Back pain may be due to strain, sprain, weather change etc. *Avoid bending forward while working on lap top or for any other work. *Don’t sit continuously working on laptop etc. Get up periodically and move around. *Restrict bike driving as far as possible. *Don’t carry laptop or any load on your shoulder. Carrying very light loads equally with both hands is better *Sit straight so that back bone gets proper support. A dining chair is better than a laid back sofa. *You can use mustard oil for massage which is good in winter season for relieving pain. Eucalyptus oil is also good *Soft bed will not give proper support to your back bone. Hard bed with thin mattress and thin pillow is better. *Get yourself tested whether you have spondylitis problem. The Causes and Symptoms of Chronic Lower Back Pain

Lower back pain is characterized by a feeling of pain around the lumbar region (curve of the back connecting the upper back and glutes- muscles in the buttock that are responsible for thigh movement) caused by muscle or ligament injury. The spinal area is most likely to be affected by lower pain as it may impair routine activities like walking and standing. Any lower back pain that lasts for more than ninety days can be considered as chronic.

Causes of lower back pain

There are plenty of causes for lower back pain. Anything that exposes the lumbar area of the spine to a level of stress beyond its capacity can cause lower back pain. These causes are-

1. Disorders of the discs - The spinal area in the lower back comprises of small bones called the vertebra, which are connected to each other by pieces of cartilage known as discs. These discs act as shock absorbers, and when exposed to excessive shock can cause the disc to bulge, resulting in herniated discs. Wear and tear of the discs can also occur.

2. Spinal stenosis - Spinal stenosis occurs when the area around the spine becomes narrow. It may affect the nerves surrounding the lower back, resulting in pain.

3. Wear and tear of the joints and ligaments - Normal wear and tear of the joints and ligaments cause difficulties in holding the spine in its proper position and this progresses with age. This may cause a vertebra to slide over another and affect the nerve endings, giving rise to pain.

Symptoms of lower back pain

The symptoms of lower back pain are usually a dull or excruciating pain in the lower back. It can also affect other parts of the body such as the hips and the legs. In case of severe pain, simple activities like walking, bending and sitting will be impaired.

For leg pain, please follow the below advise: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

I am 50 years old house wife. And last one year I am not able to lift my right hand up to my head. It gives lot of while lifting my hand. What you have described is very classic history of rotator cuff muscles tear. There is definitely cure for it. First confirm the diagnosis by checkup of shoulder and mri of shoulder at 3 tesla mri machine. After mri reports, I can tell you treatment.

Sciatica refers to pain that begins in the hip and buttocks and continues all the way down the leg. The term “sciatica” indicates that the sciatic nerve, which travels from the lower back through the buttocks and into the leg, is thought to be the cause of the pain in this condition. All About Shoulder Dislocation

A shoulder dislocation is a shoulder injury which is characterized by the upper arm bone popping out of the socket of your shoulder blade. The shoulder joint is the most mobile among all the joints in the body, making it the most prone to dislocation. The dislocation of the shoulder joint can either be partial or complete depending on the injury. In a partial dislocation, the head of the upper arm is partially shifted out of the socket whereas in a complete dislocation, the head comes out of the socket in its entirety.

Symptoms-

The symptoms of shoulder dislocation are:

  1. Swelling or bruising of the affected area.

  2. Deformed shoulder.

  3. Excruciating pain in the shoulder region.

  4. Mobility of the joint is greatly reduced.

Apart from these symptoms, there may be tingling, numbness and weakness in the affected region.

Causes-

There can be multiple ways in which a shoulder can be dislocated because of its ability to move and swing in all the directions. The fibrous tissues that connect the bones of your shoulder may also get stretched or torn. These injuries occur due to a sudden blow or a strong force exerted on the shoulder joint.

It is caused by:

  1. Trauma to the Shoulder Joint: Hard blows to the shoulder joint such as one suffered from a vehicle accident can lead to shoulder dislocation.

  2. Sports Injuries: In contact sports such as hockey and football, the sudden forceful contractions of the shoulder region may lead to dislocation of the shoulder.

  3. Falls: Tripping or falling from an elevated place and landing on the shoulder can cause shoulder joint dislocation.

Treatment-

The treatments for shoulder dislocation are –

  1. Medication: Medications such as pain relievers can be prescribed by the doctor to reduce pain.

  2. Surgery: Surgery of the shoulder joint can help in treating chronic shoulder dislocations. Surgery is also required if the blood vessels or the nerves along the shoulder joint are damaged.

  3. Immobilization: This process involves attaching a sling or a splint to the affected area to prevent it from moving. This allows the shoulder joint to heal and recuperate faster.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. It may be because of vertebral disc problem. Avoid forward bending. Sleep straight on hard couch in supine position. Consult an orthopedic surgeon for detailed examination.

