Diagnosis of NTM lung disease requires the clinician to integrate clinical, radiographic, and microbiological data, but, ultimately, diagnosis can be confirmed by (1) at least two positive cultures from sputum, (2) one positive culture in the case of bronchoscopic wash or lavage, or a transbronchial or other lung biopsy with a positive culture for NTM or compatible histopathological features such as granulomatous inflammation or stainable acid-fast bacilli (AFB), and one positive sputum or bronchial wash culture for NTM regardless of the mycobacterial strain

Yes montoux test will come positive for a very long time. Since you are having sputum positive pulmonary tuberculosis I would advice you to get your sputum checked for afb and gram stain, until you get a negative result you need to take precautions like wearing a mask because people around you may catch tuberculosis from you. I would suggest you to continue your att till your treating doctor stops it, and eat high protein diet along with att.

Hello, mantoux test is not worth doing now. Sputum for afb may not be positive always, for sputum test to be positive there should be at least 10 to the power 4 organisms to get detected. So we need to go along with ct scan. If you are from metropolitan city you can try to get gene expert test on sputum which is more sensitive than microscopy and gives result in 2 hrs. Other is sputum culture and sputum culture for tuberculosis. Sputum for gram stain we need to give antibiotics and see for response.
