any part of the knee can cause chronic pain. So how can you protect your knee from damage and injury? Take a look at these three points!

Dealing with Load: Weight is a big one. Carrying around extra pounds and kilograms can be detrimental for your knees in the long run. Your knee is not equipped to handle the excessive weight. It already supports three to five times your weight when you get up and walk around or indulge in some mild jogging. One must be careful of those extra pounds to take the load off the knee and keep it from falling prey to damage.
Exercise: Moderate to intense exercise is prescribed for one and all. This, of course, depends on one's overall health and age. If you are already suffering from joint pain, then you may want to go easy on the exercise with a focus on building muscle strength and foundation. If you are healthy, then some amount of daily exercise as a routine will keep those knees in prime working condition and well lubricated as well. Inactivity and a sedentary lifestyle can affect the knees very badly.
Do not Ignore: Warning signals such as chronic or acute knee pain should not be ignored. Remember to see a doctor or an orthopaedic specialist to find out if you have contracted an infection or if you may have suffered a fracture due to an injury. Also, it is important to take a bone density test regularly, especially after the age of 40 or for women going through menopause, as this will point to any deficiencies and even warn you in case there is any trace of osteoporosis.

Rest, exercise, and food should be kept in fine balance for a healthy body. These are a few measures that one can take to keep knee pain at bay. Yet, it is also advisable to speak with a doctor at length regarding measures to protect your knees.

Remember to ask us for a private consultation today!

Hi Sivasen. Normally periods begin within 5-7 days after stopping progesterone tablets. Please mention which medicines you were taking and for how long so we can help you specifically. Incase you have not responded to progesterone we can add estrogen also and that you solve the problem. All the best

Conception is possible with single patent tube. But before planning a pregnancy you should meet a gynaecologist and get your tests done to ascertain the cause of repeated early abortion. Once the cause of abortion is known you will have to take some medicines before conception so that pregnancy continued smoothly.

Even if semen flows out, there are millions of sperms which swim up in the few drops that are left in vagina. 6 months is a short while, try for an year atleast, practise midcycle coitus.

This is the time when every woman goes through a change, due to decline in hormones and periods gradually stop, risk of heart attack is more, bones start getting hollow due to depletion of calcium, eyes weak, some amount of mental changes like depression, gain in weight. Get an ultrasound abdomen done, basic tests for heart like ecg, halter test and blood test for sugar, lipid profile done. Let her work like a normal woman but no sleeping tabs, if she is going for walks, doing yoga, taking a healthy diet, her weight will not increase and she will sleep well. Do write back the details of gyne problems if any!

Treating ovarian cysts

Royal College of Obstetricians and Gynaecologists (MRCOG), MS, MBBS
Gynaecologist, Ghaziabad
Treating ovarian cysts

All women have two ovaries located on either side of the uterus. They are fundamental for the process of child birth. However ovarian cysts have emerged as major problems which most women suffer from. In this, small pockets filled with fluids develop on the surface or within the ovaries. Though they are generally harmless, often times disappearing without any treatment, they do have the potential to create complications during child birth. Treatment of ovarian cysts depends on the size and the quantity of the cysts.

Some of the treatments available are-

1. Patience and a careful watch
In case medical examinations reveal tiny cysts, patience can be the key word. If you did not suffer from perceptible symptoms and the cysts were revealed only through tests, you must follow it with regular visits to the doctor. Few medications and regular ultrasound examinations are all you need in such a case.

2. Contraceptives
Birth control pills or contraceptives may be sometimes prescribed to you as possible medications to treat the cysts. Regular administering of these medicines deters the possibility of further development of cysts.  They also benefit greatly in reducing the possibilities of suffering from ovarian cancer in the future.

3. Operations
In case the tests reveal large cysts, operations or surgical removal of them may be the only possible solution.  Operations are generally suggested if the cysts have persisted for a long time and are accompanied by terrible pain in the lower abdomen.

4. Ovarian cystectomy
This is performed when the ovaries are kept intact and only the cysts are carefully removed.

5. Oophorectomy
In this form of surgery, only the ovary that is affected with the cysts is removed, while the other is kept intact. Both the ovaries might also have to be surgically removed if they are found to be affected.

6. Hysterectomy and subsequent removal of fallopian tubes and uterus
This is done when the cysts developed have the potential of becoming cancer us. In such a situation, an extreme measure is adopted with the complete removal of the ovaries along with the uterus and fallopian tubes. This is done in order to prevent the subsequent spreading of cancer. If you wish to discuss about any specific problem, you can consult a gynaecologist.

Hello. Sometimes menses get delayed due to increased stress and physical activity, nutritional deficiency or some underlying medical condition. So, don't worry, but of course if you are having active sexual life, get pregnancy test done always!

My wife age is 35 and my age 24. we are love married I have doubt she pregnant or not she have this possible to get baby.

You're most likely to get pregnant if you have sex within a day or so of ovulation (releasing an egg from the ovary). This is usually about 14 days after the first day of your last period. An egg lives for about 12-24 hours after it? s released. For pregnancy to happen, the egg must be fertilised by a sperm within this time. If you want to get pregnant, having sex every couple of days will mean there? s always sperm waiting in the fallopian tubes to meet the egg when it? s released. Any queries inform me.

Hello, PCOS or pcod s a problem in which a woman's hormones are out of balance. It can cause problems with your periods and make it difficult to get pregnant. PCOS also may cause unwanted changes in the way you look. If it isn't treated, over time it can lead to serious health problems, such as diabetes and heart disease. The most common symptoms are: Acne. Weight gain and trouble losing weight. Extra hair on the face and body. Thinning hair or even hair loss on scalp. Irregular periods. Often women with PCOS have fewer than nine periods a year. Some women have no periods. Others have very heavy bleeding. Fertility problems. Many women who have PCOS have trouble getting pregnant (infertility), depression. Regular exercise, healthy foods, and weight control are the key treatments for PCOS. Treatment (hormonal) can reduce unpleasant symptoms and help prevent long-term health problems. Try to fit in moderate activity and/or vigorous activity often. Brisk Walking, jogging, dance or aerobics are great exercise that most people can do. Eat heart-healthy foods. This includes lots of vegetables, fruits, nuts, beans, and whole grains. It limits foods that are high insaturated fat, such as meats, cheeses, and fried foods. Most women who have PCOS can benefit from losing weight. Even losing 5 kg may help get your hormones in balance, regulate your menstrual cycle and can make you ovulate.
