Hi. First of all you have to keep in mind your father have 2 risk factors. Bp n dm. Which can affect any organ including brain, heart, eyes, nerves, kidney etc. Now you should search and treat for other risk factors like, obesity, dyslipidemia etc. Our target bp is to keep around 130/80 if having diastolic dysfunction. So physician will go on increasing medicine till target achieved, slowly over 6 to 8 weeks. And then watch for effect and will then titrate doses. No need to monitor that much frequently in a day. U can do once. And if when feels symptomatic like headache, giddines etc. Keep watch on creat trends. 2 to 3 monthly. Also do urine test to watch for protein loss. Diet. Is according to diabetes. To prevent further increase in sugar levels. Have freuent meals, include more of protein, fish is good, vegetables, dry fruits, low fat, low carbs, dairy product. If potassium increased in kft test then avoid potassium reach fruits. U can take one fruit depend on your sugar level.
