Neuro Pysiotherapy Exercises/stretches in Spastic Stages of Stroke/hemiparalysis

MPT, BPT
Physiotherapist, Noida
Neuro Pysiotherapy Exercises/stretches in Spastic Stages of Stroke/hemiparalysis

TREATMENT IN SPASTIC STAGES:

In case of spasticity the therapist should try to achieve the following aims:·

First normalize the tone.

Development of normal functional pattern.

Prevention of contracture and deformity.

Train the patient to be functionally independent.

Achieve highest possible physical security for the patient.

Spasticity in the muscle can be reduced by the following methods:

Gentle rhythmic passive movement.

Sustained gradual stretching either manually or by using splints.Prolonged icing over the spastic muscle bulk for about 15 to 20 min.

Faradic stimulation to the weak antagonist muscles can reduce the

Spasticity of the agonist muscle by the principle of reciprocal inhibition.

Reflex inhibiting postures or patterns.

Biofeedback: This can be used to relax spastic muscle as well as to activate its antagonist.

EXERCISES IN SPASTIC STAGE:

EXERCISES IN LYING:

Scapular movement: The patient scapula should be mobilized passively and also the patient should be asked to perform protraction and elevation movement of the scapula.Touching the opposite shoulder: The patient is trained to take his arm from extension, abduction and external rotation position towards his opposite shoulder into flexion , adduction, supination and external rotation.Touching the head: The patient is trained to touch the head by maintaining external rotation and supination. The Therapist maintains the hand in extension of the fingers with the thumb in abducted position and then she moves the hand into abduction and elevation maintaining the elbow in extension. This exercise can be progressed to active the as the patient learns to control the movement.Elbow extension with shoulder in 90 degree of flexion. The patient is then trained to maintain the arm  in space in different directions. This exercise will help the patient in developing good control of the upper limbs and also increase the proximal fixation.

Bridging:

Bridging should be done by weight bearing on the affected limb only. The therapist maintains the normal limb in flexion and encourages weight bearingt hrough the hemiplegic lower extremities.

Unilateral rotation of the pelvis:

The patient performs hip-knee flexion of the affected lower limb and then rotated the pelvis to the opposite side. The patient tries to maintain this posture so that the spasticity of the trunk is inhibited. This exercise also help in encouraging forward rotation of the pelvis and correcting pelvic retraction. The patient is encouraged to perform flexion adduction and extension abduction pattern of the lower limb keeping the knee in extension position throughout.

The affected lower limb is kept in abduction at the edge of the plinth such that the knee is in flexion. In this position the patient is encouraged to perform knee extension and flexion without any adduction or flexion movement at the hip.

The patient may be trained to perform dorsiflexion with hip extension and plantar flexion with hip flexion. In case the patient is unable to perform dorsiflexion he may be trained to do with hip-knee flexion and the flexion at the hip and knee gradually reduced.

I have a concern regarding weed I have vasculatis reltated disease more like auto immune disease and I have seen the weed is helping me with getting rid of my steroid which was also a big concern for doctors. My blood and urine test reprt seems to improve with the use of weed. My doctors are happy now that I have got rid with steroids and progress in blood reports. But I cannot tell my family and doctor about weed cause of narrow mentality. I overall feel good and see no more disease related stress.

Dear sir, weed is not healthy for the body as well as brain. Chronic use of weed can cause many neurological symptoms, even dementia. Tell your doctors about this and try to quit this addiction of weed as soon as possible. Its not good to counter one bad habit with another.

Ok so you are taking proper protein in your diet but that malnutrition is not only responsible for hair falling there are many more factors that may cause increased hair falling. Eg: pollution, dandruff, scalp psoriasis, hormone disturbances etc etc. You should take proper consultancy.

It looks like you are anaemic. And also you have to check with your vitamin and calcium levels in the body. If the bones are weak then automatically the pain gets triggered more as there is less energy during any metabolic activity.

