For headache take tablet paracetamol 650 mg eight hourly and I will suggest you to buy a steam inhaler machine of crescent company and take a flow of steam (of plain water only) over the forehead through the inhaler and can repeat the process after eight hours as and when required.

Nightfall is the uncontrolled ejaculation of semen during sleep. This is generally caused by sexual arousal and orgasm from dreams. Night fall or nocturnal emission is completely normal for males during teenage and early adult years. When boys enter puberty, there occur a lot of hormonal changes. These changes can result in spontaneous erections during sleep or, during the day and nocturnal emissions. These are simply a part of normal development. Causes of night fall discharge the exact cause of night discharge is not known. Some individual think that night discharges are the direct result of the stimulation caused by either dreams or memories of waking sexual activities. Hi, Blood in motions is seen in piles, fissures, ulcerative colitis, colon cancer, etc. For diagnostic purpose, do a colonoscopy and consult a general surgeon for further management. Avoid hot and spicy food. Take some laxatives if you have constipation. Drink plenty of fluids and consume high fiber diet. Get treated at the earliest, otherwise, you will suffer from severe anaemia due to heavy blood loss.

As of now for headache you can take tablet paracetamol. But we will have to make a diagnosis to get to the cause of it which can be either due to frontal sinusitis or due to migrane or due to refractive error of the eyes or due to other causes for which we have to investigate. If you want medicine than, I will prescibe.

Piles are haemorrhoids that become inflamed. Haemorrhoids are masses, clumps, cushions of tissue in the anal canal - they are full of blood vessels, support tissue, muscle and elastic fibers. Piles can be of various sizes and may be internal (inside the anus) or external ones (outside the anus). Typically, internal piles occur from 2 to 4 cm above the opening of the anus. External piles (perianal hematoma) occur on the outside edge of the anus. The internal ones are much more common. Symptomatic haemorrhoids affect at least half the population at some time in their lives before the age of 50. In the majority of cases, piles are effectively treated with over-the-counter medications, a good fluid intake, and by following a diet high in fibre. In severe cases, the piles may have to be surgically removed. About 10% of patients who go and see their doctor about piles eventually require surgical intervention. Internal haemorrhoids are classified into four grades. Grade 1 - there are small inflammations, usually inside the lining of the anus. They are not visible Grade 2 - larger than grade 1 haemorrhoids, but also inside the anus. When passing a stool, they may get pushed out, but return unaided Grade 3 - often called 'prolapsed haemorrhoids'; these appear outside the anus. The patient may feel them hanging out. They can be pushed back in if the patient presses with their finger Grade 4 - these cannot be pushed back in and need to be treated by a doctor. They are large and stay outside the anus all the time. External haemorrhoids are called perianal hematoma. These are small lumps that are located on the outside edge of the anus. They are very itchy and can be painful if a blood clot forms inside (thrombosed external haemorrhoid). Thrombosed external haemorrhoid requires medical treatment straight away PILES OCCUR when the blood vessels around the anus and in the rectum will stretch under pressure and may swell or bulge. Inflamed veins (haemorrhoids) can develop when pressure increases in the lower rectum. This may be due to: Chronic constipation9 Chronic diarrhea10 Lifting heavy weights11 Pregnancy12 Straining when passing a stool. 13 The tendency to develop haemorrhoids may also be inherited. The risk of developing piles grows with age Treatments for piles In the majority of cases, piles resolve on their own without the need for any treatment. Treatments can help significantly reduce the discomfort and itching that many patients experience. Diet - piles can be caused by too much straining when doing bowel movements, which is the result of constipation. A change in diet can help keep the stools regular and soft. This involves eating more fiber, such as fruit and vegetables, or switching your cereal breakfast to bran. Water is the best drink, and the patient may be advised to increase his/her water consumption. Some experts say too much caffeine is not good. Simple things you can do yourself to help prevent piles: 10 Try not to strain when you go to the toilet Avoid laxatives Exercise. Ointments, creams, pads and other OTC medications - there are some over-the-counter (OTC) medications which help soothe the redness and swelling around the anus area. Some of them contain witch hazel, hydrocortisone, or some other active ingredient which can relieve symptoms of itching and pain Surgery - used for particularly large piles, or grades 3 or 4 haemorrhoids. Generally, surgery is used if other procedures were not effective. Sometimes surgery is done on an outpatient basis - the patient goes home after the procedure.

Inguinal hernia is protrusion of intra abdominal organs to outside abdomen because of weakness in strength of abdominal wall. This weakness can be because of various reasons, excess of pressure due to weight lifting or straining too much during defecation or straining in urination etc or it may be due to unknown reasons. Prevention is be avoiding constipation or urinary problems, it these occur treatment should be taken without delay, by going for correct excercises and not lifting heavy weight suddenly or beyond one's capacity. Gradual increase in weight is recommended. Other then this if hernia has already occured it has to be treated (operated) without much delay.

Cyclic Vomiting Syndrome in Children - Causes, Symptoms, Treatment

Cyclic vomiting syndrome (CVS) is characterized by periodic bouts of nausea and vomiting that happens at cyclical intervals. It affects all ages, but is more common in children. The condition is quite stereotypical in that there are paroxysms or bouts of vomiting that is recurrent and follows days of normal health.

Causes: There is no definite reason identified, but it is said to have a strong hereditary correlation. Studies have shown mitochondrial heteroplasmic (abnormal growth of mitochondria, which is a cellular component) to be one of the factors that can lead to CVS. The genetic correlation, however, is very difficult to establish, specifically because vomiting and nausea are common symptoms that occur with most conditions in children. And CVS is most commonly noted with conditions like infections and emotional excitement. Infection could be either tooth decay or sinusitis or anything else. Lack of sleep, anxiety, holidays, allergies, overeating, certain foods, menstruation - a host of factors have been shown to induce CVS. There is also a strong association with migraine and conditions that lead to excessive production of stress hormones.

Symptoms: The syndrome (a group of symptoms) usually has 4 phases:

  1. Symptom-free interval phase: The child is completely normal in this phase, which happens in between bouts.
  2. Prodromal phase: Prodrome is an indication that a disease or a condition is about to happen. In CVS, this is usually nausea and abdominal pain that can last from a few minutes to a few hours. Treatment in this phase can curb the disease. However, there could be some children in whom this may not manifest and the child may directly start with vomiting.
  3. Vomiting phase: Repeated bouts of paroxysmal vomiting happen associated with nausea, exertion, fatigue, and drowsiness.
  4. Recovery phase: As the nausea and vomiting begin to subside, which may take a couple of days, the child returns back to normal slowly. However, the lethargy and energy levels will take a couple of days to return to normal.

Treatment: Treatment again depends on the severity and the phase at which it is being recognized. If a child has repetitive bouts, then the parent and the doctor would have identified a pattern to it.

  1. If the causative agent has been identified, for instance, infection or migraine, then managing that takes care of the CVS also.
  2. If identified during the prodromal phase, again it can be managed with suitable anti-emetic medications.
  3. If identified after full onset, rest and sleep and medications to control nausea and vomiting are required. Adequate hydration with electrolyte replenishment and sedatives can provide additional support.

However, in most cases of childhood CSV, the pattern will be identified and that helps in better management, both the child/parent and the podiatrist.
