Brain Stroke - Factors That Can Lead To It!

A stroke, also called cerebrovascular accident (CVA), cerebrovascular insult(CVI), or brain attack, occurs when a part of the brain is deprived of blood flow. When the brain cells are deprived of oxygen they begin to die. When brain cells die, the functions controlled by that part of the brain also stops, which results in different types of disabilities among stroke survivors.

There are two types of strokes

  • Hemorrhagic
  • Ischemic stroke

The first is caused when a brain aneurysm or a weak blood vessel bursts. Most of the time, this type of stroke leads to death. The second one happens when a clot blocks the flow of blood to the brain. Patients suffering from stroke suffer from various side-effects, the most common ones being paralysis or loss of feeling in a certain part of the body, problem in understanding or talking and loss of vision in one side. The side-effects start showing up regularly after a person has had a stroke.

In certain conditions, blood flow to a certain part of the brain stops for only some time and hence the body suffers stroke like symptoms which only last a couple of hours before disappearing. This is known as a transient ischemic attack (TIA). Unfortunately, the effects of a stroke can be debilitating and also permanent. Hence its important to know the symptoms of a stroke and rush the patient to a doctor as soon as possible. Sometimes early treatment can save a lot of damage.

The primary symptoms of stroke are as follows:

  1. Confusion and problems with talking and comprehension
  2. Headache along with alteration of consciousness or vomiting
  3. Numbness of the face, arms or legs, especially on one side of the body
  4. Issue with seeing, in one or both eyes
  5. Inability to walk with stability, including disrupted coordination
  6. Problems with the bladder and bowel control
  7. Acute depression
  8. Body temperature fluctuates, and pain worsens with movement
  9. Paralysis on one side of the body along with fatigue
  10. Problem in expressing or controlling emotions

Risk Factors:

Diagnosis of stroke

Several tests are carried out to determine the type of stroke acquired. They are:

  1. Physical examination, which involves observing the patient's overall condition.
  2. Blood tests
  3. CT scans
  4. MRI scans
  5. Cerebral angiogram
  6. Echocardiograms

Epilepsy - How Seizures Affect It?

Epilepsy is a disorder of the central nervous system, thus it is basically a neurological disorder. In this the nerve cell activity of the brain is disrupted and causes seizures along with episodes of unusual behavior along with the loss of consciousness. The symptoms of the seizure can vary and some people with epilepsy just stare blankly for few seconds when a seizure takes place, while some twitch their arms and legs repeatedly.

Symptoms of Epilepsy:
As epilepsy is caused by the abnormal activities in the brain cells, seizures affect the processes with which the brain coordinates. Some signs and symptoms are:

  1. Staring blankly
  2. Confusion which is temporary
  3. Uncontrolled jerking movements of the legs and arms
  4. Loss of awareness and consciousness
  5. Psychic symptoms

Mostly the symptoms vary depending on the type of the seizure. The seizures are further classified into focal and generalized depending on which part of the brain shows abnormal activity.

Focal seizures
When seizures are due to the abnormal activity of the cells in just one area then it is known as focal seizures. These seizures are of two types:

  1. Simple partial seizures or the focal seizures without loss of consciousness. These seizures basically alter the emotions and the things like change in smell, look, sound and taste. These also result in the involuntary jerking of the body like the legs and arms, with spontaneous sensory symptoms like the tingling and flashing of the lights.
  2. Complex partial seizures or the focal dyscognitive seizures. These seizures have a loss of awareness and consciousness. One might just stare blankly into space in this seizure and performs repetitive activities like swallowing, chewing, walking in circles and chewing.

Generalized seizures
There are six types of generalized seizures. These are:

  1. Absence seizures, also known as petit mal seizures, are commonly seen in children and are characterized by subtle body movements like lip smacking and eye blinking and staring blankly into space. These also cause loss of consciousness.
  2. Tonic seizures lead to stiffening of the muscles. These affect the muscles of the legs and arms and also cause to fall to the ground.
  3. Atonic seizures are also called as drop seizures. These cause loss of control in the muscles, thereby leading to collapse or fall.
  4. Clonic seizures are the rhythmic and jerking muscle movements. These affect the arms, face and neck.
  5. Myoclonic seizures appear as twitches in the arms and legs.
  6. Tonic-clonic seizures also known as grand mal seizures are very dramatic and lead to body stiffening, abrupt loss of consciousness, shaking and loss of bladder control and tongue biting. If you wish to discuss about any specific problem, you can consult a neurologist.

