Last year my a1c was 7.3 and now as recent report it is 6.6. My Dr. Has not prescribed any medicine but asked me to do regular walk and exercise of one hour which I do without fail. Except on occassion once in 4/5moths I do not take sweets and no sugar in tea. At present my fasting sugar is 105/112 and 2 hrs after meal it comes 140/155. Random also 105/111 what is your suggestion.

PGDD, RD, Bachelor of Home Science
Dietitian/Nutritionist, Mumbai
for a diabetic it is best advised to have a diet planned as per the sugar levels, lifestyle and history. Though you can follow some guidelines - have small frequent meals. Do not fast for more than 2-3 hours. Avoid a very heavy meal at one time. Avoid fruit juices, aerated drinks or sugarcane juice, honey, jaggery or sugar. Fruits should be had as a mid meal. Avoid junk foods. Exercise and stay active. Do not attempt to try out too many remedies as you could go into hypoglycaemia. Sleep well. It's all a matter of discipline.

Masturbation is individual choice. You seems to be in good health so do have your own personal healthy sexual choices. Nothing wrong with masturbation unless it's in excess.

General Physician,
Dr. iam facing with sexual problem as I get to sex I got early discharge pls help.
Ejaculation of semen during sexual intercourse before or immediately after penetration. Premature ejaculation occurs when a man ejaculates sooner during sexual intercourse than he or his partner would like. Premature ejaculation is a common sexual complaint. Estimates vary, but as many as 1 out of 3 men say they experience this problem at some time. As long as it happens infrequently, it's not cause for concern. The primary symptom of premature ejaculation is the inability to delay ejaculation for more than one minute after penetration. However, the problem may occur in all sexual situations, even during masturbation. Premature ejaculation can be classified as lifelong (primary) or acquired (secondary). Lifelong premature ejaculation occurs all or nearly all of the time beginning with your first sexual encounters. Acquired premature ejaculation has the same symptoms but develops after you've had previous sexual experiences without ejaculatory problems. Many men feel that they have symptoms of premature ejaculation, but the symptoms do not meet the diagnostic criteria for premature ejaculation. Instead these may have natural variable premature ejaculation, which is characterized by periods of rapid ejaculation as well as periods of normal ejaculation. Psychological causes Some doctors believe that early sexual experiences may establish a pattern that can be difficult to change later in life, such as: • Situations in which you may have hurried to reach climax in order to avoid being discovered • Guilty feelings that increase your tendency to rush through sexual encounters Other factors that can play a role in causing premature ejaculation include: • Erectile dysfunction. Men who are anxious about obtaining or maintaining an erection during sexual intercourse may form a pattern of rushing to ejaculate, which can be difficult to change. • Anxiety. Many men with premature ejaculation also have problems with anxiety — either specifically about sexual performance or related to other issues. • Relationship problems. If you have had satisfying sexual relationships with other partners in which premature ejaculation happened infrequently or not at all, it's possible that interpersonal issues between you and your current partner are contributing to the problem. Biological causes A number of biological factors may contribute to premature ejaculation, including: • Abnormal hormone levels • Abnormal levels of brain chemicals called neurotransmitters • Abnormal reflex activity of the ejaculatory system • Certain thyroid problems • Inflammation and infection of the prostate or urethra • Inherited traits • Nerve damage from surgery or trauma (rare) Common treatment options for premature ejaculation include behavioural techniques, topical anaesthetics, oral medications and counselling. Keep in mind that it may take a little time to find the treatment or combination of treatments that will work for you. Behavioural techniques In some cases, therapy for premature ejaculation may involve taking simple steps, such as masturbating an hour or two before intercourse so that you're able to delay ejaculation during sex. You may try avoiding intercourse for a period of time and focusing on other types of sexual play so that pressure is removed from your sexual encounters. You can also try diverting your mind before ejaculation is ready to come and proceeding after a brief break, thereby prolonging duration of intercourse / action The pause-squeeze technique You and your partner can use of a method called the pause-squeeze technique. This method works as follows: 1. Begin sexual activity as usual, including stimulation of the penis, until you feel almost ready to ejaculate. 2. Have your partner squeeze the end of your penis, at the point where the head (glans) joins the shaft, and maintain the squeeze for several seconds, until the urge to ejaculate passes. 3. After the squeeze is released, wait for about 30 seconds, then go back to foreplay. You may notice that squeezing the penis causes it to become less erect, but when sexual stimulation is resumed, it soon regains full erection. 4. If you again feel you're about to ejaculate, have your partner repeat the squeeze process. By repeating this as many times as necessary, you can reach the point of entering your partner without ejaculating. After a few practice sessions, the feeling of knowing how to delay ejaculation may become a habit that no longer requires the pause-squeeze technique. Topical anaesthetics Anaesthetic creams and sprays that contain a numbing agent, such as lidocaine or procaine, are sometimes used to treat premature ejaculation. These products are applied to the penis a short time before sex to reduce sensation and thus help delay ejaculation. A lidocaine spray for premature ejaculation is available Although topical anaesthetic agents are effective and well-tolerated, they have potential side effects. For example, some men report temporary loss of sensitivity and decreased sexual pleasure. In some cases, female partners also have reported these effects. In rare cases, lidocaine or procaine can cause an allergic reaction. Oral medications Many medications may delay orgasm. Although none of these drugs is specifically worthwhile as people believe, approved medications may be prescribed for either on-demand or daily use, and may be prescribed alone or in combination with other treatments to treat premature ejaculation, some are used for this purpose, including antidepressants, analgesics and phosphodiesterase-5 inhibit