I am (girl- age 28) did MBA worked in IT company, married to Mr. Venkat (age - 39 or 40) Mtech just 9 months back (Nov 27 2015, residing in hyderabad. My husband (Venkat) is suffering from Psychosis since 2 years but I came to know after marriage as I was not aware of it. He uses "Arpit 10 mg" and "dyskinil" even now. I just wanted to know his health condition. He left office 2 years back and rejoined office after confirmation from his physician that he is normal. But I don't feel he is normal because he screams at night sometimes in a different voice pointing out his finger in some other direction. There is misunderstanding between us, he is always suspicious, negativeness, anxiety, mismatch words while talking, stubbornness, very childish (acting wanted to show others that he is innocent but not really he is ), not involved with family members, not thinking about family life, uncleanliness, chain smoker, untruthfulness, fear not bothered about anything like health etc, and looses his temper frequently which is really affecting our married life. Sir, these are not my intention but it's happening lively. I would like to know Is this abnormal behaviour or inborn from childhood ? There is lot of disturbance, upset in our sexual relationship as he could not fulfils his desire as he takes out his anger in another manner. Because of above reason I took him to urologist (from Apollo hospital) suggested tablets - " Tazzle 10 Mg "and "Nano leo" After using above medication I got conceived now I'm 5th month pregnant, he stopped using "tazzle and nano leo" for one month in my absence. After I came back from my mother's house finished my 3rd month pregnant he ordered " L- Arginine " without doctor consultation used for two days for testing purpose, it was ok for a while later no result. He seems to be normal but his behaviour differs sometimes, infact he has mental pressure from his family members as he is taking care of them financially (no job for his younger brothers, his sister got married who has 4 years kid but divorced) and his old parents. I am desperately looking for a solution, I'm afraid will my married life continues normally ? I will be happy if my husband becomes normal.

Your husband seems to be suffering from schizophrenia, you need to confirm the same from his psychiatrist. They have cheated you by hiding, from what you say. You must have reported this to your parents and got off the marriage in the 1st instance, since you faced the hurdles and living entire life with someone suffering from schizophrenia is a challenge. Since, you have decided to live with him and now even have a child, you have to take care of his mental health. When he visits his psychiatrist, go along with him describe the symptoms and get the medications tailored for the same. Learn about living with spouse suffering from this illness, from the doctor and reliable forums and get adjusted. Hope this helps.

Hi lybrate-user. Brahmi and Ashwagandha both have its own benefits but for different purposes like for example: Brahmi: Potential therapy for chronic discomfort and neuropathy, memory enhancer, aphrodisiac, and general tonic, Stress reduction, short attention span, foggy memory, and blurred focus. Wherein, Ashwagandaha is for Generalized Anxiety Disorder which is again related to stress, weakness and to gain weight. The Term Generalized Social Anxiety is overall different in psychology: Its important to understand the symptoms of what makes you say that you are suffering from Social Anxiety. Emotional signs and symptoms Excessive self-consciousness and anxiety in everyday social situations Intense worry for days, weeks, or even months before an upcoming social situation Extreme fear of being watched or judged by others, especially people you don't know Fear that you'll act in ways that will embarrass or humiliate yourself Fear that others will notice that you're nervous Physical signs and symptoms Red face, or blushing Shortness of breath Upset stomach, nausea (i.e. Butterflies) Trembling or shaking (including shaky voice) Racing heart or tightness in chest Sweating or hot flashes Feeling dizzy or faint Behavioral signs and symptoms Avoiding social situations to a degree that limits your activities or disrupts your life Staying quiet or hiding in the background in order to escape notice and embarrassment A need to always bring a buddy along with you wherever you go Drinking before social situations in order to soothe your nerves Now Generalised Anxiety is different from Social Anxiety. Here are the symptom: Excessive, ongoing worry and tension An unrealistic view of problems Restlessness or a feeling of being "edgy" Irritability Muscle tension Headaches Sweating Difficulty concentrating Nausea The need to go to the bathroom frequently Tiredness Trouble falling or staying asleep Trembling Being easily startled To answer your question. Bhrami and Ashwangadha will not really help you if you have Social Anxiety or Generalised Anxiety. Reason being it will not change your thought process. Those are only good from a perspective of keeping your Body healthy. Please identify to your symptoms and visit a counselor near you. I hope this helped. Take care.
