I am a diabetic patient and my blood pressure is always high. I have a spondylitis problem. What should I do? What are the exercises that I can do in the morning?

COMT, MPTh/MPT
Physiotherapist, Surat
Due to inflammation in cervical spine you may face problems like neck pain and stiffness, discomfort, unable to keep neck straight etc This is what we called Spinal Spondylitis. It has can be cured by physiotherapy so I would recommed you to consult any physio and start taking physio modalities and manipulations to reduce pain and increase mobility. I have specially designed one program as I-care specially in cases of neck and back pain which I have pratically applied on many of my patients and received a very good result. The treatment include: 1. Ice massage - Take ice pain perform massage over the area which is painful and have swelling. Do “to” and “fro” motion. Apply it for 8-10 minutes and then relax your body for another 3-5 minutes. 2. Deep massage - Using your finger tips start kneading your affected area. 3 finger kneading or 2 finger kneading or you can use thumb also for kneading. Once done with kneading again relax your body for 3-5 minutes. 3. Now apply any pain relieving ointment like volini or oraflame etc. Do not massage with the ointment. Just apply it gently involving all the area. Do not penetrate it. Once done with application again relax your body for 3-5 minutes and start performing exercises. Exercises: Because of pain and stiffness your neck range starts decreasing so with the entire treatment program we can help you to make your neck mobile and reduce pain to live happily. 1. Position - Sitting Bend your head forward until you feel stretch behind your neck. Hold for 5-10 secs. And relax. Repeat 5 -10 times. Perform same by taking neck backward and then side ways. Perform the exercises slowly until a stretch is felt. Do not over stretch, perform slowly and gently. 2. Position - Sitting Isometrics: Infront of mirror. Place your both hands on your forehead. Apply contradict pressure from hands towards your head and with your head towards your hands. There should not be any neck moment. Instead an isometric contractions should be there in neck. Hold for 5 sec and relax. Repeat it for 5-10 times. Perform same by placing your hands on the back of your head and giving isometric contractions to make posterior muscles stregthening. Then same as with placing one hand on right side and giving pressure from hand to head and vice versa. 3. Position - Sitting Stretching - Bend your head forward and and then gently pull your head further by applying pressure from your hands. A stretch is felt on back of your neck. Same a stretch your side neck muscles. 4. While exhaling push your upper rib down with your one hand and then tilt your head and give a slight stretch by rotating your head to opposite side. Hold for 5 sec and then relax. Repeat for 5-10 secs. 5. Neck and back straight, keep your chin in. Hold for 5 sec and feel a stretch in your neck. Relax. Repeat for 5-10 times. 6. Lie on bed with towel roll under your neck. Press the roll into the floor by using your neck muscles. Hold for 5 sec and relax. Perform 5-10 times. Advice - 1. Use very thin pillow while sleeping. 2. Do not sit for long time with your neck forward bending. 3. Lying on bed and reading book or newspaper should be avoided. 4. If there will be any pain while performing exercises or headache stop doing exercise and consult physio right away. 5. Sit in erect posture without bending your back or neck. 6. Position your desktop so that it should be just infront of your neck you do not have to bend your hands or neck. .Ill reply you with the exact pictures showing how it can be done. There are many mobilisation techniques that can only be given by physio. You cannot do those at home. Thanks and Good Luck. Get well soon.

Is osteoarthritis a part of aging, and is there a way to reduce risks, even after age 50?

Fellowship of the Royal College of Surgeons (FRCS), Membership of the Royal College of Surgeons (MRCS)
Orthopedist, Trichy
primary osteoarthritis is a part of ageing. but secondary arthritis can be following any fractures involving the joint surface, following infection in the joint and also secondary to inflammatory arthritis. Arthritis cannot be prevented, but the progress can be slowed down by weight reduction and strengthening the muscles around the joints and also avoid high impact activities.

Dear try the remedies they will help him..:: 1.Use warm sesame oil to massage stiff and painful joints in the morning. Massage improves blood circulation and reduces inflammation and stiffness. 2. Put one teaspoon of fenugreek in a cup of water and set aside overnight. Then next morning, mix well and drink. 3. Boil 10-12 slices of ginger in 8 cups of water for 5 minutes and let it steep for another 10-15 minutes. Drink this tea at a warm temperature throughout the day. 4. Revert back with his reports for further management.

This is a general low back ache and for this you can follow these measures: one keep a pillow right under the knee while sleeping, next is you can keep ice in the painful area for about 5--10 minutes, if pain still persists you can stretch your body by twisting the waist on both sides how we used to do in the school drill similarly you can try which will help you relax as well will reduce the pain. It looks like you are anaemic. If you have back pain after you sit for long hours then it is due to your haemoglobin levels as it is important to check that. Anaemia always leads to the symptoms of being tired and also having back / leg pain though there may not be any pathological reasons for back pain.