I am 63 yr old diabetic male, suffering from frozen shoulder(left) since last two months. Shoulder is partially unlocked after two weak physiotherapy treatment but the pain has increased. I have only one kidney with creatnine stable at 1. 9 and thus no pain killer is advised. The physiotherapist has now advised ultrasonic treatment. I seek your advice.

Frozen shoulder with diabetes needs more care. Other metabolic disorders should be ruled out. Advanced physiotherapy measures like ift, us, laser, micro wave, swd etc along with remidial exercises will help.

Carpal Tunnel - How Endoscopic Surgery is Beneficial?

The carpal tunnel is the passageway for the median nerve to pass through on the inner side of your hand, when your palms are facing down. For example, while typing, there is a lot of movement of the fingers and this puts a lot of pressure on this tunnel. In fact, people who are involved in a lot of typing are one of the most prone people for carpal tunnel syndrome (CTS). Other causes would include trauma, fractures, etc., where the tunnel is injured.

Conventional measures to treat CTS included a combination of exercise and pain killers in mild to moderate cases. In moderate (sometimes) to severe cases, surgery was also done. However, with advancements in medical technology, endoscopic surgical procedure is gaining popularity.

What is done?
In endoscopic surgery, a small incision is made in the wrist or one each in the palm and the wrist. Through this, a flexible tube with a camera and minute cutting tools are inserted into the wrist. Another ligament known as transverse carpal ligament is cut. This reduces the pressure on the median nerve, which provides relief from the CTS symptoms.

What to expect as benefits?
The intention behind the surgery remains the same in both traditional and endoscopic procedures. The pressure on the median nerve is released, restoring movement and reducing pain in the joints. In addition, the endoscopic procedure has the following benefits.

  1. Less invasive: Needless to say, there are only one or two small incisions done for this procedure, so less surrounding tissue is damaged.
  2. Same day surgery: No hospitalization is required, and unless required, the patient can return home on the same day of surgery.
  3. Reduced recovery time: The recovery period is usually substantially less compared to the traditional surgical approach.
  4. Back to work: Depending on which is your dominant hand, you would be able to return to your regular work earlier than with regular surgery. If your non-dominant hand is involved, a day or two of rest is required. If your dominant hand was involved, give it up to four weeks for complete recovery.
  5. Reduced chances of nerve injury: The area of operation has many nerves and blood vessels, and endoscopic surgery reduces the chances of damaging adjacent nerves.
  6. Anesthesia: This is done under local anesthesia. Therefore, complications arising out of general anesthesia are completely avoided.

Indications
While these sound promising, your doctor will decide if you are a candidate for endoscopic procedure depending on the following:

  • Nonsurgical treatment including exercise and pain killers have not provided relief, even after several months.
  • Severe symptoms including those of nerve damage, like loss of sensation, altered coordination, etc., is affecting your normal daily routine.
  • Person requiring bilateral surgery like severe trauma, wheelchair dependent, etc.

Hello, Some primary headaches can be triggered by lifestyle factors, including: Alcohol, particularly red wine Certain foods, such as processed meats that contain nitrates Changes in sleep or lack of sleep Poor posture Skipped meals Stress insufficient sleep or rest Overexertion Nutritional deficiencies A secondary headache is a symptom of a disease that can activate the pain-sensitive nerves of the head. Any number of conditions — varying greatly in severity — may cause secondary headaches. Get your blood pressure checked, make a chart. Let me know about the readings, will give you medication accordingly. Also, get eyes tested. Medication: Meantime, can start with homoeopathic medicine - Bakson's Mig Aid tablet/ after every 2 hours. For Backache: Maintain a right standing and sitting posture, keep your back always straight, avoid carrying weight on back. Close the eyes and focus the attention on the body, breathing normally. Medication: Take homoeopathic medicines - Arnica 200 and Rhus Tox. 200 - take both of them thrice daily for 1 week. Also, apply Schwabe's Topi-Arnica and Topi MP Gel on the affected area in circular motions gently thrice a day. Get back to me thereafter.

Apply hot fomentation twice daily. Avoid bending in front. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang asana -- lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core strengthening exercises- straight leg raised with toes turned outward, repeat 10 times, twice a day.

I am 60 year old lady. From februar ymy out side thigh muscles are very tight and painfull, it looks that shiatica nerve got stretched. What should I do?

Sciatica is pain affecting the back, hip, and outer side of the leg, caused by compression of a spinal nerve root in the lower back, often owing to degeneration of an intervertebral disc. Ayurvedic medicines are very effective in case of sciatica. Apply prasarini oil with light hand on affected part and give hot fomentation. Take maha rasnadi kwath 2-2 spoon two times a day. Pranacharya sciatica capsule 1-1 after lunch and dinner. Maha yograj guggul -1 after lunch and dinner. Avoid tea. Coffee. Fermented food. Junk food. Spicy food. Cigarettes. Alcohol. Take milk. Fruits. Green vegetables. Do pranayama early in the morning. Walk for 30 minutes a day. Do sciatica exercise stretching. And leg exercises.
