Hi, Is your father having any other symptoms Ina edition to weight loss like nausea, vomiting, stomach pain, constipation, fever, night sweats, any lumps in the body, any pain in the body, jaundice, urine or kidney problems, any skin problems etc? Weight loss can be due to many reasons. A few of them are mentioned below 1) general debility due to poor intake of food due to old age 2) infections like TB etc or any other chronic infections 3) diabetes 4) kidney failure, heart failure, liver failure etc 5) gastritis Lisa ding to nausea and lack of appetite 6) any tumours in body etc. All of the above would result in other symptoms in addition to weight loss. Only age related malnutrition in initial stages would give rise to weight loss without their symptoms. So I would suggest you to take him to a good general physician and get him throughly assessed. Hope this helps you. Thank you.

I am 37 years male. I am suffering from uric acid problem and high BO. How can I reduce it?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, high uric acid in the blood may be present in many persons, but it may not cause any difficulty. But in some it leads to joint pains and gouty arthritis. In such situations it is necessary to reduce the uric acid. It is better to reduce overall fat intake as fat reduces uric acid excretion. Liberal intake of water helps excretion of uric acid. Restrict or preferably eliminate alcohol intake. Avoid certain sea foods - sardines, shell fish, scallops, herring, trout, haddock, tuna, anchovies, salmon, mussels, mushrooms makeral. Whole pulses like rajmah, chana, chole and masur. Others are good. Avoid red meat, organ and glandular meat, avoid excess salt intake, avoid highly processed foods, salted nuts, papad and pickels. Avoid bakery products. Sweetened drinks. Eat fresh vegetables, berries, orange, cabbage, bananas, tomatoes, potato and other vitamin c containing foods. Low fat milk products are good and complex carbohydrates (whole grain flour). For controlling bp also restrict salt intake. If reading is high start on antihypertensive drug.

I want to know about the uric acid increase problems.

MD - Internal Medicine, MBBS
Endocrinologist,
Firstly, we have to understand where uric acid comes from. Animal cells (including human cells) have structures called DNA, which reside with the nucleus of the cell. This DNA is broken down into its constituents, called amino acids, when a cell dies through the action of the immune system. Some of these amino acids are called purines, which in turn are converted into other chemicals. Our body recycles some of these chemicals with those left over proceeding to become uric acid, which is in some ways a waste product that is then excreted by our kidneys into the urine. It is a very complicated process but with this concept in mind you may understand why certain diseases are associated with high levels of uric acid. For example, if a person has a high burden of dying cells, such as occurs in certain cancers or in those undergoing chemotherapy for the treatment of cancer, then more DNA is broken down to purines, resulting in excess uric acid production. Fig 2. Xray of a finger joint with severe destructive gout. Fig 2. Xray of a finger joint with severe destructive gout. Conversely, when the uric acid cannot be efficiently excreted, such as is the case with many kidney diseases or due to many medications that interfere with this process of excretion. This is particularly a concern with diuretics, which remove water from our body at the expense of other products such as uric acid. Rarely, a person is born with a defective step in uric acid metabolism, whereby gout develops at a young age, sometimes associated with a very strong family history of early and aggressive gout. When the uric acid level is high in the circulation it is termed Hyperuricaemia. This situation is also strongly associated with obesity. It is believed that fatty tissue enhances the production of uric acid. Hyperuricaemia is part of the ‘metabolic syndrome’, which is associated with an increased risk of heart attacks and strokes. We also obtain purine amino acids from our diet. It is estimated that 10-15% of our purine load is dietary in origin. Foods that are high in purine include offal, seafood (in particular crustaceans), red meat, and alcohol.

Mr. Lybrate-user, looking at the sugar levels, it is clear that insulin dosage is not adequate. There needs a better titration of insulin dosage. If need be a new drug can be added like sglt - 2 inhibitor. According to me first concentrate on fasting glucose level, keep a target value of 110 mg. If level is much higher, then increase the night time insulin by 2 units every 3 rd day, till the target level is achieved. Then target post lunch level, and titrate insulin dosage similarly. Plus can look at adding above mentioned drug. Those measures along with diet control and exercise should help in controlling blood sugar levels. Thanks.

Do not take medicine of ayurvedic and homoepathic together. Home Remedies of Acidity: Avoid all types of fast food, non-vegetarian food and oily/spicy food. Take as much fruit as possible. This will help to increase alkaline in the body, neutralizing the acidity affects. Banana, Water Melon and cucumber are few recommended fruits which are proved helpful in acidity. Drink sufficient water (At least 3 ltr). Take water more frequently. Almond helps in quick control over heart burn. Whenever you feel like heart burn, take few pieces of Almonds. Jaggery (gur) is a proven home remedy for acidity. Put one piece in mouth and suck it slowly. Green coconut water is very helpful in acidity. Sleep sufficient. Do not work till late night. Fresh mint juice helps in reducing the side effects of acidity. You can have a glass of mint juice after food for a fast relief. Take a small piece of raw garlic; this is a very good digestive element. But don’t take overdose, or else it will have negative effect on acidity.

high cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Avoid fast foods, oily foods, junk foods, alcohol, smoking, fatty foods, non-veg.,avoid stress, take green veg., whole grain.garlic,. Take anti cholesterol medicines .

Lybrate-user, with bilirubin 1.7 mg/dl, there is presence of jaundice. It is not such a serious condition. He should continue taking normal food. Since he is also has diabetes, the precautions being taken in diabetes are good enough. He should avoid sweets, deep fried foods, junk foods, pastries, bakery items, items made out of maida, sweetened aerated drinks (colas), sweet corn syrup containing products, highly processed food products, red meat, high fat containing milk products. Should consume more of green leafy vegetables, salads and permitted fruits. Refrain totally from alcohol and smoking. Total diet should provide exact calories needed on the basis of his ideal body weight, exercise done daily and blood sugar levels. Thanks.
