Symptoms of Postpartum depression and ways to deal with it!

Hi, my name is Dr. Armaan Pandey and this talk that I'm going to have maybe very beneficial if you are a to be parent or you are a new parent. So I'm going to tell you about three aspects of postpartum mental health. Postpartum refers to post delivery, as a lot of you might know. The first thing I want to talk about is a very serious medical condition which is called as postpartum depression. So right before or after the birth of a baby a mother's body suffered through a massive hormonal changes and often these hormonal changes can result in depression. Like we see in depression too the symptoms are very very similar like loss of interest in everything, feeling sad all the time, doesn't want to take care of the babies also at time, doesn't feel like worth living, disturbance in sleep, disturbance in appetite, crying spells, suicidal thoughts and sometimes even thoughts of hurting the baby. So yes your hormones can do this to your brain. Solution is quite simple. A short course of anti-depressant therapy is very very helpful, very efficient in taking away these symptoms. The symptoms go away very quickly and it is quite safe as well.

Another aspect that one must be aware of when it comes to postpartum depression is a milder form called as postpartum blues. Here the woman experiences mood swings, often crying jags, easy irritibality, but these are very short lasting. So most of the days the woman is fine, the mother is fine but sometimes she may display these things and usually postpartum blues gets better on their own, you don't have to do much about it. But it is advisable that you go to a doctor, get yourself checked out and if it is depression then get treatment and get better. The second aspect of mental health, when it comes to mothers, usually when new mothers are concerned is that you have to be aware of something called as mother's guilt.

Now this has got more to do with how the society treats new mother's than what biology does to you. When you deliver your first baby, or even second child for that matter, you are loaded with a barage of instructions, you are supposed to do this, you are not supposed to do that. And these instructions comes from every corner of the world. Your spouse tells you something, parents tell you something else and your in-laws tell you something else, your doctors are also advising you, your neighbours, your relatives and even your maid servants at times. What do you do in that case, and sometimes the advises are contradictory. So, the mother has the burden of making small, tiny decisions for the baby. And every tiny decision she makes she feels guilty that maybe what I'm doing for my child is not right. You need to be aware of this, there is nothing like a perfect mother or a perfect parent. All you have to do is be a good enough parent, a good enough mother. You cannot make sure that your baby is 24x7 happy or comfortable. Your baby is going to cry when she is not comfortable, when she is hungry, when she has some pain and this is going to continue till she can vocalise or verbalise her feelings better. So, do not feel absolutely guilty if you are not able to soothe your baby immediately or if someone tells you otherwise. You have to make a decision, sometimes you have to be assertive with other individuals. Don't be aggressive, don't break into a fight but do not take everything passively too.

The third aspect that is important when it comes to mental health, especially for new parents is that you lose focus of your relationship at times. Because the baby is the centre of the family now, you start ignoring your relationship with your spouse, probably you are more engaged in taking care of the baby for the first two years, which is required, it is inevitable. But during this time a lot of couples experience distancing and people who have problems later on in their marital life often report that this was the turning point. So one has to be mindful. What I can advice you is like you treat your relationship with your partner as your first child, because your relatonship is something you both have created together. It requires constant nurturing, it requires constant attention from you. So if you have another child after your first child you are not going to ignore this first child because you are having another baby.

Of course you can't pay as much attention but you can't neglect it. So for your partner also take out some quality time, do certain small things for them that makes you happy and whenever you can find time do spend quality time together bonding with each other. Remember this fact that you are becoming parents because you are together so you have to take care of this relationship as well. If you are aware of these few aspects of mental health and a lot of other things as well pertaining to parenting I'm sure you are going to enjoy parenthood rather than be stressed about it.

The immune system is extremely important in an individual's system. A strong immune system helps to combat the invasion of foreign particles and consequently resists the diseases.  Vaccination in such a context becomes imperative as it strengthens an individual's immunity. In vaccination, antigens or germs are given in very small doses. They stimulate the immune system to produce antibodies to fight against that particular infection. Vaccinations are provided to both children and adults to protect them from a number of diseases.  However, different vaccinations are provided in different ages according to the susceptibility to diseases.

Some of the vaccinations that are provided to newborns are:

1. Hepatitis B vaccine: This vaccination is given in order to prevent the child from having Hepatitis B. Hepatitis B is a liver disease that if persists can lead to liver failure or even liver cancer. This vaccine must be injected immediately after the birth of the baby. The first dose must be followed by administering a second dose within a span of a month or two. 