10 days rest is not enough. Take rest for 4 weeks for improvement. If pain persists, get an MRI done.

Getting hallucinations is a mental condition where a person sees, feels, hears, and tastes things that actually don't exist beyond one's heightened imagination or delusion. It involves the experience of perceiving something not present. Hallucinations can be pleasing or frightening. However, there is almost always an identifiable cause behind it. 

They can be triggered by:

  1. Taking hallucinogenic or psychotropic substances
  2. Mental conditions like dementia and schizophrenia
  3. Neurological conditions like Alzheimer’s disease or Parkinson's disease
  4. Macular degeneration, leading to loss of vision
  5. Migraines and brain tumor can also lead to such delusions

Some of the signs of hallucinations:

  1. Hearing voices: The medical term for hearing voices is called an 'auditory hallucination'. A person may sense sounds or noises coming from inside or outside of their mind. The noise might be random or disrupting. One might also feel the voices talking to each other or trying to tell them something. Most of the times, these voices come from inside the person's mind; or in some cases, one's heightened perception may make a normal noise delusional.
  2. Seeing things: This is also called visual hallucinations. For instance, one may see unnatural things like a floating chair in thin air. It all depends on a person's perception. Sometimes these hallucinations appear as bright flashy spots or rays of light.
  3. False sense of taste and smell: Technically, these are known as gustatory and olfactory hallucinations respectively. One may feel a kind of odor coming from one's body or surrounding; or a person might feel that something he/she is drinking or eating has an odd taste. This is again too much thinking, causing delusional sensory activities. 
  4. Tactile hallucinations: This is when a person feels things that don't exist. One may feel that he/she is being touched or tickled even when no one else is around or that insects are crawling beneath the skin. Many scientists have shown that the size of the brain shrinks as we age and this does not have any visible effect on our cognitive abilities.
  2. It’s all relative: The size of the brain does not really have a bearing on the way a person’s cognition gets shaped. Even large mammals like elephants and whales are finally hunted and tamed by humans who have smaller brains in comparison. The brain is made up of billions of neurons, which need to function properly. It may be seen that scientists consider the brain mass in relation with the rest of the body so as to speculate about the cognitive abilities of the person. Why is this required? Large animals need a well functioning and proportionate brain size to control and run their organs with proper cognition for satisfactory results, which is what we humans seem to have done.
  3. Neanderthal brain: Historically, the earliest man or the Neanderthals are said to have had larger brains than we do. These people are believed to have brains that are at least 10% larger than the brains that we have in our modern times. The shape of their brain was different too. They were also heavily muscled people which had a bearing on the size and shape of the brain and bodies as well as the lean tissue within the brain. They also survived very successfully for a period of over 200,000 years, which obviously points to some form of elevated cognition, as per many scientists.
  4. Animals: While animals with small brains like lizards and reptiles do not perform too well on IQ tests, the animals with bigger brains like elephants and dolphins perform much better. But the medium sized brain of monkeys, lemurs and other animals are said to perform in the best manner. The correlation between the body size and the brain does not seem to hold good here, as per various researches.

Can the Immune System Trigger Alzheimer's Disease

Alzheimer's Disease is a neurological problem that is characterised by a cognitive decline and memory loss. It is a type of neurodegenerative dementia. The symptoms of Alzheimer's Disease include not being able to absorb and retain new information, lack of reasoning and judging, not being able to take on complex tasks, impaired visuospatial abilities, problems in reading, writing and speech, among many others. If a person has at least two of these symptoms in a debilitating manner, then the diagnosis can be made in favour of Alzheimer's Disease. The main causes of Alzheimer's are shrinkage of the brain size and death of the brain cells. The immune system is also said to trigger this neurodegenerative disease.

