Coronary Artery Disease   Causes, Symptoms, and Treatment

Coronary artery disease is one of the major killer diseases of the modern society. It is not a solitary problem but brings with it a multitude of issues including obesity, diabetes, stroke, and other metabolic disorders. A thorough understanding of what causes it and how to manage it can help save thousands of lives.

Causes: The circulatory system is mainly made up of the heart and a complex network of arteries and veins. The inner walls of these are lined with smooth muscles, allowing for free flow of blood. Gradually, over a period of time, given the density, the fat from the blood flowing through these vessels settles along the walls of these vessels. This attracts more fat, lipoproteins, and other inflammatory cells and so the process continues. This reduces the diameter of the blood vessels, therefore reducing the amount of blood supply to the target organs. If the target organ is a vital one like the brain or the heart, then it could lead to stroke or heart attacks.

Main causes for coronary artery disease include high blood pressure, high cholesterol, sedentary lifestyle, obesity, smoking, diabetes, and stress. Men are more prone than women, and family history and age puts them at higher risk.

Symptoms: Reduced blood supply to the target organ causes the following symptoms:

  1. Shortness of Breath: A person with coronary artery disease will feel short of breath and tired with most activities, even like walking a few meters.

  2. Chest Pain: A strong pressure sensation on the left side of the chest is an indication of coronary artery disease. Known as angina, it comes with stress and goes away once the stress is removed. It could sometimes radiate to the shoulder, down the arm, or up into the jaw also. These are classical symptoms of angina or heart attack and is considered an emergency. Of note, this is often mistaken for indigestion.

  3. Palpitations: A sensation where you are able to hear your heartbeat.

  4. Nausea and Excessive Sweating: Nausea and excessive sweating are also seen during angina.

Treatment: There are 3 modes to manage this.

  • Medications:

  • Surgical Procedures:  Surgical procedures including balloon angioplasty followed by placement of drug-eluting stents and coronary artery bypass surgery are done to manage more severe cases.

  • Lifestyle Change: Lifestyle changes including reduced fat consumption, decrease body weight, stop smoking, increased physical exercise, and reducing stress are highly important in preventing further damage.

Once suspected, coronary artery disease can be effectively managed and the extent of damage controlled using the above techniques.

You have to go for regular BP checkup for a week morning and evening both and then see The BP which is more is your correct BP and don't check it by digital BP meter.

If the block in the artery has been clearly demonstrated in investigation like coronary angiography and the extent of block is significant then angioplasty or bye pass surgery is indicated. If you are not having risk factors like diabetes, hypertension, the blocks are less than 50 percent of the coronary diameter and the plaques are calcified and stabilized then conservative medicinal management can be considered. You should seek advice from a cardiologist and decide.

Dear Dr, I want to know as my brother had gone under angioplasty two stents were paced in LAD and as third stent in RAC was not visible as arteries were very thin was told to be under medication, may I know the patient will be ok with such type.

There is no harm in RCA not being stented as its supply will be covered by other arteries and collateral will develop there . Medication is needed to prevent clotting in small blocks

Dear lybrateuser, - Do not stop medicine on your own, take advise of your treating doctor - stopping the medicine can cause an increase in BP much more than the earlier level - BP has to be monitored regularly, also take a well balanced diet with more of fibrous foods, proteins & healthy fats as in olive oil, avocado, fish, almonds, walnuts & flax seeds.

Tinnitus is a common problem usually associated with deafness. It is of two types, subjective and objective. In objective tinnitus you can find out the cause of tinnitus while in subjective it is difficult to find the cause. Treatment consisit of reassurance and certain vasodilator and vitamin treatment. In objective tinnitus you may require surgery if cause is confirmed by all mod of diagnosis including ct scan.

Dear lybrateuser, -A hole in the heart is usually from birth, it is a small passage between the chambers of the heart(a developmental defect) which closes on its own but in some people it remains open -it can heal on its own which is a very slow process -if it remains open & causes problem then check up & surgery may be required .

Hello, Thanks for the query. A person with diabetes is more likely to get coronary artery blockage, because dyslipidemia is more common in diabetes. So it is very essential to keep blood glucose in strict control, with fasting glucose < 100 mg, PP around 160 to 170 mg & HbA1c% <7. Plus person has to take a balanced healthy diet. Also do necessary exercise as advised. There is also the need for taking medications to control lipid levels. In other words, the person has to follow a very disciplined life. Refrain from smoking and alcohol. There is no permanent CURE for these conditions. Patient has to learn to live with those. Thanks.