Most people use the term piles if they see any bold in motion, which may be just mild from minor injury in the excretion pathway resulting from constipation, or fistula or gastroenteritis and see in blood is NOT piles, in all cases. Piles are haemorrhoids that become inflamed. Haemorrhoids are masses, clumps, cushions of tissue in the anal canal - they are full of blood vessels, support tissue, muscle and elastic fibers. Piles can be of various sizes and may be internal (inside the anus) or external ones (outside the anus). Typically, internal piles occur from 2 to 4 cm above the opening of the anus. External piles (perianal hematoma) occur on the outside edge of the anus. The internal ones are much more common. Symptomatic haemorrhoids affect at least half the population at some time in their lives before the age of 50. In the majority of cases, piles are effectively treated with over-the-counter medications, a good fluid intake, and by following a diet high in fibre. In severe cases, the piles may have to be surgically removed. About 10% of patients who go and see their doctor about piles eventually require surgical intervention. Internal haemorrhoids are classified into four grades. Grade 1 - there are small inflammations, usually inside the lining of the anus. They are not visible Grade 2 - larger than grade 1 haemorrhoids, but also inside the anus. When passing a stool, they may get pushed out, but return unaided Grade 3 - often called 'prolapsed haemorrhoids'; these appear outside the anus. The patient may feel them hanging out. They can be pushed back in if the patient presses with their finger Grade 4 - these cannot be pushed back in and need to be treated by a doctor. They are large and stay outside the anus all the time. External haemorrhoids are called perianal hematoma. These are small lumps that are located on the outside edge of the anus. They are very itchy and can be painful if a blood clot forms inside (thrombosed external haemorrhoid). Thrombosed external haemorrhoid requires medical treatment straight away PILES OCCUR when the blood vessels around the anus and in the rectum will stretch under pressure and may swell or bulge. Inflamed veins (haemorrhoids) can develop when pressure increases in the lower rectum. This may be due to: Chronic constipation9 Chronic diarrhea10 Lifting heavy weights11 Pregnancy12 Straining when passing a stool. 13 The tendency to develop haemorrhoids may also be inherited. The risk of developing piles grows with age Treatments for piles In the majority of cases, piles resolve on their own without the need for any treatment. Treatments can help significantly reduce the discomfort and itching that many patients experience. Diet - piles can be caused by too much straining when doing bowel movements, which is the result of constipation. A change in diet can help keep the stools regular and soft. This involves eating more fiber, such as fruit and vegetables, or switching your cereal breakfast to bran. Water is the best drink, and the patient may be advised to increase his/her water consumption. Some experts say too much caffeine is not good. Simple things you can do yourself to help prevent piles: 10 Try not to strain when you go to the toilet Avoid laxatives Exercise. Ointments, creams, pads and other OTC medications - there are some over-the-counter (OTC) medications which help soothe the redness and swelling around the anus area. Some of them contain witch hazel, hydrocortisone, or some other active ingredient which can relieve symptoms of itching and pain Surgery - used for particularly large piles, or grades 3 or 4 haemorrhoids. Generally, surgery is used if other procedures were not effective. Sometimes surgery is done on an outpatient basis - the patient goes home after the procedure.

If you are having thoughts about sex, so be it. What is wrong in it? yes, if those thoughts disturbs your day to day tasks, you should address them. Symptoms And Prevention Of Lice!

Symptoms And Prevention Of Lice!

A head louse is an insect that lives on the human scalp and feeds on human blood. The female louse lays eggs in the head and makes sure the eggs are attached to the hair shaft. It takes around eight days for the egg to hatch. The louse takes another 11 days to mature into an adult louse.

Causes:
The primary cause of head louse is poor personal hygiene. Other causes due to which a person can be infected by lice are:

  1. Using combs and towels that are infested.
  2. Coming in contact with another person who has this condition.
  3. Sharing the bed with a person who is affected by lice.

Symptoms:

When a person is affected by this condition then the following symptoms are observed:

  1. The saliva of the lice causes an allergic reaction in the scalp which leads to constant itching. It can occur in various places such as ears, scalp and the neck. It may not occur initially, but it tends to occur after a few weeks.
  2. Constant scratching can lead to formation of small red bumps on the skin. A bacterial infection usually occurs from constant itching.
  3. It may lead to symptoms of dandruff in the scalp.
  4. Presence of various insects in the scalp.

Prevention of Lice:

Lice can be prevented by using medicated shampoos and conditioners to wash the hair. The shampoos are usually based on a chemical called pyrethrin. This compound is toxic for lice and helps in getting rid of it.

You can also opt for the following lifestyle changes to prevent lice:

  1. You should avoid sharing your hairbrush with others.
  2. In case you have long hair, tie it in buns or ponytails.
  3. Add tea tree oil to your shampoo before using it.
  4. Keep the scalp clean to prevent infestations.
  5. Avoid sharing pillows and clothes.