I have a cold and head pain and legs pain. please give me a solutions for my healthy health.

AUTLS, CCEDM, MD - Internal Medicine, MBBS
General Physician, Faridabad
I have a cold and head pain and legs pain. please give me a solutions for my healthy health.
There are several things you can do to prevent a cold—they are all easy and inexpensive and worth the effort. Wash your hands often-with soap and warm water. The germs stick to the soap and get washed down the drain. Always wash your hands after coughing or sneezing into your hands, blowing your nose, after touching someone who is sick, or when using public restrooms or telephones. Cover Your Mouth and Nose When You Sneeze or Cough- Clean your hands every time you cough or sneeze. Avoid Touching Your Eyes, Nose, or Mouth-Germs are often spread when a person touches a contaminated surface and then touches their eyes, nose, or mouth. Stay Home When You’re Sick-Get plenty of sleep, be physically active, don’t smoke, manage stress, drink plenty of fluids, and eat nutritious food so you can fend off viruses. In general you should: -Take healthy balanced diet...like boiled eggs.daily in breakfast .. -Avoid people who have a cold or cold symptoms. -Routinely clean and disinfect common areas of your home. -get plenty of rest so your body can use its energy to fight the cold -staying at home will keep you from spreading germs -drink plenty of liquids like herbal tea or hot broth/vegetable/tomato soups. Avoid foods or drinks with caffeine which can keep you from getting enough sleep -Runny Nose and Sneezing- if you have post nasal drip, gargle to keep from getting a sore throat. Drink extra fluids. (avoid sugary and high calorie drinks) -Use saline nasal sprays or nasal irrigation • Tablet Nasorest-P once/twice daily may reduce the amount of mucous. • Do steam inhalation once or twice daily Treatment:There is a gradual 1-3 day onset of symptoms. It often starts out as a sore throat, then a fever and coughing. As the cold progresses, the nasal mucus may thicken which is the last stage before the cold dries up. A cold usually lasts for a week or two. But, taking good care of yourself at home can relieve symptoms and help prevent complications if you are otherwise healthy. Unless you develop complications, you shouldn’t need medical treatment. Complications include bacterial infections of the ears, throat, sinuses or lungs which must be treated with antibiotics. Cough-Coughing is how your body gets rid of debris and mucus from the airways during a viral infection. Unless your cough is severe or is keeping you from resting at night, it is better not to treat it. A severe cough can be treated with a cough syp, but don’t try to completely stop the cough. There are two types of coughs. Productive and non-productive or dry cough. Non-productive, dry cough. A dry, hacking cough may develop near the end of a cold. Dry coughs that follow viral colds are often worse at night and can last up to several weeks. Take syrup Alex-D for a dry cough. Cough drops can sooth an irritated throat, but most don’t affect the cough. Productive cough. A productive cough is one that brings up mucus from the back of the throat or the lungs. Productive coughs should not be suppressed to the point they no longer bring up mucus. Use syrup Alex. Drink lots of water if you have a productive cough. Head and Body Aches- Take tablet dolo-650 mg for fever once or twice daily

Hello. Firstly you should get your x-ray done for the sake of precautions. You are having a fractured leg so you must be on bed rest for about 2-3 months or maybe more. And it is usually said that longer bed rest changes your circadian rhythm. Various hormones and enzymes are released in a 24 hour schedule. Since your leg was bandaged so there must be lack of vitamins and calcium. If you feel that your bones are getting weaker then you can 1 gm of wet lime stone (chuna. Along with 100 gm of curd, juice or water. You will feel all the deficiencies will vanish. Please do not use it if you have a problem of kidney stone or other such problems. Do consult a dietitian or a wellness coach. Thanks.

N- for leg pain, please follow the below advise: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Hello Doctor, This question is for my 53-year-old mother-in-law who has been having rheumatoid arthritis for about 30 years. She has had bilateral hip and knee replacements, and her last operation was done about 5 years ago. Things were going fine until about 2 months ago when she started having severe neck and shoulder pain, sometimes radiating to her arms. We did consult her regular rheumatologist, who has asked her to begin Adalimumab injections, twice a month. We came to know that there are severe sideeffects to the drug and would like to know more about it and the success percentage of Adalimumab injection. It would be great if you could help us find other treatment alternatives that we can look up to.

Dear Radha, Namaskar, Adalimubab is a biologic drug, very effective in RA.......Ofcourse all the modern medicines do have some untoward and side effects.......but the treating Doctor has to titrate the dosage and choose the lesser evil out of Pendora of various drugs.... I am sure he will closely monitor the situation...There is no alternative treatment of this disease as for as scientific evidence is concerned.... Have faith and patience.... Take care.