Let us find out what medical science has found so far:

  1. Connections: Many a times, in Alzheimer's disease, the memory and behaviour of the person changes because the brain is unable to make proper neural connections which can lead to memory loss of how a person behaved and the elements that formed the basis of the patient's cognition. Apparently, the immune system behaves in the same way within the brain and blocks the connection. This happens because there is constant communication between the brain and immune system along neurological lines, which is where the disease first emanates.
  2. Inflammation: The brain is prone to inflammation or swelling that is not the normal kind. This inflammation happens as a result of the activation of the infection fighting neurotransmitters and the chemical changes that happen in the brain when an infection strikes. The inflammation usually happens in the plaques or clumps which the brain tries to protect. These clumps are made up of a protein called Amyloid. The immune system is responsible for creating this inflammation in the brain of the patient.
  3. Pattern Recognition Receptors: Many of these receptors work in different manners and cooperate with each other to create a response in the brain. These PRRs can be found in the brain plaques, and they develop the signs of danger which further fuels the inflammation in the brain as a matter of protection.
  4. Activation of Cells Linked with the Immune System: When the PRRs begin to respond, it basically activates the immune system and the cells of the same. This is the basic reaction that causes the changes in brain which then leads to the attachment of the protein to the tissue that is diseased, in which case Alzheimer's Disease starts. The inflammation that we had spoken about earlier basically happens in the nervous tissue.

It is important to recognise and act on the initial signs of Alzheimer's Disease before it progresses beyond one's control.

This is regarding my brother in law. He has been experiencing seizures since childhood whenever he has a lack of sleep the previous night. Recently we got his EEG, MRI (FOR EPILEPSY PROTOCOL) and other blood tests done which was referred by Dr., Mumbai. All the tests were normal and the doctor prescribed Valprol CR 200 thrice daily for 2 years and also said that if the medicines are taken on time for the said period then the condition would be cured forever. So could you please confirm me is it true if the medications are taken on time then he would lead a seizure free life forever? Please let me know.

Your brother might be suffering from idiopathic generalized epilespy. If he has additional myoclonus then it can jme. Valproate is a good choice for such epilepsy. 2 years of seizure control is required but it is not necessary that after that he will be seizure free for life. Because sometimes seizures recur if the precipitating factors are not taken care of. Like sleep deprivation, stress, fasting, etc.

I have the problem of fits (epilepsy) since 2008. It happens one or two times in a year. As it gives no symptoms like headache etc. Before it. Why this is happened? I took treatment in two hospitals. All the reports are normal. Doctor suggested to take tablets eptoin 100 mg twice a daily upto lifetime. Is it have any side effects?

In epilepsy, your reports can be normal when it is called idiopathic epilepsy. Lf the seizures are not controlled with eptoin you require increase in dose or change in medicines. The side effects of eptoin are giddiness, hair growth, sedation, tremors, bone loss due to vit d def, anemia, etc.

Please let me know if a person who had meningitis many years before will still need to fear of any side effects? Also please advice me if a girl who had meningitis during childhood will develop any complications during pregnancy or delivery? Whether those who had meningitis are good for married life?

If the meningitis is cured and there is no problem currently, there is nothing to fear. No problems occur in marriage, pregnancy or delivery.

Age 75. He got paralytic attack 3 days before in left side. Because of immediate treatment he survived. Can not stand but has movement in all organs. He has high b. P. Sugar is on little higher side but controlled. Please advise how to care also suggest diet according to his condition. He is very week. thanks all.

He has to undergo physiotherapy and get his bp and sugars checked regularly. Diet should be balanced with more protiens and less fat specially foods containing oil, butter and sugars. Depending on the type of stroke, he may require blood thinners.

Sir I wanted to know what are the symptoms of moto neuro disease. What are the treatment available for it.

Symptoms range from weakness of limbs, slurring of speech, difficulty in swallowing, flickering movements over skin known as fasciculations. Currently treatment is physiotherapy and a medicine called riluzole.

I am diagnosed with Epilepsy and its been 3 years that I am on medication. I am taking Epilex Chrono (600 mg) in morning and same at night with 100 mg of Lamictal. Though my EEG report is normal now so just wanted to know how long treatment will take and I have to take medicines.

In case of epilepsy since you are on meds for 3 years, if the seizures are completely controlled and eeg is normal, a trial of gradual taper of medicines can be given. But always taper medicines after consulting your neurologist. And there is always a chance of recurrence so that risk has to be borne in mind.

I recently read that researchers believe certain foods might cure Alzheimer’s disease. Is this true? If so, which foods do this and how much would you have to consume to get the benefits?

As a neurologist, I can tell no food can cure alzheimers. It can be managed and progession can be slowed with medicines.