2. Rotavirus Vaccine (RV): This vaccine, taken orally, prevents the infant from Rotavirus. This virus causes vomiting and diarrhea in children that often leads to severe dehydration. This vaccine is administered within two to four months of the baby's birth. Sometimes, on doctor's prescription a second dose may be necessary in the sixth month. 

3. Acellular Pertussis Vaccine and Diphtheria and Tetanus Toxoids: This is a combination of various vaccines that protects the child from tetanus and diphtheria. Newborns are extremely prone to diphtheria that causes fatal illness and sometimes even deaths in children. This vaccination thereby, is extremely important and must be administered within two or four months and must be followed up with secondary doses later under the doctor's supervision. Vaccinations do not end with childhood. In many cases adults too need to be vaccinated against certain diseases. Some of them are:

  • Hepatitis A Vaccine: You can get this vaccination if there is any risk of you suffering from Hepatitis A. Much like Hepatitis B, Hepatitis A too is an acute liver disease. This is extremely fatal and is seldom accompanied by any symptoms. 
  • Human Papillomavirus Vaccine: This is a sexually transmitted virus that causes cervical dysplasia in women and men.  The apt age for both men and women for this vaccination differs. Women who are twenty six years of age or younger and men below or at the age of twenty one are most suitable for this vaccination. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

I am a 19 year old girl and I had sex a month back. We used condoms and to be sure I took i-pill as well .an within 9 days after having sex I got my period .but this month my period has not yet happened. Dear user. To TEST PREGNANCY AT HOME simply add urine to a small piece of soap, if froth forms, or if the soap bubbles up, the test is positive. Mix ¼ cup of tuna juice and ¼ cup of vinegar in a plastic cup. Now pee in a separate cup and then add your urine to this mixture. After a few minutes, if the color changes to green, then you are pregnant. Take care.

How To Maintain Dental Health in Pregnancy?

Pregnancy is a transformation phase in itself -it brings with it changes to almost all body systems. The oral tissues and the teeth are also affected significantly. Extra precaution is required to maintain regular oral health and avoid severe decay and/or gum disease. If not avoided, the dental infections can cause severe systemic infections and require strong antibiotics, x-rays, minor surgeries, and root canal therapy which may not be safe during pregnancy.

Higher levels of estrogen during pregnancy in the blood lead to a lot of visible changes in the oral cavity -higher incidence of tooth decay, gingival and periodontal inflammation, and even minor benign tumours in some cases. The notorious morning sickness plays havoc with the person's oral hygiene habits, thereby further complicating the situation.

Prior to pregnancy: A pre-pregnancy dental check-up to look for gum health and decay would go a long way in a healthy pregnancy, from the dental point of view at least. A thorough scaling and screening for cavities done before pregnancy can help avoid dental visits during the term, other than for routine checkup.

Pregnancy: If that pre-pregnancy visit could not happen, then visiting your dentist should be one of the first things to do as soon as you have confirmed your pregnancy. At this stage, no dental treatment can be done. Any elective procedures (cosmetic, etc.) will have to be done only after delivery. If the dentist identifies no cause for worry, that is great news. However, if there are any causes for concern, like a decay, the non-invasive treatment should be done at the earliest. When you are pregnant, note the following from a dental point of view:

1. Noninvasive treatments like minor fillings can be done
2. Regular scaling and polishing is not a problem
3. Let the dentist know about all the medications you are taking
4. Visit the dentist every 3 to 4 months for a regular check-up
5. Follow good oral hygiene practices including, brushing, rinsing, and flossing
6. Switch to a bland toothpaste in case of severe morning sickness
7. Watch your diet - the teeth forming in the fetus require nutrition through you, so ensure adequate intake of minerals like calcium and potassium
8. Avoid sweets and sticky/chewy foods that can lead to plaque formation

Though dental procedures can be done during the 4th to 6th month, they are best avoided, which can be done with better planning and some minimal care.

After delivery: After the delivery of the baby, please visit your dentist to ensure you have again ensured there is no emergent dental condition requiring attention. Resume your regular dental care after delivery.
With a little planning and extra care, dental health can be managed nicely during pregnancy with minimal to no pain. If you wish to discuss about any specific problem, you can consult a Dentist.

Hi I am Dr. Parul Katiyar. I am a senior IVF specialist in Max Hospital Delhi and in Max Gurgaon. So my interest is basically into preserving fertility and this is what i will be talking of in today’s discussion.

