No, chicken pox and mmr are not same. They are two different vaccines. There is however, no harm if you have given a second dose of chicken pox by mistake. You an take her after a gap of one month (compulsory between 2 vaccines) and give her the mmr vaccine. No harm done but don't miss the mmr vaccine.

Tuberculosis(TB) - How To Treat It Effectively?

MBBS.DTCD
Pulmonologist, Hyderabad
Tuberculosis(TB) - How To Treat It Effectively?

Tuberculosis is a serious infectious disease that can affect the lungs. The bacteria spread through sneezes and coughs making it a communicable disease. This disease was rare and limited to the Western Countries until 1985. The emergence of HIV virus has made tuberculosis a global disease to reckon with. Although the rate of TB has come down since 1993, it remains a top concern for many countries. The biggest worry is the fact that TB bacteria can build resistance to many drugs. Therefore, it is very important that a TB patient takes a variety of medication suggested by the doctor at the right time.

Although your body is already in possession of the bacteria leading to tuberculosis, your immune system is able to prevent you from becoming sick. Doctors have made a distinction between latent and active tuberculosis (TB)--

In case of latent TB, the bacteria in the body in a passive state and it causes no symptoms, and therefore it is not contagious. But, in the case of active TB, you would become sick and may even spread the disease to others. It can take place in the first few weeks or even after several months of being infected with TB bacteria.

Types of Tuberculosis:
There are two types of tuberculosis- Latent and Active.

  1. Latent Tuberculosis is a condition where the bacteria stays in the system of a person but doesn’t show up any symptoms of TB. Although this type of TB is not contagious, it requires treatment so that TB can be controlled from spreading.
  2. Then there is active tuberculosis. It shows symptoms in the first few weeks and can spread to another person as well.

What are the symptoms of active TB?

  1. If you are coughing for over three weeks and sometimes even coughing up blood, it can be a sign of TB.
  2. Chest pain and pain while coughing and breathing along with fatigue, fever, chills and night sweat are the common symptoms of TB along with loss of appetite and unintentional weight loss.
  3. TB may even affect other organs of your body, including your brain, spine and kidneys.
  4. When TB takes place outside the lungs, then the signs of TB can vary as per the organs that are involved. For instance, TB in the spine can cause back pain and that in kidneys may cause blood in the urine.

What are the causes of TB?

  1. TB is stemmed from a bacteria which spreads from individual to individual via the microscopic droplets that are released into the air.
  2. This may happen when an affected person is left untreated and he speaks or sneezes or coughs or laughs. Though the disease is contagious, it is not easy to be affected by it.
  3. As a result, you are much more likely to get affected with active tuberculosis from a person you live with or come in regular contact with, rather than a stranger.
  4. It is important to note here that people who are affected with TB and going through proper medications for over two weeks are no more contagious.

Tuberculosis - How to effectively treat it?

With the treatment for tuberculosis, one should have patience as it may take time to cure a person completely of tuberculosis and the associated symptoms. Many factors are taken into consideration while determining the course of the treatment for tuberculosis, some of which include

  • Type of tuberculosis (Latent or Active).
  • Age of the patient.
  • The condition being drug resistant.
  • The general health of the patient (if there are any other health conditions that may interfere with the treatment).

In the case of Active TB and that too, a drug-resistant type, the use of multiple drugs (simultaneously) may be needed to produce the desired results. The drugs commonly used include Pyrazinamide, Rifampin, Ethambutol, and Isoniazid. In most cases, the doctors suggest the drugs be continued for at least six- nine months for positive results.

Some patients with Active TB (drug resistant) may also benefit from the use of fluoroquinolones (antibiotics) taken along with capreomycin, amikacin, or kanamycin (mostly given in the form of injections). Most patients are required to follow the medication for 20-30 months duration.

Unlike the Active TB, in the case of Latent TB, a combination of drugs and medications is not required. A single TB specific drug is enough to treat the condition.

Therefore, it is important to seek medical assistance and detect if you have any such health complications concerning TB.

Diarrhea - How to Treat it in Children?

Diarrhea - How to Treat it in Children?

Diarrhea occurs when the body is expelling germs out of it. An episode of diarrhea can last anywhere from a couple of days to a week. Dehydration, cramps, vomiting, nausea and fever often accompany diarrhea. Usually, children tend to get diarrhoea more than adults.

Diarrhea treatment according to causes behind it

  1. Treating diarrhea due to infection: Some common causes of diarrhea include infection from rotavirus, salmonella (a type of bacteria) and rarely giardia (a type of parasite). In children, virus is a common cause of diarrhea. Besides watery or loose stools, viral gastroenteritis infection symptoms include fever, headache, stomach ache and vomiting. Gastroenteritis diarrhoea can last for 5-14 days. During this time, fluid loss must be prevented. In younger children, oral rehydration solution (ORS) or breast milk is enough. Just feeding them water won’t replenish the potassium, sodium and other nutrients they lose. Older children, to prevent fluid loss, can be given ORS and popsicles to counter dehydration and vomiting.
  2. Treating diarrhea due to medications: Antibiotics or laxatives can cause diarrhea in children. If the diarrhea is mild, make sure your child is hydrated. If the antibiotics are causing diarrhoea, you need to continue medication, but do consult a doctor immediately.
  3. Treating diarrhea due to food poisoning: When it comes to food poisoning induced diarrhea, the same method as countering infection-induced diarrhea should be followed. Keep your child hydrated and call your doctor.

