What causes Hashimoto's thyroiditis, and what is the best method of treatment? Can iodine help this condition?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
, hashimoto's thyroiditis is an auto immune disease, where there is lymphocytic infiltration of thyroid gland and anti bodies against thyroid peroxidase enzyme are produced by our own autoimmune cells, which destroy thyroid secreting cells. This leads to hypothyroidism. One of the cause is said to be excess consumption of salt through food. (iodinated salt). This is the major cause of hypothyroidism in usa. In india it is less common. Women get it more often. A person with ht usually has other family members also having some or the other autoimmune condition. Some of them may also have another autoimmune disease too. Only treatment is to give thyroxine hormone for life time. Iodine has no role in treating the condition.

Hello Mr. Lybrate-user. Only your hba1c average blood glucose, triglycerides and alp are not in range. Remaining all tests are almost normal and nothing to fear about. You need to start insulin as your hba1c is not in the limit. Actually maintaining 12 years on oral diabetic drugs is appreciative. Average blood sugar is 194 which need to be controlled immediately. It may be the reason for raise in tgl. And you need to test for ggt as your alp has raised. Do some regular exercise like walking, jogging are any game which ever you feel better. And consult a good physician for good diabetic care. Contact if need further assistance.

Hye, thanks for the query. For a diabetic it is best advised to have a diet planned as per the sugar levels, lifestyle and history. Though you can follow some guidelines - the sugar levels will fluctuate depending on what you ate or did not eat, stress levels and activity levels. You have to try and manage a balanced carb intake. Have small frequent meals. Do not fast for long hours. You have to eat 3 main and 3 mid meals. Your diet must include whole grains, seasonal fruits, vegetables, small amounts of nuts and dairy and pulses or dals or lean meats like chicken or fish and egg. Starchy vegetables need to be planned into the diet. Avoid a very heavy meal at one time. Avoid fruit juices, aerated drinks or sugarcane juice, honey, jaggery or sugar. Avoid junk foods. Exercise and stay active. Avoid exercise if sugar levels are above 250 mg/dl. There are many exercises you can do even in this position. Do not attempt to try out too many remedies as you could go into hypoglycaemia. Sleep well. It's all a matter of discipline.

According to Ayurveda mixing of honey with earn or hot stuffs is a contraindicated combination.. Such foods can give rise to severe metabolic dysfunction .. It is a misconception to take honey with warm water for weight loss .. Hence it is advised to avoid use of such combination and drink water to ur body's need and don't over load

Dear Sir,
I am suffering from Diabetes from last 5-6 years, but about 3-4 months I feeling very weakness, nervousness.
Jamun is a very good ayurvedic source to keep diabetes in check and so is methi dana and bitter gourd. 1. Mix ½ tsp Turmeric Powder + 1 tsp Amla Powder + ½ tbsp Methi Seeds Powder in half glass of water and take daily in the morning on empty stomach. You can also grow methi sprouts and eat them. 2. Remove the skin and seeds of 4-5 bitter gourds. Crush them to a make a paste. Press this paste on a sieve an extract the juice. Drink this juice on an empty stomach every morning. 3. Boil 1 L of water. Let it simmer for 20 min on low flame. While simmering, add 3 tsp of cinnamon powder. Strain the mixture. Drink entire amount of this water everyday. 4. Take a tablespoon of jamun powder everyday.

Hello doctor, Though my thyroid level is normal, my weight does not reduce and I have got acne too, what should I do?

MBBS, Diploma in Diabetology

Hello doctor,
Though my thyroid level is normal, my weight does not reduce and I have got acne too, what should I do?
This could be due to pcos (excessive weight) and insulin resistance. Get your insulin fasting and hba1c test done. Diet and exercise is key to loose weight. Consult any diabetologist for weight loss medication if you are not able to loose weight even after 3 months of diet and exercise.

