Even doctors are ignorant of genital tb causing infertility

Having the highest number of population aff ected by tuberculosis, india also has the highest incidence of genital tuberculosis. The number of indian women suffering from the condition is particularly high.

Nagpur: having the highest number of population affected by tuberculosis, india also has the highest incidence of genital tuberculosis. The number of indian women suffering from the condition is particularly high. It is among the most common causes of infertility, yet not many doctors consider the possibility of genital tuberculosis while treating infertility.

According to some estimates, around 40% indians are infected with tb bacteria. Most of these are latent cases, wherein the sufferer doesn't show symptoms of the disease but can still spread the disease. Genital tb is found to affect around 1% women in india. Ways to manage the condition were discussed during the conference masterclass infertility organized in the city recently.

"the percentage of infertility in the country increases due to genital tb. Among those seeking treatment for infertility, 10-19% of females are affected by it. However, since the condition doesn't have any noticeable symptoms it remains undetected in most patients and infertility caused by it gets categorized as something else" said senior gynaecologist from the city Dr. Sushma deshmukh. She added that the lack of symptoms means that doctors often don't address genital tb as a reason causing infertility.

"the biggest problem is that very few bacteria can cause the disease. This low number makes most tests made to diagnose the disease unsuccessful. This is why it is recommended that bacteria culture, a molecular biology test for ccr and histopathology on the lining of the uterus must be done together" said bangalore-based gynaecologist Dr. Reeta biliangadi. The positive outcome of all these tests, along with observation of clinical signs indicates that the patient is suffering from genital tb.

Among the warning signs of the condition that doctors and the patients must look for include menstrual problems, constant pain in lower abdomen and white discharge" the condition generally flares up during menstruation in the adolescent age. If detected at this time, the damage to the reproductive organs like uterus and fallopian tubes can be minimized. If not, the damage can worsen with time, making it practically impossible for the woman to conceive naturally" said Dr. Biliangadi.

As the fallopian tubes get affected in all cases of the disease, chances of natural conception among the patients are less than 20%, added Dr. Deshmukh.

"despite the availability of tests to detect genital tb, less than 40% patients with it get the chance to go through the tests" she said.


Affect on reproductive health of patient

It starts by affecting the process of releasing the egg into the uterus, which also leads to menstruation

It destroys endometrium or the lining of the uterus where eggs get implanted. This makes natural conception difficult

Leads to obstruction of fallopian tubes, or gradual destruction of the tubes

Increases the possibility of diseases like fibroids.

lybrate-user, Thanks for the query. In hypothyroidism following foods, vegetables are supposed to be avoided: Soy beans, soy containing products. Vegetables to be avoided or taken very sparingly include cabbage, cauliflower, broccoli, onions, spinach, radish & mustard. Plus peanuts and coffee. However, most important is to avoid soy beans and soy containing products. Others (mentioned above) can be taken in limited quantity. These foods are supposed to be affecting thyroxine action. Thanks One should consume a good deal of green leafy vegetables, salads and fruits. Use whole grain flour for chapati and roti. It is important to know what is TSH value and not T3. Because in hypothyroidism there is T4 deficiency leading to increase in TSH value above 10 mU/L.Thanks.

Mr. lybrate-user, Thanks for the query. Dono dawaye aap khane ke (breakfast) pahale le sakate hai. But please tell me following details: FBG, PP blood glucose levels, HbA1c%, dietary pattern, extent of daily exercise, BP readings and lipid profile details. Because unless I am aware of your blood glucose level, advising on drugs dosage is not really possible. Thanks.

lybrate-user, Thanks for the query. The products mentioned are having mainly carbohydrates and considerable amount of sugar, hence not advisable for a person with diabetes. There are some protein powders available specially made for diabetes patients, those can be considered. However, first and foremeost is achieve a strict blood glucose control with fasting glucose < 100 mg, PP 160 to 170 mg & HbA1c% <7. With that kind of control patient feels fairly energetic. Plus diet should include good deal of green leafy vegetables and fruits. Thanks.

I were in a job where I hv to stand for 6 hour but from past 2 year I'm not doing that job but still whenever I'm walking or playing it starts again. Dear lybrateuser, - Your problem could be due to a bony growth in the heel known as spur - avoid standing for long, wear a sport shoe with proper fitting & cushion sole for proper padding effect while walking - dip foot in warm water 2-3 times a day for 10 min - do x- ray foot to know the exact cause, consult a doctor.

It is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (self-monitoring of blood glucose? smbg). Though difficult, this can be done by a glucometer and keep a record. Get an idea of fasting sugar (fbs) and 2 hours after food (ppbs) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ insulin will be needed. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me privately with all details like your daily eating pattern and medicines you take. I assure you a proper guidance. If you keep in touch. most men have occasionally experienced some difficulty with their penis becoming hard or staying firm, but this is not erectile dysfunction, which is a concern only if satisfactory sexual performance has been impossible on a persistent number of occasions for some time. Broadly, two forms of erectile dysfunction can affect men's sex lives - the main cause is either medical or psychosocial. Here, psychosocial means psychological effects - including the effects of social relationships - on sexual performance. Of course, medical conditions can also affect psychological well being, and while the main cause of sexual impotence will be medical or psychological, there can be overlap between the two. Erectile dysfunction used to be known more widely as impotence' before the causes became better understood and successful treatments came into use. Impotence is a term that is still in use, however.

Ayurveda provides a holistic way of treatment. One should address other symptoms along with hypertension. Doctor should suggest diet lifestyle modifications if needed to a patient. This will eliminate cause for the disease. Further, Ayurveda advocates a personalized treatment. Though Mukta Vati is a formulation designed for the most common condition, it will not equally effective for all the patients. It may be more useful in certain disease conditions and in certain patients. Patient should leave choice of drug and overall treatment on Ayurveda expert. Otherwise if did not get result from Mukta vati you will start blaming Ayurveda. Accept Ayurveda to its fullest, not partially; if you expect sustainable results.

You should get ultrasound of pelvis done to r/o polycystic ovaries. Nothing to worry in hypothyroidism. Once thyroid levels are controlled you should conceive.

First thing is be regular with ur meds. Dont be lazy. If ur back on ur thyroid meds , then dont blame the thyroid for everything. Take meds regularly, have a low carbohydrate and low fat diet. Dont be lazy, exercise for a minimum of 1 hr/day.Get ur vitamin d tested

There is no special diet for hypothyroidism, but certain foods should be avoided or taken sparingly. Soya beans and soya containing products are to be avoided along with vegetables like cabbage, cauliflower, broccoli, mustard and coffee. Other vegetables can be taken, plus whole grain flours and breads made out of that are good. Iodinated salt should be used. Since you also have high bp salt intake also should be less. Soya does not cause the condition but it can aggravate it. Plus prescribed medication must be taken without a break.