So what is fertility preservation? Fertility preservation is a process of preserving one’s eggs, sperms or any other reproductive tissue, which can be used for child bearing later on in life. It is a very important tool for preserving fertility for individual’s or persons where in child bearing is not likely to happen in near future however they would like to preserve their option of having a biological child in future. People who benefit from fertility preservation are people suffering from cancer because treatment for cancer is toxic to the eggs and sperms and therefore may lead to a permanent damage in one’s reproductive organs. Such people once they are cured would like to have their own biological child and therefore if they undertake fertility preservation before embarking for a cancer therapy, it is very useful for them. It is also helpful for women who want to delay their child bearing because of social reasons, career prospects or because they have not been able to find out the right partner as of now.

Women undergo biological aging and as the age advances the chance or the probability of becoming pregnant with one’s eggs will decline. Therefore, a woman who has frozen her eggs would be able to use those eggs later in life once she is ready to have her own child. People suffering from certain genetic diseases and immune disorders like LUPUS will also benefit from fertility preservation because genetic diseases may lead to a decline in one’s fertility at an accelerated rate and treatment of lupus and other immune disorders may again be toxic to one’s reproductive organs.

How do we preserve fertility? The various tools that are being used to preserve fertility in men are, most importantly, is the sperm cryopreservation or Sperm freezing. In this procedure what happens is that the man will have to ejaculate and produce a sperm sample in a laboratory and that sample is then processed and frozen in liquid nitrogen at -190 degree centigrade and preserved for whatever time they want to keep it. Freezing the sperms or any reproductive tissue has come out in a big way and it is very safe and with very good outcomes whenever those sperm samples or reproductive tissues are thought back and used for fertility.

The other important tool that is used in men is called Gonadal sparing, where in in individuals who need radiation therapy, the radiation is directed in such a manner that the organs that produce the sperm i.e. the testicles are spared in such a manner that there is testicular sparing, and therefore the spermatogenesis is not as badly effected as it would be if we do not take care of that. The tools for preserving fertility in women are first, embryo cryo-preservation; this is one of the most standardized technique for preserving one’s fertility.

In embryo cryopreservation, we inject the women with hormones so that they are able to produce multiple eggs and then those eggs are extracted and then fertilized with husband’s or partner’s sperm and then these embryos are cryo-preserved. However, in women who do not have a partner, they can undergo an oocyte cryopreservation as well. In oocyte cryopreservation, the same process is followed; however, as there is no partner, the eggs are frozen without even fertilization, which has become one of the most successful therapies for women who are undergoing cancer therapy at a young age or women who are not ready to have a child. Another technique that is being used is ovarian cryo-preservation or ovarian tissue cryo- preservation. The purpose of ovarian tissue cryo-preservation is that the entire ovary is taken out and then it is cut into slices and that ovarian cortical l tissue is then frozen in liquid nitrogen so that it can be re-implanted into the body of the women once she is ready to have the baby.

So these are various techniques, it’s an ever evolving field and we will have more and more of fertility preservation techniques that will come and help people conceive at a later stage in life. It is very aware that all of us make ourselves aware. As this is a very interesting field and has got a huge scope for a lot of people, it is very aware that all of us make ourselves aware of such technologies that exists.

These are important facts that patients need to understand, before going in for a hair transplant. Hello lybrate user as you asked about your problems related to your sister and her uterine cyst. I would like to advise you to follow regular daily routines of getting all dietary measures and daily behaviours as well as yoga and exercise every day morning walk before sunrises. She may use hot water sipping boiled with ginger, haldi and manjeeth every hour for whole day. She may follow some regular oil massage over whole body then bath with warm water. She may use green vegetables and moong daal soups fried with cumin seeds and ghee and garlic. Light diets and moong daal soups, khichari and dariya with green vegetables and moong daal soups. Avoid oily or junk food stuff, heavy proteins like mutton, rajma, paneer, sour products e. G. Pickles etc and fried foods, and yogurt. She may follow some regular routines of ayurveda medicines as given below. 1: arogyavardhini vati. 1 tab chandra prabha vati. 2tabs shilajatwadi vati. 2tabs laxamana loh. 2tabs twice daily with warm water after meals. 2: laxamanarist. 15 ml dashmularist. 15 ml twice daily with water after meals. 3: varanadi kasayam. 10 ml saptsaram kasayam. 10 ml twice daily with warm water on empty stomach. Follow regular the above mentioned medicines for three months regular.

It limits foods that are high insaturated fat, such as meats, cheeses, and fried foods. Most women who have pcos can benefit from losing weight. Even losing 5 kg may help get your hormones in balance, regulate your menstrual cycle and can make you ovulate.