Diarrhea and children - recognising dehydration

The most serious complication of diarrhea is dehydration, especially if the diarrhea is severe. Severe dehydration is critical as it can cause brain damage, seizure, even death. Your child may need immediate medical attention, if you notice the following symptoms:

  1. Light-headedness and dizziness
  2. Sticky, dark mouth
  3. Dark yellow urine
  4. No or few tears when crying
  5. Dry, cool skin
  6. Loss of energy

When should you visit a doctor

Diarrhea resolves within a few days, but it can have serious complications. Call your doctor if you notice signs such as:
1. Looks very sick
2. Diarrhea has persisted for more than 3 days
3. Your child is less than 6 months old
4. Your child is vomiting bloody yellow or green fluid
5. Seems dehydrated
6. The fever reads above 105 degree Fahrenheit
7. Bloody stools
8. Rashes
9. Stomach pain persists for more than 2 hours
10. Infrequent urination

Vaccines are organisms to fight various kinds of diseases but body usually treat these organisms as foreign bodies. The body fight off the strain of organisms injected through vaccines to give signs in the form of rise in temperature. When the individual is attacked by organisms, the body fights them back to save life of child. High temperature kills bacteria and viruses which thrive in the body at normal temperature. Rise in temperature help the body to produce more white blood cells, antibodies and other infection-fighting agents for infants’ protection. In fact, fever is beneficial in infants after vaccination.Dont wory it will subside automatically in a day or two.

4 Excellent Ways to Break Your Child's Addiction to Sweets

Diploma in Child Health (DCH), MBBS
Pediatrician, Gurgaon
4 Excellent Ways to Break Your Child's Addiction to Sweets

If your child has become increasingly addicted to sweets and other sugary items, this addiction can lead to certain behavioral problems (like mood swings, irritability, anger, etc) in children. Moreover, a high sugar intake can lead to your child falling prey to serious diseases and conditions, including diabetes, obesity and cardiovascular diseases.

Here's what you can do to break your child's indulgent habits and wean him from ill-health causing culprits like cakes, ice creams, candies, chocolates and rich and creamy desserts etc.

1. Practice what you preach -
In order to develop healthy eating habits in your child, it is imperative for you as a parent to lead by example. When it comes to weaning your child from sugary foods and sweets, you yourself should limit your intake. So, if you tell your child to avoid sweets but love sweets yourself, you'd be setting a bad example by eating them yourself and not letting your child have them. The best thing to do is to be a role model to your child as children learn everything from their parents.

2. Curb consumption of sugar by deferring intake to a later date -
Instead of depriving your child completely, it would prove to be more useful to tell them that they could have sweets on some other day. This is because if you deny him his indulgence, your child would invariably go on an eating spree the moment an opportunity arises. A study on a group of restrained eaters and non-diet individuals revealed that restrained eaters were found to consume more food than the non-dieting individuals. Not knowing when they could have their favourite foods again, they went on an eating spree.
Make frequent yet small offerings of sweets while explaining to him the importance of moderation and why he/she shouldn't overindulge but space out sugar intake.

3. Try the 90/10 rule -
Teaching older children the 90/10 principle is another sure-shot way of breaking your child?'s addiction to sweets. The rule calls for 90% of the diet to be made up of healthy food with the remaining 10% being devoted to foods your child wants to indulge in such as junk food, sweets or foods with high sugar content. The total calorie intake that arises from consumption of about 10% or 1-2 fun foods is only about 100-200 calories everyday.

4. Present your child with healthy alternatives to sugary items -
The next time your child wants to indulge in his sugar craving, instead of giving a processed sugary food, give healthier sweet foods like dates, honey or a fresh sweet fruit.

Delayed Speech - Causes And Symptoms!

MD - Paediatrics, DNB (Pediatrics)
Pediatrician, Rourkela
Delayed Speech - Causes And Symptoms!

It might happen that you may not find your child, at twelve months to two years of age, at the same level as their peers in verbal communication. You think it's just a developmental problem they are facing and put off seeking professional advice; an intrinsically wrong step to take, because your child might be suffering from Speech Delay.

Delayed speech, or alalia, can be roughly defined as a delay in the development and use of the biological mechanisms that produce speech. Delayed speech is sometimes caused from hearing defect also. A deaf child is always dumb. Before we jump into conclusion of delayed speech, hearing assessment is mandatory.

The symptoms of speech delay are roughly categorized into age related groups, generally beginning at the age of 12 months and continuing through the early adolescence, and they are:

1. Age-12 months
a. It is indeed a symptom if your child cannot point at objects or cannot manage gestures, such as waving good-bye.
b. Another symptom is that if your child does not prefer to communicate verbally as much as his/her peers.

2. Age-15-18 months
a. If your child is unable to pronounce familiar syllables or simply cannot call you even by this time, it's a worrying symptom.
b. You find your child unable to, or simply not reciprocating to 'no', 'hello', 'hi', 'bye'.
c. If your child is unable to extend his/her vocabulary up to 15 words by fifteen months, then it's a symptom.

3. Age-2-4 years
a. You find your child unable to spontaneously produce speech and words.
b. Another worrying symptom is if your child is lacking consonant sounds at the beginning and end of words while speaking.
c. If you still find your child unable to form simple sentences and words, then it is indeed a troubling symptom, confirming the disorder.

The causes for the speech delay disorder are:
1. A primary cause can be physical disruption in parts of the mouth such lips or palate, which may be deformed.
2. Another serious cause can be an oral-motor dysfunction which is the disruption in the creation of the specific area of the brain which deals with speech and communication.
3. The disorder can also be attributed to impairment in the development of the child's intellectual, receptive and expressive abilities.
4. There can also be psychological causes involving school environment and peer relationships which might lead to disruption of speech patterns and reluctance in speech expression and development. If you wish to discuss about any specific problem, you can consult a pediatrician.