Is ime 9 tablets is best for diabetes or specially sugar please give me answer shortly, thank you.
Mr. lybrate-user, The particular tablet which is being advertised heavily in TV, is just a combination of some herbal remedies, based on encient ayurvedic texts. There is very little scientific evidence to support. Unless a patient is on modern medicine to treat diabetes or to control blood glucose, there is no way to control it. The medicine mentioned by you can be at best can be taken as a supportive therapy. I have absolutely no belief on the various claims made. Thanks.

Precautions and daily care for diabetes type 2.which kind of medicines works well.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Precautions and daily care for diabetes type 2.which kind of medicines works well.
Hello, If one has diabetes then most important is to start appropriate treatment with either oral anti-diabetics or Insulin depending upon the severity of hyperglycemia, plus strict dietary control and regular exercise for at least 1 hour a day. Other precautions are- avoid sweets, deep fried foods, junk foods, pastries, other bakery items, sweetened aerated drinks (colas) and refrain from alcohol and smoking. Dietary intake should match the daily needs based on ideal body weight, extent of exercise and blood glucose levels. As regards medicines there are a large number of them, but the choice depends on the specific needs of a patient. Unless all the details are made known - like fasting, PP glucose levels, HbA1c%, height, weight, extent of daily exercise and dietary pattern, there is no way to recommend any medication. Plus in this communication we are supposed to give only general suggestions/guidelines, but no prescriptions. For that one has to come in a private conversation. Thanks.

Hypoglycemia: Going Lower Is Not Always Good

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Hypoglycemia: Going Lower Is Not Always Good

Hypoglycemia, commonly referred to as low blood sugar is the condition of the body, when the level of blood sugar falls down below the normal level. This accounts for clumsiness, feeling shaky, feeling confused, losing consciousness, serious seizure. Severe lowering of blood sugar may cause death. You are likely to feel exhausted and very hungry. The symptoms usually show quite quickly.

Hypoglycemia is commonly caused due to diabetes medicine like insulin, sulfonylureas or biguanides. Starving for long hours, over straining the body or extreme alcohol consumption may lead to hypoglycemia. Failure of the kidneys, liver diseases, metabolism disorders also lead to hyperglycemia.

Fall in the the blood sugar level must be taken very seriously and never be ignored. If you feel the symptoms of lowered blood sugar, it is advised that you immediately take a sugar test. You must also indulge in high calorie food items such as sweets or chocolates. You need to increase the glucose intake, so that the blood-sugar level rises up.

Here are some steps you should follow to control hyperglycemia:

  1. Keep away from the practices, which lead to hyperglycemia: The best way of keeping away from hyperglycemia is by taking all measures, which would avoid the condition to develop. The medical schedule of diabetic patients should be closely monitored and optimized by the patient's caregivers. Their lifestyle should be kept under check. It should be ensured that the patients do not skip meals. Insulin should be avoided as much possible.
  2. Discuss problems with the patient: Patients and their guardians must be properly educated about the disease and what important steps they should take, in spite of all kinds of prevention treatments they take. Teach them about the symptoms and the steps to be taken in case of any emergency. Use of glucose tablets should be encouraged. They must know that the process is ongoing. Every patient is different from another and hyperglycemia differs from person to person.
  3. Use glucagon: Glucagon is basically a hormone, which stimulates the liver to release pre-stored glucose all over your bloodstream in case your blood-sugar level falls down steadily. Glucagon is usually injected, and kits are available. Prescribed by doctors, glucagon can be used to treat a person who has fallen unconscious after a fatal reaction due to insulin. You should consult an expert before buying a glucagon kit.

Hypoglycemia is a serious problem. Lowering or fall in your blood sugar level must never be ignored, and measures must be taken to regain your sugar balance immediately.

Do it after 2-3 days without medicine and see again. If it comes normal do it again after a week and if consistently both the samples, fasting and post meal remain normal, then stop medicine but continue dietary restrictions and exercises. Afterwards do monthly follow up checks and act as per reports.
