Idiopathic Infertility!

Sexologist, Hyderabad
Idiopathic Infertility!

Unexplained infertility is infertility that is idiopathic in the sense that its cause remains unknown even after an infertility work-up, usually including semen analysis in the man and assessment of ovulation and fallopian tubes in the woman.

Possible causes

In unexplained infertility abnormalities are likely to be present but not detected by current methods. Possible problems could be that the egg is not released at the optimum time for fertilization, that it may not enter the fallopian tube, sperm may not be able to reach the egg, fertilization may fail to occur, transport of the zygote may be disturbed, or implantation fails. It is increasingly recognized that egg quality is of critical importance and women of advanced maternal age have eggs of reduced capacity for normal and successful fertilization. Also, polymorphisms in folate pathway genes could be one reason for fertility complications in some women with unexplained infertility. Aberrant reproductive immunology such as decreased maternal immune tolerance towards the embryo may also be a possible explanation. However, a growing body of evidence suggests that epigenetic modifications in sperm may be partially responsible.

Prevalence

In india up to 25% of infertile couples have unexplained infertility.

Management

  • Potential methods in unexplained infertility include oral ovarian stimulation agents as well as  (IUI), intracervical insemination (ICI) and in vitro fertilization (IVF).
  • In women who have not had previous treatment, ovarian stimulation combined with IUI achieves approximately the same live birth rate as IVF.
  • On the other hand, in women who have had previous unsuccessful treatment, IVF achieves a live birth rate approximately 2-3 times greater than ovarian stimulation combined with IUI.
  • IUI and ICI has higher pregnancy rates when combined with ovarian stimulation in couples with unexplained infertility, for IUI being 13% unstimulated and 15% stimulated, and for ICI being 8% unstimulated and 15% stimulated. However, the rate of twin birth increases substantially with IUI or ICI combined with ovarian stimulation, for IUI being 6% unstimulated and 23% stimulated, and for ICI being 6% unstimulated and 23% stimulated.
  • According to oral ovarian stimulation agents should not be given to women with unexplained infertility. Rather, it is recommended that in vitro fertilization should be offered to women with unexplained infertility when they have not conceived after 2 years of regular unprotected sexual intercourse. IVF avails for embryo transfer of the appropriate number of embryos to give good chances of pregnancy with minimal risk of multiple birth.
  • A review of randomized studies came to the result that IVF in couples with a high chance of natural conception, as compared to IUI/ICI with or without ovarian stimulation, was more effective in three studies and less effective in two studies.
  • There is no evidence for an increased risk of ovarian hyperstimulation syndrome (OHSS) with IVF when compared with ovarian stimulation combined with IUI.

Prognosis

Prognosis in unexplained infertility depends on many factors, but can roughly be estimated by e.g. the Hunault model, which takes into account female age, duration of infertility/subfertility, infertility/subfertility being primary or secondary, percentage of motile sperm and being referred by a gynecologist, sexual medicine specialist or N infertility specialist.

Hello lybrate-user, if you have missed your periods and you are suspecting pregnancy, you must first undergo a urine pregnancy test. If the test is positive, it will be better if you undergo a medical termination of pregnancy at an authorized center. Chemist will not give you abortion pills without a proper prescription and the pills can even cause some major side effects. It is advisable that if you are confirm on aborting the pregnancy, you get it done at an authorized mtp hospital.

My husband drinking alcohol daily, what can I do, to stop this habit, suggest me any tablets plz.

Reparenting Technique, BA, BEd

My husband drinking alcohol daily, what can I do, to stop this habit, suggest me any tablets plz.
You have all my sympathies and I am very concerned that your husband has not reached what we call, ‘Hitting Rock Bottom’ as yet. Ideally, he must want to stop it himself. Anyway do not despair as yet and you may attempt the following and see how it works: you may have tried a de-addiction center but he should not have been released if he was not ready. So in this attempt, he may be taken to another de-addiction center that is stricter and will also have follow-up treatment and AA meetings for him to compulsorily attend. You will need the help of some adults in the family. So whatever I suggest, kindly involve or share it with mom or elder family members to make it effective and suffer the least backlashes, if any. Ideally, your husband should want to give up alcohol himself to begin with. If not, it is going to be an uphill task and to the most part impossible. However, you should not be in want of trying. If he is unwilling to seek help, he needs to be admitted to a rehabilitation center for at least 6 months. If not, there are centers who will come and pick him up when he is drunk, when his resistance will be ineffective. This will require your permission and written consent. After your husband returns from therapy, there will be a lot of follow-ups, both in the house and in his daily activities that must be meticulously adhered to. There is a medicine called Disulfiram, which only the doctor can prescribe, and it is very powerful that even if he attempts to drink a small quantity, he can have serious reactions that may sometimes entail hospitalization. You have to monitor his intake of the medicine and keep a close watch on him. He also has to go for regular counseling for at least three years! Apart from his health, he will also affect the family and all his children will carry genetic factors for alcoholism. If he is into business or working, he will soon not be able to continue and he will go into financial ruin. He will lose all his friends and become ostracized by his society. He will lose his dignity, self-esteem, self-confidence, and become a misfit. He will also lose his sexual capacity, liver function, heart function, kidney etc. Now it is difficult to give up because he has become addicted to alcohol for some time and has a lot to do. The consumption of Antabuse has to be carefully monitored to see that he not only consumes the medication but is also avoiding alcohol. Ultimately, it is his will power and the support that he receives from the medical fraternity and his close and dear ones. He must also learn to substitute and deal with the oral need, a rigid value system, the script issue, and of course take a look at all the genetic factors to plan a strategy not to get into what is called ‘cross addictions’ i.e. Another form of addiction that may appear alright but is in fact as bad as the primary addiction. The center or hospital and the counselor will advise and guide him on several measures and precautions he will need to take to stay with his resolve. Even after the rehabilitation he must attend AA meetings and continue this support for a long time. The family will also need to attend some sessions and go for Al-anon meetings for their co-dependency issues. He cannot be treated in isolation because the family has gotten used to his drinking and have made some unhealthy adaptations to somehow cope. The children, including you and whoever else is there, will also have to attend meetings to work out their issues because of the father’s habit. In fact, they are all suffering from the Adult Children Of Alcoholics Syndrome (ACOAs), which in effect means that they are genetically predisposed to alcoholism or can have cross addiction problems and that you will have similar traits of the abusing alcoholic but in a milder form. There are special support groups for them all over the world. Should they touch or indulge in alcohol or any addictive substances or behaviors, they could also become full-fledged addicts themselves. Now you should not give up on him but neither should you harass him. Be continuously after him and in the end, you will succeed.

You should take coffee and black salt twice daily to increase your bp. Increase your fluid intake. Before increasing your salt consumption do check with your doctor also, cut down on alcoholic beverages and high carbohydrate foods such as bread rice pasta and potatoes. The proper treatment for this condition depends on the exact cause.
